This study aims to investigate the feasibility of this transition and identify areas of unmet needs from post surgical patients through electronic Patient Reported Outcome Measures (ePROMS). Patients and Methods The responses from the three self-reported questionnaires completed by breast cancer patients were collected between September 2022 and March 2024. Results Of the 133 patients invited to the Nurse-led Clinic, 47 (35.3%) completed pre-clinic PROMs and attended. The completion rate was 98%, 52% and 58% respectively. From a smaller sample of 9 survey respondents, 100% reported being satisfied with the program, would recommend the Care Companion breast survivorship program. 89% of respondents felt able to complete the tasks, felt actively involved, found care companion easy to use and were comfortable using devices. Conclusion This pilot study has demonstrated that Care Companion is a user-friendly electronic tool for a long-established paper-based service. This study aims to enhance our comprehension of post-surgical care by examining the unmet needs and distress areas on referrals from survivorship clinics, as well as the influence of survivorship clinics on interactions with the hospital beyond scheduled visits. Figures Figure 1 Figure 2 Introduction The Royal Melbourne Hospital(RMH) established the Nurse-led Survivorship Clinic as part of our Breast Cancer Survivorship Program in 2007 for patients diagnosed with early breast cancer( 1 ). The program involves a Breast Care Nurse-led consultation following the end of definitive in-hospital care, which, for the majority, is approximately 6 to 12 months after a breast cancer diagnosis. In the pre-COVID era, this clinic required completion of Patient Reported Outcome Measures (PROMs) questionnaires 4 weeks prior to their nurse-led appointment. During this appointment, the Breast Care Nurse (BCN) reviews issues raised by the patient and those identified from their PROMs questionnaires. After identifying the areas of distress and need for intervention, the breast care nurses discuss these with the patient, and then generate a shared-care plan for use between the hospital and primary care physicians. This care plan is reviewed and signed by the treating hospital clinicians and the BCNs. A digital platform Care Companion was launched to facilitate the Nurse-Led Survivorship Clinic. Care Companion (Fig. 1 ) is a feature available in the Parkville Health Service’s patient portal App, known as Health Hub, that is powered by MyChart® licensed from Epic Systems Corporation, © 1999–2024. This was co-designed by the RMH electronic medical record team, BCNs, surgeons and consumers. The Care Companion is a personalised electronic plan of care with breast cancer resources, contact details for staff and relevant links to groups such as Breast Cancer Network Australia (BCNA) and Cancer Council of Victoria. It also contains three PROMs i.e. the Supportive Care Screening Tool (SCST) (Appendix 1), the Functional Assessment of Cancer Therapy–Breast (FACT-B + 4)( 2 ) and the Menopause Rating Scale (MRS)( 3 ). This study aimed to 1) investigate the feasibility of collecting PROMS through the Care Companion, assessing the rate of participation and completion, 2) identify areas of distress and unmet needs from our patients through PROMs and inform the Nurse-led Survivorship Clinic. Methods This study was set up prospectively to enable evaluation of women with breast cancer 6–12 months post their treatment at the RMH between September 2022 to March 2024. The inclusion criteria were patients > 18 years old, undergone surgical treatment (wide local excision or mastectomy), completed definitive hospital care and waiting to attend a nurse-led consultation survivorship clinic. Each participant provided verbal informed consent, which was subsequently formalised through the completion of ePROMs. Invitations were disseminated to participants through the Health Hub; hard copies were mailed to those who did not have access to Health Hub. Patients are invited to sign up to the Epic Health Hub and can access the Care Companion from the beginning of their treatment. A notification to complete PROMS occurs 4 weeks prior to the Nurse-led clinic with reminders up to the clinic appointment. A dashboard built in the EMR allows staff to monitor the progress of completion of the PROMS and interrogate results prior to the patient attending the clinic appointment. The BCNs can document the results into the EMR and action any escalations in ‘real-time’, by sending resources to the patient through the Health Hub or to generate referrals to allied health services. Patients who have scored high in distress and/or have given additional feedback will be identified via the Dashboard, prioritised contacted by the nurses by phone. The SCST was a questionnaire developed by the multidisciplinary supportive care team at The RMH and Royal Women’s Hospital, which has been a standard tool in use since 2007. The goal is to assess the level of distress felt by patients and identify the potential factors contributing to distress levels in the last 7 days prior to questionnaire completion. It consists of a modified form of the National Comprehensive Cancer Network (NCCN) Distress Thermometer (2012) and 28 questions across physical, emotional and social domains (Appendix 1). The FACT-B + 4 version 4.0( 4 ) is a 41-item instrument with high reliability and validity to assess the quality of life in breast cancer patients. A higher score indicates a better quality of life (QoL) for the patient. The average time required to complete the FACT-B + 4 is 10–15 minutes( 2 ). The third questionnaire used is the MRS( 3 ). It quantifies the severity of aging-related symptoms and their impact on QoL. The typical time to finish MRS varies from 3 to 7 minutes, with a mean of 4.8 minutes( 5 ). Data analysis: Descriptive statistics were for the demographic data. Questionnaire responses were analysed with Excel (Microsoft 2018) with frequency (percentage) and mean (standard deviation). In particular, we assessed the number of patients who signed up to the portal, the percentage of patients who completed the PROMS and who attended a nurse-led clinic and the key areas in the psychosocial domains identified as ongoing issues prior to the Nurse-Led Clinic. Results During the period from February to June 2023, there are 701 cancer patients have accessed the Health Hub (93%). 133 breast cancer patients received invitations for Nurse-Led Clinic appointments scheduled through the Health Hub; 47 (35.3%) patients have subsequently completed at least one of three ePROMs and attended an appointment. The mean age was 59 years old (SD = 10, Range 35–77). The majority spoke English as their primary language (n = 42, 89.4%), followed by Arabic and Vietnamese (n = 2, 4.3% each) (See Appendix 2). The Supportive Care Screening Tool (SCST) The SCST was completed by 45 women (96%) where 42 women (89%) provided a score for the distress thermometer (Table 1 ). The average mean score for the distress thermometer was 3.45 (SD 2.8), which is considered as low distress as per the NCCN guidelines. When examining practical considerations, the over 75% of women stating no concerns with their day-to-day activities (Accommodation, Home care/Help, Culture/Language/Communication). There were 30–40% women reported memory or concentration difficulties, difficulties in their work or academic environment and experienced a lot of worry or anxiety. Over 84% of women reported did not experience difficulties with their children or partners and reported adequate social support. Over 75% women did not report any concerns about their sexual or intimate relationship or their sexual attractiveness. With regards to background issues, more than half (23/45) women had some forms of mental illness in the past. Between 26% and 38% of women indicated a desire for supplementary materials, such as written information or supportive resources (music, relaxation techniques). In the physical wellbeing domain, most patients reported no symptoms to moderate symptoms. This included lack of energy, nausea, trouble meeting the needs of family, and pain. In the social wellbeing domain, most patients reported good and excellent support from family and friends. Of note, 15 (55.56%) women failed to answer the statement ‘I am satisfied with my sex life’. There were 7(25.93%) women worried about dying, 12 (44.44%) women feared their condition would get worse and 6 (22.22%) women lost hope fighting again their illness. Most patients were content with their functional wellbeing, with more than 85% women expressed satisfaction with their quality of life. Regarding the statement, "My work (including work at home) is fulfilling," a similar percentage of respondents selected "not at all" (26.92%) and "very much" (19.23%). In the breast cancer subscale, a majority of women reported concerns regarding weight fluctuation and experienced moderate to severe pain. Furthermore, more than half of the women surveyed reported stress related to their illness and concern about their family members with the same condition. It is worth noting that most women, exceeding 50%, did not report any signs of lymphedema, such as arm swelling, tenderness or stiffness. Within the realm of physical symptoms, there is approximately 30% women reported some degree of hot flushes and sweating, sleep disturbance and joint and muscular discomfort. Majority women (69–83%) experienced some degree of psychological symptoms including depression, anxiety and physical/mental exhaustion. In the urogenital domain, 44–62% women reported sexual problems, bladder problems and vagina dryness. There is a paucity of literature on the experiences of Australian breast cancer patients with nurse-led clinics. Our institution previously performed a retrospective audit( 6 ) from 2016 to 2017, but there were limitations. These included a significant time delay between nurse-led and medical consultant assessments, which may not accurately reflect patient symptoms and challenges in documenting all symptoms and issues on paper before the introduction of the EMR in 2020. There is one similar study conducted in a private hospital in Western Australia in 2023, with a focus on the issues of chemotherapy( 7 ). The benefit of this model is to capture PROMS at a defined timepoint with the safety net of a defined clinic appointment at a short interval to identify areas of distress and unmet needs and act on them in the real-time. Although, PROMS can be evaluated for research purposes, there is a duty of care implicit and requires action and intervention in the real-time. Most cancer survivors experience a broad range of physical symptoms, such as fatigue, nausea, chronic pain, difficulties concentrating or sleeping. In MRS, 65.51% of women reported moderate-to-severe sleep disturbance. This is consistent with 57.78% women have fatigue and/or sleep problems in SCST. In Fact-B + 4, only 19.23% of women reported sleeping well. Similarly, 24.14% of women experienced severe or very severe pain in Fact-B + 4. This is consistent with 26.67% of women suffered from chronic pain management in SCST. According to Chan's systematic review, nurse-led interventions were found to be as safe and effective as physician-led care for symptom management( 8 ). Through implementing a Nurse-led survivorship clinic, the BCN team can effectively monitor and address ongoing health concerns by promptly identifying symptoms and providing necessary follow-up and management/action strategies. From this study, there were 55.56% of participants chose not to record their level of satisfaction with sex life. Of the twelve respondents, two indicated dissatisfaction with their sex lives. In SCST, 22.22% of women have concerns about their sexual or intimate relationship. In the MRS, 20.69–31.03% experienced sexual dysfunction, vaginal dryness, and urinary incontinence. These findings suggest that breast cancer survivors are dealing with concurrent health conditions or are in the post-menopausal phase, as breast cancer is frequently diagnosed in women over the age of 61( 9 ). Furthermore, treatment such as chemotherapy or long-term hormone therapy could negatively impact on sexual desire and sexual experience( 10 ). Within the nurse-led consultation, symptom management may encompass collaborative communication with treating physicians to change endocrine agents, prescription of vaginal gels, or referral for psychosocial interventions( 11 ). Women who receive a breast cancer diagnosis are at a higher risk of developing or exacerbating pre-existing mental health conditions, such as depression and anxiety( 12 ). In SCST, 51.11% of women indicated their anxiety or depression or other mental illness in the past and 19.07% indicated a high level of distress on the distress thermometer. Across the 3 questionnaires, approximately 33–40% of women are bothered by their anxiety and 10–24% of women felt sad/depressed most of the time. These results do not differ from emerging literature across different clinical settings. The prevalence of anxiety is higher than the reported prevalence from the systematic review on patients with ≥ 1 year after breast cancer diagnosis, which is 17.9–33.3% in 2015( 13 ). The incidence of depression in our patient group corresponds to the global prevalence of major depression of 13.2% among breast cancer patients in the meta-analysis conducted in 2019( 12 ). A prospective and 2-year longitudinal study from the Royal Women’s Hospital confirmed the past or pre-existing psychological morbidities significantly predict the symptom severity in women with diagnosis of breast or gynaecological cancer( 14 ). Current literature suggested the screening monitoring of psychological health issues can be conducted and targeted interventions can be offered to prevent their resistance to treatment within the model of nurse-led survivorship care. Returning to work is important for cancer survivors and is also an important aspect of quality of life. Evidence shows returning to work could provide a sense of self and dignity and income, which is invaluable for cancer survivors to support themselves and family and is often regarded as a milestone of complete recovery and re-establish normality( 15 ). The diagnosis and treatment of breast cancer, along with the related physical and mental health challenges, frequently result in increased sick leave, a higher risk of job transitions, unemployment, and early retirement( 16 ). Moreover, the reintegration into the workforce is often prolonged, and cancer survivors' long-term employability is negatively affected( 17 ). In our MRS questionnaire, 14.81% of women struggled to work at all (including working at home) and 38.46% did not think their work was fulfilling. This correlates well with their answers in SCST; 31.11% of women indicated difficulties in their work/study setting and 20.00% had difficulties in managing their finances/insurance. Supporting patients with breast cancer transition to meaningful and sustained work is a complex process. While major focus of follow-up treatment by breast surgeons and oncologist is managing treatment related side-effects and surveillance for recurrence, the BCNs provide a holistic assessment of a range of health and lifestyle domains and discuss strategies that patients can self-manage or require assistance from other service providers within the hospital or community setting( 18 ). Limitations We acknowledge that this pilot study is primarily constrained by the small sample size and the fact that it was conducted at a metropolitan tertiary cancer centre which may not be applicable to other settings. Nonetheless, it was developed and introduced during the active and prolonged phase of Victorian COVID lockdowns and perhaps as seen in other areas of living, the pandemic hastened the use of electronic platforms globally. Women with low health and or digital literacy and culturally and linguistically diverse (CALD) populations may have difficulties navigating or accessing the digital platform of Care Companion( 19 ). Our ePROMS may be limited by ‘survey-fatigue’ and repetitive questions across questionnaires, a problem ICHOM have tried to address with their modified Breast ICHOM PROMS( 20 ). Lastly, some women state sleep or body image & several other areas as an issue on one tool but not on another. There are calls for establish a comprehensive tool that capture a more complete response of each domain. Conclusion This pilot study has demonstrated Care Companion successfully transitions PROMs onto an electronic platform. It provided insight into the breast cancer survivors' various unmet needs and their perception health and wellbeing following surgical care treatment. Supported by these results, we continue to refine the user-friendly electronic platform to establish a routine collection of PROMs prior to their dedicated breast nurse-led survivorship clinic. By integrating this approach, we aim to enhance post-surgical follow-up, ensuring timely referral made from the clinics and broader survivorship challenges. Only then, can the value-added care of collecting PROMS and Nurse-Led Survivorship clinics in surgical recovery be fully understood. This study aims to investigate the feasibility of this transition and identify areas of unmet needs from post surgical patients through electronic Patient Reported Outcome Measures (ePROMS).\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003ePatients and Methods\u003c/strong\u003e\u003c/p\u003e\n\u003cp\u003eThe responses from the three self-reported questionnaires completed by breast cancer patients were collected between September 2022 and March 2024.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults\u003c/strong\u003e\u003c/p\u003e\n\u003cp\u003eOf the 133 patients invited to the Nurse-led Clinic, 47 (35.3%) completed pre-clinic PROMs and attended. The completion rate was 98%, 52% and 58% respectively. From a smaller sample of 9 survey respondents, 100% reported being satisfied with the program, would recommend the Care Companion breast survivorship program. 89% of respondents felt able to complete the tasks, felt actively involved, found care companion easy to use and were comfortable using devices.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eConclusion\u003c/strong\u003e\u003c/p\u003e\n\u003cp\u003eThis pilot study has demonstrated that Care Companion is a user-friendly electronic tool for a long-established paper-based service.
