Study Design: Descriptive cross-sectional study. Methods The study involved four AI chatbots (ChatGPT, Gemini, Claude, Copilot) and four groups of dental professionals (pediatric dentists, general dentists, pediatric dentistry PhD students, and fifth-year dental students). Participants answered 23 fluoride-related questions based on IAPD, AAPD, EAPD, and ADA guidelines. Chatbots were tested 28 times per question under identical settings. Results Significant differences were observed in the accuracy of chatbot responses across fluoride application categories. Claude and Gemini outperformed ChatGPT and Copilot, particularly in systemic fluoride and fluorosis-related topics. Among professionals, pediatric dentists consistently had the highest accuracy. Statistics: Chi-square and Fisher’s Exact tests were used to assess differences in response accuracy between groups. A p-value < 0.05 was considered statistically significant. Conclusions Claude and Gemini demonstrated greater reliability in fluoride-related questions than ChatGPT and Copilot. However, expert oversight remains crucial in pediatric dental care. Pediatric dentistry fluoride artificial intelligence chatbots accuracy Introduction Fluoride has been used in dentistry for over a century and is now considered the key factor behind the significant global reduction in dental caries (Buzalaf et al. 2011 ). It helps prevent dental caries in three key ways: - by reinforcing enamel through the formation of fluorapatite, - by aiding in enamel remineralization, and - by influencing bacterial metabolism to decrease acid production from cariogenic bacteria (AAPD 2023). Systemic fluoride (water fluoridation and dietary fluoride supplements), professional topical fluoride applications (acidified phosphate fluoride gel or foam, sodium fluoride varnish, and silver diamine fluoride), and home-use products (toothpaste and mouthwash) are all integral components of community caries prevention programs (Toumba et al. 2019 ). While fluoride offers significant benefits, excessive exposure can harm both dental and overall health, leaving patients uncertain about safe usage levels. Concerns also arise from its inclusion in food, hygiene products, and water sources [Buldur and Sezer., 2023]. Misinformation about fluoride is prevalent on social media, often distorting its benefits. Likewise, the avoidance of fluoride in dental practices seems to be a consequence of misleading information spread online [Lotto et al., 2022 ]. Artificial intelligence (AI) is based on computers carrying out human tasks and is actively used in many fields today, including healthcare services [Schwendicke et al., 2020 ]. With the advancements of AI, analysis of large amounts of data has become possible, which speeds up the decision-making process by providing accurate information [Mahajan et al., 2023 ]. A frequent use of AI is chatbots, which utilize text or speech to mimic human-like interactions [Rokhshad et al., 2024 ]. AI in dentistry has the potential to used to reduce administration, simplify diagnostics combine various data types, and eventually lower healthcare costs [Schwendicke et al., 2020 ]. In pediatric dentistry, AI can be used for detecting early childhood caries, analyzing radiographic images, identifying dental anomalies, detecting dental plaque, tracing cephalometric landmarks, and assessing growth patterns. These applications improve diagnostic efficiency while reducing errors and treatment time [Mahajan et al., 2023 ]. Companies such as OpenAI, Google, Anthropic, and Microsoft have developed advanced chatbots which are able to provide human-like responses. OpenAI's ChatGPT, a platform built on natural language processing, delivers accurate and pertinent responses using a large database [Dilan S. Hiwa et al., 2024 ; Fattah et al., 2025 ]. Google's Gemini, previously known as Google Bard, was designed as a conversational chatbot that can comprehend multimodal input and adapt to different kinds of data such as text, audio, and video [Fattah et al., 2025 ]. Anthropic's Claude is another well-known AI chatbot which generates responses that mimic those of a human and assists with problem-solving [Rokhshad et al., 2024 ]. Microsoft's Copilot, originally known as Bing Chat, is an AI tool that combines language models with organizational to make recommendations and offer help, hence improving productivity [Dilan S. Hiwa et al., 2024 ]. The reliability of chatbots that parents can use to obtain information about fluoride applications in children is unknown. There is no research evaluating the responses of different chatbots to questions about fluoride. The aim of the study is to evaluate the accuracy and consistency of responses from AI chatbots in pediatric dentistry. Therefore, AI chatbots were compared to pediatric dentists, general dentists, PhD students in departments of pediatric dentistry and dental students in answering questions related to fluoride usage in dentistry. Material and Method Study design and participants The present descriptive cross-sectional study was conducted between December 2024 to February 2025. Two groups were analyzed in this study. Chatbots including ChatGPT 4.0 Mini (OpenAI, USA), Gemini 1.5 (Google DeepMind, UK), Claude 3.7 Sonnet (Anthropic, USA) and Microsoft 365 Copilot (Microsoft, USA). Pediatric dentist, General dentist, Pediatric dentistry PhD students and fifth grade of dentistry students. Our study included pediatric dentists with at least two years of experience in their specialty, general dentists with at least two years of clinical experience, and third-year PhD students in pediatric dentistry Sample size The sample size was determined using the G*Power 3.1.9.2 software (Universitat Kiel, Germany). With a study power of 80% and an alpha error probability of 0.05 with an effect size of 0.25, the minimum sample size needed is 112. In the present study, 120 participants were included. Questionnaire The questionnaire was based on the guidelines of the International Association of Pediatric Dentistry (IAPD), American Association of Pediatric Dentistry (AAPD), European Association of Pediatric Dentistry (EAPD) and American Dental Association (ADA), which encompass the widely accepted approches for fluoride. A total of 23 questions were prepared for the questionnaire and the questions were divided into four subheadings as -individual topical fluoride applications (n = 8), -professional topical fluoride applications (n = 8), -systemic fluoride applications (n = 5), -fluorosis (n = 3). The questionnaire was sent to participants as online Google Forms document and answers were evaluated according to guidelines as true or false. Each question was asked 28 times to each chatbot, selecting the 'new conversation' option each time, and the answers were saved as true or false in an online Microsoft Excel spreadsheet. The test was administered from three different accounts, all starting at the same time to avoid time and account variations. This approach allowed us to investigate the consistency of responses. Statistical analysis The analyses were performed using IBM SPSS version 27 (IBM Corp., USA). For testing the relationship between categorical variables, the Pearson Chi-Square test was used when the sample size assumption (expected value > 5) was met, and Fisher’s Exact test was applied when this assumption was not met. Results The distribution of responses given according to AI types for topics is presented in the Table 1. As a result of the analyses, statistically significant differences were determined between AI types in terms of 'Individual Topical Fluoride Applications', 'Professional Topical Fluoride Applications', 'Systemic Fluoride Applications', 'Fluorosis', and total success levels (p < 0.05).For questions about Individual Topical Fluoride Applications, Claude had the highest correct answer rate (77.2%), while Copilot had the lowest correct answer rate (51.3%). Copilot (87.5%) and ChatGPT (87.9%) performed worse than Claude and Gemini, which had a 95.5% accuracy rate for questions pertaining to Professional Topical Fluoride Applications. Claude answered all the questions about Systemic Fluoride Applications correctly (100%), whereas the other AI models had lower accuracy, with ChatGPT scoring the lowest (94.3%). Gemini exhibited the highest level of accuracy in Fluorosis-related questions, accurately answering 76.8% of them. Conversely, ChatGPT demonstrated inadequate performance in this category, as evidenced by its accuracy of only 3.6%, which suggests that the majority of its responses were incorrect. With 86.2% accuracy, Claude had the highest overall success rate, followed by Gemini (84.0%), ChatGPT (74.8%), and Copilot (73.9%). The distribution of responses by AI types across the topics was presented in a Table 2. The analyses revealed statistically significant relationship (p < 0.05) between the type of dental professional and their performance about fluoride questions. In the category of Individual Topical Fluoride Applications, the highest correct response rate was observed among pediatric dentists (78.3%), followed by PhD students in pediatric dentistry (75.0%). General dentists demonstrated lower accuracy (52.1%), with the lowest correct response rate recorded among fifth-year dental students (59.3%). A similar pattern was noted in the area of Professional Topical Fluoride Applications. Pediatric dentists exhibited the highest accuracy (86.7%), closely followed by PhD students in pediatric dentistry (84.1%). In contrast, general dentists (73.3%) and students (74.6%) had comparatively lower rates of correct responses. Regarding Systemic Fluoride Applications, pediatric dentists achieved the highest accuracy (82.7%), while general dentists had the lowest performance (64.0%). Students and PhD students in pediatric dentistry demonstrated moderate success rates, ranging between 72% and 76%. In questions related to Fluorosis, pediatric dentists scored the highest (80.0%), whereas all other groups showed lower levels of success. Notably, general dentists exhibited the lowest correct response rate in this field (43.3%). Pediatric dentists demonstrated the highest overall accuracy at 82.3%, followed by PhD students in pediatric dentistry at 77.8%. general dentists (61.3%) and dental students (66.8%) exhibited significantly inferior performance in contrast. The distribution of responses by AI types across topics is presented in the Table 3. The analyses revealed statistically significant associations (p < 0.05) between the accuracy of responses and both the AI types and the dental professional groups of the participants across all topics. The highest correct response rates to questions about Individual Topical Fluoride Applications were observed for Claude (14.2%), pediatric dentists (15.4%), and pediatric dentistry PhD students (14.3%). In comparison, Copilot (9.4%) and general dentists (10.2%) had lower accuracy. In the field of Professional Topical Fluoride Applications, Claude and Gemini acquired high correct response rates (13.5%). Pediatric dentists (13.1%) had similar results, whereas general dentists (11.1%) and fifth-year dental students (11.7%) performed poorly. For Systemic Fluoride Applications questions, Claude (14.2%), Copilot (13.9%), and Gemini (13.8%) had greater accurate response rates. Pediatric dentists (12.6%) showed comparable knowledge, however general dentists (9.7%) had the lowest accuracy. In fluorosis-related questions, pediatric dentists (19.5%) and pediatric dentistry PhD students (15.9%) had the highest correct responses. Gemini (17.5%) also did well in this category. However, ChatGPT has a significantly low correct answer rate (0.8%). Overall, the AI types with the highest correct response rates on questions related to fluoride applications and fluorosis were Claude (13.7%) and Gemini (13.4%), while among professional groups, pediatric dentists (14.1%) were the most successful. In contrast, general dentists (10.5%), ChatGPT (11.9%), and Copilot (11.8%) demonstrated lower levels of accuracy. Discussion Dental fluoride research has been ongoing for over a century, providing substantial evidence of its effectiveness in preventing dental caries and improving oral health [Unde et al., 2018 ]. In recent years, the global debate on fluoride has been divided into two groups: those who support topical fluoride applications—citing scientific evidence of their effectiveness in preventing tooth decay when used appropriately in children—and the anti-fluoride lobby [Buldur and Sezer, 2023 ]. Based on scientific evidence, topical fluoride applications, combined with community-based fluoride initiatives, are widely recognized as the gold standard for preventing dental caries, provided that appropriate attention is given to the risks of acute and chronic toxicity [Ulah et al., 2017]. This study is one of the most detailed in dentistry to assess the accuracy of various chatbots compared to clinicians in answering fluoride-related questions. Chatbots, powered by vast datasets drawn from diverse textual sources such as books, articles, and web content, leverage advanced deep learning techniques—including neural networks—to understand and generate responses [Huynh et al., 2022]. While some studies affirm the effectiveness of chatbot systems and the relevance of their responses, others point out their limitations and caution against potential shortcomings, emphasizing the importance of critical evaluation [Helvacioglu Yigit et al., 2025; Mustuloglu and Deniz, 2025]. Additionally, the literature includes research examining the reliability and consistency of chatbots in clinical settings, including both medical and dental fields [Rokshad et al., 2024]. Strong et al. ( 2023 ) emphasized that although ChatGPT performed satisfactorily on approximately half of the questions derived from clinical reasoning exam cases, its consistency was limited and its accuracy varied with each individual query. According to Hirosawa et al. ( 2023 ), in clinical cases involving common chief complaints, ChatGPT-3.5 demonstrated a high level of diagnostic accuracy in generating differential diagnoses. Similar to our study, some studies involved both chatbots and clinicians, presenting identical questions to both groups in order to compare the consistency and accuracy of human and chatbot responses [Bayraktar Nahir, 2025 ; Buldur and Sezer, 2023 ]. A recent study reported that the ChatGPT 3.5 chatbot provided answers to questions in the field of pediatric dentistry that were similar to those given by specialists, suggesting that it can assist both dentists and patients, although improvements are still needed [Bayraktar Nahir, 2025 ] while another study showed that ChatGPT were found to be sufficient and comprehensive in fluoride releated questions [Buldur and Sezer, 2023 ]. In this study, although the percentage of correct answers given by ChatGPT was lower compared to the other chatbots included in present study, its accuracy rate was still higher than its incorrect answer rate and was considered sufficient. The results of the current study are consistent with the findings of previous studies [Bayraktar Nahir, 2025 ; Buldur and Sezer, 2023 ]. On the other hand, Rokhshad et al. ( 2024 ) posed pediatric dentistry-related questions to AI-powered chatbots and dentists with varying education levels, finding that pediatric dentists were significantly more accurate than both other clinicians and chatbots similar to our study, and that ChatGPT demonstrated the highest accuracy among the chatbots. In the present study, ChatGPT demonstrated lower accuracy compared to Claude, Gemini, and pediatric dentists, with Claude consistently outperforming all other chatbots and dentist groups across most question categories. In a previous study, pediatric dentistry-related questions were presented to general dentists, dental students, and AI-supported chatbots, with the chatbots exhibiting lower performance compared to both the dentists and the students [Azadi et al., 2024 ]. Furthermore, prior studies have revealed considerable discrepancies in accuracy among different chatbots, as also demonstrated by our findings [Mohammad-Rahimi et al., 2023]. These variations in chatbot performance are believed to be influenced by differences in study design, regional and cultural differences, and the time periods in which the studies were conducted. Sismanoglu and Capan (2025) evaluated the academic performance of ChatGPT-4.0 and Gemini using questions from the Turkish Dental Specialty Examination (DUS), reporting that both achieved passing scores while also identifying knowledge gaps and stressing the need for caution before fully relying on chatbot outputs [Sismanoglu and Capan, 2025]. Furthermore, s contrasting study by Jung et al. ( 2024 ) in Korea found that none of the chatbots evaluated met the minimum passing score required by the Pediatric Dentistry National Examination [Jung et al.,2024]. In present study, it was found that Gemini and Claude chatbots exhibited the highest levels of accuracy in answering questions related to fluoride, even surpassing the accuracy rates of general dentists and dental students in Turkey, who are expected to take similar examinations. A comparison of the results from these studies reveals that although both emphasize the ability of chatbots to deliver accurate responses within a specialized field, discrepancies in accuracy levels are evident, with certain chatbots exhibiting superior performance based on factors such as the type of exam and the geographical context. This study has several limitations. First, all questions were presented in English, which may have advantaged AI models predominantly trained on English data, limiting the applicability of results to non-English contexts. Second, the focus on fluoride-related content restricts the generalizability of the findings to other areas of dental or medical knowledge. Third, as AI models are continuously updated, the results reflect performance at a specific point in time and may vary with future versions. Finally, the use of a binary (correct/incorrect) evaluation system may have overlooked partially accurate or contextually appropriate responses, particularly in complex clinical scenarios. Conclusion This study highlights the varying accuracy and consistency of AI chatbots in addressing fluoride-related questions in pediatric dentistry. Among the chatbots evaluated, Claude and Gemini demonstrated higher levels of accuracy across all fluoride-related categories, often surpassing the performance of general dentists and dental students. Conversely, ChatGPT and Copilot showed relatively lower performance, particularly in the category of fluorosis, where ChatGPT's accuracy was notably poor. These findings suggest that while some AI chatbots are capable of providing reliable dental health information, their effectiveness can vary significantly depending on the specific model and the topic addressed. The comparison between AI-generated responses and those provided by dental professionals emphasizes the continued importance of expert oversight, especially in clinical decision-making. While advanced chatbots like Claude and Gemini may serve as useful educational tools or adjuncts in patient communication, they should not replace professional consultation. As AI technologies continue to evolve, further research is needed to monitor their performance across broader domains of dentistry and in multiple languages. Efforts to improve chatbot accuracy, reduce misinformation, and establish guidelines for their responsible use in clinical settings will be crucial for their safe integration into dental care. While some studies affirm the effectiveness of chatbot systems and the relevance of their responses, others point out their limitations and caution against potential shortcomings, emphasizing the importance of critical evaluation [Helvacioglu Yigit et al., 2025; Mustuloglu and Deniz, 2025]. Additionally, the literature includes research examining the reliability and consistency of chatbots in clinical settings, including both medical and dental fields [Rokshad et al., 2024]. A recent study reported that the ChatGPT 3.5 chatbot provided answers to questions in the field of pediatric dentistry that were similar to those given by specialists, suggesting that it can assist both dentists and patients, although improvements are still needed [Bayraktar Nahir, \u003cspan citationid=\"CR3\" class=\"CitationRef\"\u003e2025\u003c/span\u003e] while another study showed that ChatGPT were found to be sufficient and comprehensive in fluoride releated questions [Buldur and Sezer, \u003cspan citationid=\"CR4\" class=\"CitationRef\"\u003e2023\u003c/span\u003e]. In this study, although the percentage of correct answers given by ChatGPT was lower compared to the other chatbots included in present study, its accuracy rate was still higher than its incorrect answer rate and was considered sufficient. The results of the current study are consistent with the findings of previous studies [Bayraktar Nahir, \u003cspan citationid=\"CR3\" class=\"CitationRef\"\u003e2025\u003c/span\u003e; Buldur and Sezer, \u003cspan citationid=\"CR4\" class=\"CitationRef\"\u003e2023\u003c/span\u003e]. On the other hand, Rokhshad et al. (\u003cspan citationid=\"CR16\" class=\"CitationRef\"\u003e2024\u003c/span\u003e) posed pediatric dentistry-related questions to AI-powered chatbots and dentists with varying education levels, finding that pediatric dentists were significantly more accurate than both other clinicians and chatbots similar to our study, and that ChatGPT demonstrated the highest accuracy among the chatbots. In the present study, ChatGPT demonstrated lower accuracy compared to Claude, Gemini, and pediatric dentists, with Claude consistently outperforming all other chatbots and dentist groups across most question categories. In a previous study, pediatric dentistry-related questions were presented to general dentists, dental students, and AI-supported chatbots, with the chatbots exhibiting lower performance compared to both the dentists and the students [Azadi et al., \u003cspan citationid=\"CR2\" class=\"CitationRef\"\u003e2024\u003c/span\u003e].
