Objective: This study aimed to assess the long-term effects of anodal tDCS on upper limb motor recovery, hand dexterity, and quality of life in stroke patients, while exploring the association between motor recovery and stroke severity. Methods: Sixty-seven stroke patients were randomized into control and experimental groups using sealed envelope randomization. The experimental group received 20 minutes of 2-mA anodal tDCS targeting the M1 area, three times per week for eight weeks, alongside task-oriented training. Outcome measures included the Fugl-Meyer Assessment, Action Research Arm Test, and Stroke-Specific Quality of Life Scale, assessed at baseline, weeks 4, 8, and 20. Result: Within-group analysis demonstrated significant improvements in upper limb motor recovery (p < 0.05), hand dexterity, and quality of life in all groups, with the experimental group showing superior outcomes compared to controls. Notable improvements were observed in motor recovery across mild, moderate, and severe stroke severities, with significant effects persisting through follow-up. Conclusion: Anodal tDCS produced substantial short- and long-term benefits on motor recovery and hand dexterity but exhibited no significant effects on quality of life in the long term. INTRODUCTION Stroke is a life-threatening condition due to abrupt compromise in cerebral perfusion or vasculature that causes focal neurological signs which persist for 24 hours or more ( 1 , 2 ). Globally, one in every six people will experience a stroke in their lifetime, with approximately 13.7 million people suffering from stroke annually, resulting in 5.8 million deaths ( 3 ). The stroke burden in developing countries is particularly challenging, accounting for 75.2% of stroke-related deaths and 81% of Disability-Adjusted Life Years (DALYs) associated with stroke ( 4 , 5 ) with a prevalence of stroke at 1200 per 100,000 population ( 6 ). Individuals often experience impairments in motor control, sensation, coordination, and strength in the affected upper limb, significantly hindering their ability to perform daily activities such as writing, eating, and dressing ( 7 ). Rehabilitation strategies like Task-Oriented Training (TOT) has shown significant effects on improving motor function, activities of daily living, and quality of life (QoL) for stroke survivors. ( 8 ) Transcranial Direct Current Stimulation (tDCS) is a non-invasive technique that uses low electrical currents to modulate neuronal activity, enhancing motor recovery, cognition, hand dexterity, and QoL in stroke patients ( 9 ). Transcranial direct current stimulation (tDCS) has emerged as a promising neuromodulatory technique to enhance motor recovery post-stroke. Conventional tDCS protocols typically involve unilateral stimulation, either applying anodal tDCS to the ipsilesional hemisphere to facilitate excitability or cathodal tDCS to the contralesional hemisphere to reduce excessive inhibition. However, growing evidence suggests that a dual-hemispheric tDCS approach simultaneously applying anodal stimulation over the lesioned hemisphere and cathodal stimulation over the intact hemisphere may be more effective in restoring interhemispheric balance and promoting functional recovery. ( 10 ). The interhemispheric inhibition model provides a theoretical framework for understanding how dual-hemispheric tDCS can optimize poststroke rehabilitation. By modulating both hemispheres concurrently, this approach aims to enhance neural plasticity, improve motor performance, and accelerate functional gains. Furthermore, when combined with TOT, dual-hemispheric tDCS may maximize motor learning by reinforcing neural circuits involved in movement execution and coordination. ( 11 ). Our study utilizes dual-hemispheric tDCS, a novel stimulation approach targeting the interhemispheric inhibition model to optimize stroke rehabilitation outcomes. By simultaneously enhancing activity in the lesioned hemisphere and suppressing hyperactivity in the contra lesional hemisphere, dual-hemispheric tDCS aims to restore interhemispheric balance and facilitate neuroplasticity. While previous studies have largely classified stroke patients based on time post-stroke (acute, subacute, chronic), this approach overlooks the heterogeneity in stroke severity and its impact on treatment response. ( 11 – 14 ) This study investigates the effects of dual-hemispheric tDCS combined with TOT on upper limb motor recovery and quality of life in stroke survivors. By targeting the interhemispheric inhibition model, we aim to assess the efficacy of this approach in enhancing motor function and dexterity across different stroke severities. The findings will contribute to the refinement of neuromodulation-based rehabilitation strategies and provide insights into optimizing tDCS protocols for stroke recovery. 2. | METHODS 2.1. | Ethics committee approval statement The institutional research ethics committee of Riphah International University with ref. no (RIPHAH/RCRAHS-ISB/ REC/LETTER-01900) before the experiment was started and that has been conducted in accordance with the principles set forth in the Helsinki Declaration has approved the study 2.2. | Study setting & subjects 2.2.1. Clinical trials A randomized control trial (RCT) was conducted in Pakistan Railway Hospital, Rawalpindi, Pakistan, from June 2024 to January 2025 with ethical approval and registered at clinicaltrial.gov (NCT06454331), first registration was made on 12/06/2024. Written informed consent was obtained from all participants prior to their inclusion in the study. All participants were randomly allocated into experimental and control groups using the sealed envelope method. A stratified randomization strategy based on NIHSS stroke severity classified participants into mild, moderate, moderate-to-severe, and severe groups using a 2×4 factorial design. Sealed envelopes contained distinct randomization assignments, ensuring balanced group distribution. The envelopes were signed, sequentially numbered, and remained sealed until the patient gave their consent to participate in the study. The envelope was then opened, and the patient was allocated accordingly to either the experimental or control group. Participants were recruited through non-probability convenience sampling, including individuals aged 40–80 years with stroke duration > 6 months. Stroke severity was assessed using NIHSS. Exclusion criteria included upper limb amputation, joint/functional limitations, neurological/musculoskeletal/cardiopulmonary disorders, metallic implants, epilepsy history, skin allergies, shunts, or open skull wounds. The sample size was calculated using G*Power 3.1.9.2 with an alpha of 0.05, power of 0.80, and effect size of 0.2, requiring 56 participants. Adjusting for 20% attrition, the final sample size was 67. A total of 80 participants were included in the study, 10 participants to each severity level (mild, moderate, moderate to severe and severe) within both the experimental and control groups, ensuring balanced representation across the groups (Figure.1). 2.2.2. | Intervention The protocol applies anodal tDCS to the ipsilesional primary motor cortex (M1), with the cathode placed on the contralesional M1, located in the precentral gyrus, was identified using the 10–20 EEG System. To locate M1, the nasion-inion distance along the midline is measured—typically 30–37 cm in adults—and the midpoint is marked. From this midpoint, 10% and 20% of the nasion-inion distance is measured laterally to the left (C3) and right (C4) to precisely identify M1. The scalp at these points is cleaned to ensure optimal electrode contact for stimulation or EEG recording ( 15 ). The dosage of anodal tDCS will be 20 minutes with an intensity of 2mA. It's administered before exercise therapy sessions over a period of 8 weeks and 3 times a week ( 16 ). The intervention for experimental group, included active anodal transcranial Direct Current Stimulation (tDCS) and control group included sham tDCS administered ( 17 ) the tDCS was applied with intensity of 2mA, 0.02mA/cm2 density for eight weeks on alternate days. Experimental group In the experimental group, participants receive a 20-minute tDCS treatment followed by 30 minutes of Task-Oriented Training (TOT), ( 18 ). In accordance with the 10–20 EEG protocol, electrodes were positioned at the M1 area (C3 and C4), with anodes over the ipsilateral M1 and cathodes over the contralateral M1. A current stimulator powered by a battery will be used to deliver tDCS. Following safety restrictions, stimulation parameters were set at 2 mA bicephalic with a relative current density of 0.02 mA/cm² for 20 minutes. ( 16 ) Control group In the control group, participants undergo a 20-minute sham tDCS treatment with randomization of treatment order. The sham stimulation follows the same electrode placement at the M1 area (C3 and C4) based on the 10–20 EEG system, with electrodes positioned similarly to the active tDCS group (anodes over the ipsilateral M1 and cathodes over the contralateral M1). Sham tDCS is delivered using a battery-driven current stimulator. Stimulation parameters mimic those of active tDCS but without actual current flow, ensuring blinding of participants. The procedure adheres to safety standards with no actual stimulation effect. ( 16 ) 2.2.3. | Outcome measures All outcome measures were assessed by professional rehabilitation assessors blinded to the randomized controlled trial (RCT) status. Assessments were conducted at baseline, 4th, 8th and 20th week. The primary outcome included, the Fugl-Meyer Assessment (FMA), was used to measure upper limb motor recovery. Secondary outcomes included the Action Research Arm Test (ARAT) for hand dexterity and the Stroke-Specific Quality of Life (SS-QoL) scale for quality of life. 2.2.3.1 Fugl-Meyer Assessment (FMA) The Fugl-Meyer Assessment for Upper Extremity (FMA-UE) is a clinical tool for evaluating motor recovery in stroke and neurological conditions affecting the upper limbs and with a sensitivity of 77%, a specificity of 89% ( 19 ). It assesses motor function through tasks like reaching and grasping, scored on a 3-point scale. Higher scores indicate better function, with a maximum score of 66. Clinicians use it to track progress, plan treatment, and assess outcomes in rehabilitation settings ( 20 ). 2.2.3.2 Action Research Arm Test (ARAT) The Action Research Arm Test (ARAT) is a clinical assessment tool used to evaluate hand dexterity and upper limb function in individuals who have experienced a stroke. The ARAT measures the ability to reach and grasp objects, providing valuable insights into motor recovery and functional ability. It is sensitive to changes in upper limb motor function and is commonly used in stroke rehabilitation research. The ARAT helps clinicians assess progress in hand function and guides the modification of rehabilitation interventions to improve functional outcomes for stroke patients ( 21 ) 2.2.3.3 Stroke Specific Quality of Life (SS-QOL) The of Stroke Specific Quality of Life (SS-QOL) questionnaire is a tool used to assess the quality of life in individuals who have experienced a stroke. The SS-QOL reveals a sensitivity of 70.0% and a specificity of 75.8% ( 22 ). It measures various domains including physical function, mobility, social participation, emotional well-being, and cognition. The SS-QOL helps clinicians and researchers understand the impact of stroke on a person's overall quality of life and modify interventions accordingly.( 23 ) 2.3. | Statistical analysis All data were analysed using IBM SPSS version 25 following a per protocol (PP) analysis approach. The Kolmogorov-Smirnov test revealed that the data for FMA, ARAT, and SS-QOL were not normally distributed. Nonparametric tests, specifically the Mann-Whitney U test for between-group comparisons and the Wilcoxon signed-rank test for within-group comparisons, were applied. Mixed ANOVA was used to assess overall interaction effects, while regression analysis was conducted to evaluate associations. 3. | RESULTS The overall mean age of participants was 56.27 ± 7.08 years, with the experimental group 57.70 ± 7.12 years and the control group 55.20 ± 6.95 years. The mean stroke duration was 8.73 ± 3.09 months, with experimental group 10.02 ± 3.20 months in contrast to control group 7.76 ± 2.66 months ( Table 1 ). Significant improvements in FMA scores were observed across all stroke severity groups. In mild strokes, significant effects were seen at weeks 4, 8, and 20 with p value of 0.048, 0.043, and 0.039, respectively. In moderate, moderate-to-severe, and severe strokes, significant effects emerged at weeks 8 and 20 with p-values ranging from 0.049 to 0.029) ( Table 2 ). Notably, the observed improvements in FMA scores exceeded the minimally clinically important difference (MCID) of 5.25 points for upper limb function post-stroke, confirming the clinical relevance of these changes. The ARAT demonstrated a similar pattern, with significant improvements in mild stroke cases at weeks 4, 8, and 20 (p value of 0.029, 0.039, and 0.031, respectively). In other severity categories, significant effects were noted at weeks 8 and 20 (p-values ranging from 0.031 to 0.019) ( Table 3 ). Improvements in ARAT scores met or exceeded the MCID of 5.7 points, indicating meaningful functional recovery in upper limb dexterity. Significant improvements in SS-QOL scores were observed across different stroke severities. In moderate strokes, significant effects were noted at weeks 4, 8, and 20, with p-values of 0.028, 0.042, and 0.039, respectively. In mild, moderate-to-severe, and severe strokes, improvements became significant at weeks 8 and 20, with p-values ranging between 0.047 and 0.031 ( Table 4 ). The SS-QOL improvements exceeded the MCID threshold of 5.0 points, suggesting these changes were not only statistically significant but also clinically meaningful. Based on the comparison with MCID values, the results indicate that tDCS and task-oriented training led to clinically meaningful improvements in motor function, hand dexterity, and quality of life. The improvements in FMA, ARAT, and SS-QOL scores exceeded their respective MCID thresholds, confirming that the observed changes are not only statistically significant but also clinically relevant. These findings support the efficacy of tDCS and task-oriented training in stroke rehabilitation, showing that the intervention leads to meaningful functional recovery The experimental group showed statistical significant results for motor recovery and QOL in all severities of stroke at almost all timepoint. Additionally, hand dexterity also showed improvement among the experimental group in moderate and moderate to severe stroke samples ( Table 5 , 6 , 7 ) . Regression analysis across weeks 4, 8, and 20 found no significant relationship between stroke severity and motor recovery (p > 0.05 at all time points), suggesting stroke severity alone did not predict recovery outcomes ( Table 8 ) . and QOL achieved by integrating tDCS with TOT in stroke survivors. These findings align with previous research that suggests a synergistic effect between tDCS and task-specific training, which enhances cortical excitability and motor learning. The similar results by combining tDCS with motor training to enhance recovery, particularly in motor function by modulating interhemispheric imbalance and TOT to reinforce the functional relevance of motor skills proved especially effective, particularly in improving upper limb function and overall QOL, even in chronic stroke patients ( 24 , 25 ). The additive benefits of tDCS when paired with motor training. further validates the therapeutic potential of this combined approach across various stroke severities strengthens the robustness of the intervention as a viable treatment for stroke rehabilitation. ( 26 , 27 ). The observed improvement in hand dexterity within the intervention group underscores the importance of targeted interventions to enhance functional independence in stroke survivors which is consistent with Wan-wen Liao et al. (2020) study, demonstrated that combining tDCS with motor training significantly enhances hand dexterity, grip strength, and fine motor skills ( 28 ). Kashoo et al. (2022) also reported that using tDCS alongside motor imagery training improved the precision and coordination of hand movements. These results collectively emphasize the potential of tDCS-based interventions in targeting and improving upper-limb motor recovery, specifically fine motor skills ( 29 ). Further studies highlight that the combination of tDCS and TOT not only enhances gross motor function but also facilitates fine motor recovery, which directly correlates with better functional independence and QOL ( 30 , 31 ). Additionally, the observed improvement in QoL for the intervention group highlights the broader impact of combining tDCS with TOT. Enhanced QoL is crucial for promoting functional independence and reintegrating patients into their daily lives. These findings are consistent with Kang et al. (2016), reported that combining tDCS with motor training significantly improved physical and psychological well-being in stroke patients ( 32 ). Navarro-Lopez et al. (2021) also found that stroke patients who underwent tDCS with task-specific training reported higher QoL scores, particularly in areas such as mobility, self-care, and social participation. These improvements reflect the role of neurorehabilitation in not only enhancing motor function but also addressing the psychosocial aspects of recovery ( 33 ). Our findings support the connection between motor function improvements and QoL, showing that better hand dexterity and increased ability to perform activities of daily living (ADLs) correlate with enhanced QoL. María Antonia et al. (2019) emphasized the link between motor recovery and autonomy, which influences the perceived QoL. Combining tDCS with TOT offers a comprehensive approach to rehabilitation by improving both physical and cognitive domains ( 34 ). The outcomes of current study revealed varying levels of improvement based on stroke severity, highlighting the differential impact of the intervention. Mild stroke survivors exhibited near-normal motor function with significant improvements in fine motor skills, hand dexterity, and functional independence. These results align with Kang et al. (2016), who found that individuals with mild stroke benefit most from combined tDCS and motor training interventions, as they have preserved neural pathways and greater neuroplastic potential. Such improvements are crucial for reintegrating these patients into daily life and restoring independence in social and occupational roles ( 32 ). For moderate stroke survivors, the intervention resulted in meaningful gains in motor recovery and daily functioning, consistent with findings by Satoshi Katai et al. (2023) on cortical reorganization and motor relearning in moderate cases ( 35 ). However, in severe stroke cases, the progress was more limited, reflecting the challenges of extensive neural damage and reduced neuroplastic potential. Similar conclusions were drawn by Fatemeh Ehsani et al. (2025), who noted that while tDCS can facilitate improvements in severe stroke cases, the gains are often smaller compared to those in mild or moderate cases. These results underscore the importance of tailoring rehabilitation interventions to stroke severity, maximizing recovery potential, and ensuring functional outcomes for all patients ( 36 ). Strengths include severity stratification and comprehensive measures, though limitations such as small sample size and short follow-up suggest further research is needed to optimize tDCS parameters and assess long-term effects. 5. | CONCLUSION This study highlights the effectiveness of combining tDCS and TOT in improving motor recovery, hand dexterity, and QOL in stroke survivors, with outcomes varying by severity. Findings align with prior research but emphasize the need for tailored interventions, especially in severe strokes. Overall, the study supports tDCS and TOT as a promising approach for personalized stroke rehabilitation.
