Farmers use huge amount of inorganic pesticides and/or fertilizers (agrochemicals) without any training and proper personal safety to fulfil the increased food demand, resulting into health problems. Therefore, the current study aims to assess the prevalence of toxicity symptoms among the farmers due to use of agrochemicals in the north-eastern state of Tripura, India. Methods To fulfil the objective of the study, primary data was collected from 995 farmers of Dhalai district using multistage stratified random sampling. A semi-structured questionnaire was developed and pre-tested. Bi-variate analysis was used to assess the pattern of toxicity and Chi-square test was applied to test the significance of association with the factors under consideration. Finally, logistic regression was applied to examine the factors independently associated with different toxicities. Findings : Above one-third (39%) of the farmers reported toxicity symptoms while using agrochemicals. Prevalence of symptoms of any toxicities was much higher among males than females. The major toxicity symptoms were skin-rash (16%), vomiting (12%), skin redness (10%), headache (7%), and excessive sweating (6%). Age, sex, occupation, smoking and drinking behaviour were significantly associated with the symptoms of toxicities. Conclusions There is need for increased awareness level and training to accustom the farmers with safe professional practices and personal hygiene. In addition, there is need to examine the farmer welfare government schemes to ensure its reach at the community level. Toxicity Farmers health Occupational health Agrochemical India Background One-third of the population in the world, works in professions related to agriculture and it’s the third most hazardous occupations among all sectors globally. 1 The majority of the time, environmental and occupational health hazards can have a negative impact on a farmer's health. To meet the increasing demand of agricultural products and declining agricultural land, farmers are extensively using the fertilisers and pesticides to increase the yield. As agricultural land has decreased during the period, farmers are using greater amounts of inorganic fertilizers and pesticides, even though, they little awareness and training about the safe use of inorganic pesticides and inorganic fertilizers (agrochemicals). As a result, farmers are more exposed to toxicities like headache, skin rash, excessive sweating and diarrhoea. 2 Toxicity is defined as the amount or degree of a substance needed to be poisonous. Poisoning is injury or death due to swallowing, inhaling, touching or injecting various drugs, chemicals, venoms or gases. There is a major occupational health risk due to the excessive use of agrochemicals caused by modern agriculture, including respiratory impairments, poisoning cancer, and reproductive impairments. 1 , 3 Human diseases some of which are classified as occupational diseases are carried on by mechanical, ergonomic, psychological, chemical, biological, and agrochemical factors in the workplace. Further these health related hazards along with hazards related to environment lead to develop diseases among the farmers. These ailments may accelerate pre-existing illnesses with non-occupational causes or bring on a new disease in previously healthy or susceptible individuals. 4 The International Labor Organization states that due to the enormous costs it causes, particularly in terms of reduced productivity and burden on systems of social security, "the inadequate prevention of occupational diseases has profound negative effects not only on workers and their families but also on society at large”. 5 India is an agrarian nation with majority of people (more than two-third) engaged in the agricultural sector. 6 In India's agricultural sector, there were 262 million of farmers (according to the census, agricultural labourers and cultivators are both referred to as farmers) accounting for about one-fifth of the population. This consists of 118 million cultivators and 144 million agricultural labourers working in agriculture as cultivator or as agricultural labours. Women constitute a considerable proportion of the agricultural workforce. There were 97.5 million female farmers, according to the 2011 Census, of which, 61.5 million were agricultural labourers and 36 million were cultivators. 7 Farmers are more likely to suffer from minor and serious illnesses and injuries because they are less skilled at using more agrochemicals (such as, synthetic pesticides or fertiliser). 8 There is lack of studies on toxicity, including occupational health of farmers in India and further little to none literature available from the state of Tripura, despite being the fact Tripura's rural population relies primarily on agriculture for employment. 7 Therefore, present study was conducted with the objective of assessing the toxicity pattern and its associated factors among the farmers in Tripura state of India. Materials and methods The current study was a cross-sectional study conducted in the Dhalai district of Tripura, India. The sample size was calculated to be adequate to estimate a minimum proportion of 10% with an absolute error of 3% and 95% confidence. The sample size for current study included 995 respondents from 350 households. The study selected the respondents using a multi-stage sampling procedure. In the first stage, the district with the highest proportions of farmers, namely, Dhalai district was selected. In the second step, three blocks were selected randomly from a total of eight blocks, and subsequently, five villages were selected in the third stage from each of the selected blocks. Finally, required number of households from each village were selected using probability proportional to size (PPS) and the household was selected using the systematic random sampling procedure. For the purpose of random sampling, mapping and listing of the households in the selected villages was carried out and a sampling frame of all the households with at least one adult farmer was prepared. A semi-structured questionnaire was administered through face-to-face interviews to collect the data from the respondents in 2021. For the current study, farmers were the respondent and were defines as “those working as a cultivator or as agricultural labour”. Among the selected farmers, 147 didn't use any inorganic fertilizer or pesticides. Thus, 848 farmers who used any kind of inorganic fertilizer and/or pesticides were only included in the study. The questionnaire included the questions on different toxicity symptoms, and economic and sociodemographic characteristics. Ethical approval for the study was obtained from the Students Research Ethics Committee (SREC) of the International Institute for Population Sciences (IIPS), Mumbai. Before administering the survey instrument, an informed and written consent was obtained from each respondent. The pattern of toxicity among the farmers was assesed by univariate analysis and prevalence of the same were presented as precentages. Any toxicity, with addition to individual toxicities (skin rashes, and vomiting), was also assessed. The prevalence rates according to various socio-demographic and economic factors were obtained applying bi-variate analysis. The association of toxicity with selected characetistics was examined through chi-square test. Finally, multi-variable logistic regression was employed to examine the average relationship of toxicity with the selected background characteristics. The variables found to be significantly associated at 10% level of significance were selected for inclusion in the logistic regression. The analysis was performed using STATA 15.0. Results Prevalence of toxicity symptoms According to the current study, around two-fifth of the farmers experienced toxicity symptoms after using inorganic pesticides or organic fertilisers (Table 1 ). Skin rash (16%; 95% CI: 13.3%, 18.3%) was observed as the most common toxicity symptom, followed by vomiting prevalent among more than one-tenth of the farmers (95% CI: 9.3%, 13.6%) and skin redness (10%). Around six percent of farmers reported having headache or excessive sweating, and some farmers additionally suffered of itchy eyes, weariness, abdominal pain, diarrhoea, and chest pain (around two percent or more). Further, none of the farmers experienced any toxic symptoms of dry cough, convulsion, vision disturbance, gastro-intestinal distress, or central nervous system (Table 1 ). However practically a negative prevalence is meaningless. Hence, it was truncated to zero. Toxicity pattern by socio-demographic factors Prevalence of symptoms of any toxicities by selected background characteristics is presented in Table 2 . Age, sex, farmer type, smoking and drinking were found to be significantly associated with any toxicity. With an advancement of age, the prevalence of any toxicity increased. For example, farmers aged 65 years or more had the highest prevalence of toxicity symptoms (62%) compared to all those between 45 and 60 years of age (48%) and those in the younger age of 15–29 years and 30–44 years (29% in each group). Male farmers showed higher prevalence of symptoms of any toxicities (57%), compared to female farmers (15%). The prevalence of toxicity symptom was highest (61%) among the cultivators (totally dependent on agriculture for their livelihood), followed by agricultural labours (29%) and other farmers (those farmers working in non-agricultural sector along with farming) (19%). The toxicity symptoms were highest among farmers who smoke (81%) and have drinking habit (69%). Although education and wealth did not show significant association, there were considerable variation in toxicities in different categories. Further, the current study also assessed the prevalence of two most common toxicities (skin rash and vomiting) to understand the association with the major sociodemographic and economic characteristics of farmers in Tripura (Table 2 ). Age, sex, education, farmer type, smoking and drinking were significantly associated with skin rash. Farmers aged 45–60 years and aged 65 or more had the highest prevalence of skin rashes (20%). The prevalence of skin rash was higher among males (22%) than in females (7%). One fourth of the cultivators experienced skin rash, whereas, the agricultural labous and other farmers reported to have prevalence rate of eight percent. The prevalence of the vomiting (second highest toxicity symptoms) also increased with increased age of farmers. Similar to any toxicity and skin rash, the prevalence of vomiting was also higher among male farmers (14%) compared to female farmers (7%) and the result was significant. The prevalence of vomiting among the widow/seperated farmers was higher (26%) compared to other which showed much lower levels. The association of the vomiting with caste and marital status was significant. The prevalence rate of vomiting was highets among cultivators (16%), followed by other farmers (9%) and agricultural labourers (4%). Around one-fifth of the smokers and drinkers reported to have been suffered from vomiting. Factors independently associated with toxicity The relationship between any toxicity and sociodemographic characteristics of farmers in Tripura, India, is presented in Table 3 . Caste, smoking and drinking are significantly associated with any toxicity. As age increases, the odds of reporting any toxicity increased. The farmers aged 65 years or more were 85% more likely to have any symptoms of toxicity compared to farmers aged 15–30 years. The results suggested that male farmers were 4.6 odd times more likely than female farmers to develop any toxicity. When compared to farmers who were not married, married farmers seemed to have 20% greater chances of suffering from any toxicity symptoms, while widowed or separated farmers had 2.6-odd times greater likelihood. In comparison to the cultivator, agricultural labourers and other worker-farmers were marginally less likely to experience toxicity. Toxicity symptoms were 2.7 odd times more likely to be experienced by smokers rather than non-smokers. Further, farmers who never chewed tobacco were less likely to be at risk of any toxicity than farmers who consumed any tobacco. Although education did not show significant association, there were considerable variation in any toxicity in different levels of education. Caste and occupation are the only variables significantly associated with skin rash. Skin rashes became more prevalent as persons aged. Compared to farmers aged 15–30 years, those aged 30–44 years had an increased risk of skin rash by 9%, and among farmers aged 45–60 years the odds of reporting skin rash was 53% higher. The odds of skin rash among male farmers were twice compared to female farmers. Compared to cultivators, agricultural labourers had a 56 percent lower risk of developing skin rash, whereas, other farmers had a 44 percent lower risk. In comparison to non-smokers, farmers who smoke were substantially more likely to experience skin rash symptoms. There was no significant relationship between any characteristics and vomiting in the current study. However, with advancement of age the odds of reporting vomiting increased. Male farmers reported 1.7 odd times higher prevalence of experiencing vomiting than females. Compared to cultivators, agricultural labourers had a 53 percent lower risk of getting vomiting problems, but other farmers had a marginally higher risk. Therefore, majority of people in the country are involved in and dependent for their livelihood on agriculture. There is variation within the country not only in the level of development but also in the proportion of population involved in agricultural activities. Tripura in general and Dhalai district in particular are known for higher dependency on agriculture. On one hand improper / unsafe use of inorganic fertilizer and pesticides is known for its hazards. On the other, farmers with relatively low level of education, lack/adequate training and knowledge, attitude and practice (KAP) related the use of agricultural practices. Therefore, present study was undertaken to assess the quantum of occupational toxicity and the socio-demographic and economic factors that may influence it. Toxicity is the quantity of a substance required to cause harm or death when ingested, inhaled, or touched by certain chemicals. In the Dhalai district, above one-third of the farmers had one of the toxicity symptoms. A prior survey also revealed that more than half of the respondents had been poisoned. 9 The present study and some earlier studies clearly reveal that the prevalence of skin rash (highest in this study), vomiting, eye itching, fatigue, abdominal pain, diarrhoea, rhinorrhoea, and chest pain were more common among the farmers. 10 – 13 Half of the farmers who were illiterate or just had an elementary education or less had higher toxicity issues, skin rashes and vomiting being the prominent symptoms. Due to lack of education, the majority of farmers are unaware of proper handling and health risks associated with improper handling of agrochemicals. 10 Education plays and important role in enhancing the level of awareness related to health issues. According to Kumari & Reddy and Oztaş et al, education has a stronger impact on using personal safety to sustain good health. However, they further noted a contrary association with wealth status. 2 , 14 Wealthy farmers were more susceptible to poisoning, With more than one-third reported experiencing any kind of toxicity, and one in ten experienced issues with vomiting. The higher prevalence rate of toxicity among wealthier farmers was associated with higher utilization of health care and better reporting among them by Shete et al according to whom, most impoverished individuals only seek medical assistance for serious medical conditions due to higher expenses. 12 As observed during fieldwork, male farmers used pesticides and fertilisers more frequently than female farmers, hence, were more exposed to health issues caused by agrochemicals and the prevalence of the toxicity symptoms was higher among males than in females. In a study conducted in southern states of India, Chitra et al found similar findings with higher prevalence of poisoning symptoms among men and was substantially associated with exposure time. 10 Moreover, Gupta et al explained that the male responders produced dust and liquid formulations of several pesticides resulting into poisoning symptoms. 15 Any toxicity, especially skin rashes and vomiting, seems to be more prevalent when people in older ages which might be attributed to the higher susceptibility in older ages. However, younger farmers are involved more in the use of agrochemicals because they are more energetic and hardworking than older farmers. Young individuals actively involved in pesticide spraying, which may have an impact on their health, according to Dey et al. 13 Agricultural labourers or other farmers had a slightly lower likelihood of experiencing toxicity, such as skin rashes and vomiting, compared to cultivators. The owner of the land and the head of the household might be the major cultivator, and they may have relied entirely on agriculture for their livelihood. To produce more goods and earn more money, they might utilize additional inorganic fertilisers and pesticides. And therefore, farmers were exposed to additional agrochemicals. Farmers are more likely to consume the tobacco product to work longer in the fields which they consider as a means to overcome tiredness, stress, sleepiness, discomforts, and laziness. According to certain studies, people even use tobacco to reduce nausea. 13 To conclude, the prevalence of any toxicity and skin rashes and vomiting in particular is high among the farmers of Tripura district of India. Similar condition might be prevalent among the farmers of other regions and there is a need to examine and validate the same. There is a need to increase the awareness level about the use of pesticides, fertilisers and agricultural tools along with the prevention steps. There are many schemes which imparts the techniques and precautionary steps of farming, however, there is a need to evaluate the extent of its implementation and strengthen the schemes accordingly to lower the toxicity levels among farmers. For better health, risk education and awareness, improved facilities and infrastructure could be provided. It is also essential to assess government programmes for farmer welfare to make sure they reach the community level. In the first stage, the district with the highest proportions of farmers, namely, Dhalai district was selected. Finally, required number of households from each village were selected using probability proportional to size (PPS) and the household was selected using the systematic random sampling procedure. Age, sex, farmer type, smoking and drinking were found to be significantly associated with any toxicity. With an advancement of age, the prevalence of any toxicity increased. For example, farmers aged 65 years or more had the highest prevalence of toxicity symptoms (62%) compared to all those between 45 and 60 years of age (48%) and those in the younger age of 15\u0026ndash;29 years and 30\u0026ndash;44 years (29% in each group). Male farmers showed higher prevalence of symptoms of any toxicities (57%), compared to female farmers (15%). The prevalence of toxicity symptom was highest (61%) among the cultivators (totally dependent on agriculture for their livelihood), followed by agricultural labours (29%) and other farmers (those farmers working in non-agricultural sector along with farming) (19%). The toxicity symptoms were highest among farmers who smoke (81%) and have drinking habit (69%). Although education and wealth did not show significant association, there were considerable variation in toxicities in different categories.\u003c/p\u003e \u003cp\u003eFurther, the current study also assessed the prevalence of two most common toxicities (skin rash and vomiting) to understand the association with the major sociodemographic and economic characteristics of farmers in Tripura (Table\u0026nbsp;\u003cspan refid=\"Tab2\" class=\"InternalRef\"\u003e2\u003c/span\u003e). Age, sex, education, farmer type, smoking and drinking were significantly associated with skin rash. Farmers aged 45\u0026ndash;60 years and aged 65 or more had the highest prevalence of skin rashes (20%). The prevalence of skin rash was higher among males (22%) than in females (7%). One fourth of the cultivators experienced skin rash, whereas, the agricultural labous and other farmers reported to have prevalence rate of eight percent. The prevalence of the vomiting (second highest toxicity symptoms) also increased with increased age of farmers. Similar to any toxicity and skin rash, the prevalence of vomiting was also higher among male farmers (14%) compared to female farmers (7%) and the result was significant. The prevalence of vomiting among the widow/seperated farmers was higher (26%) compared to other which showed much lower levels. The association of the vomiting with caste and marital status was significant. The prevalence rate of vomiting was highets among cultivators (16%), followed by other farmers (9%) and agricultural labourers (4%). Caste, smoking and drinking are significantly associated with any toxicity. As age increases, the odds of reporting any toxicity increased. The farmers aged 65 years or more were 85% more likely to have any symptoms of toxicity compared to farmers aged 15\u0026ndash;30 years. The results suggested that male farmers were 4.6 odd times more likely than female farmers to develop any toxicity. When compared to farmers who were not married, married farmers seemed to have 20% greater chances of suffering from any toxicity symptoms, while widowed or separated farmers had 2.6-odd times greater likelihood. In comparison to the cultivator, agricultural labourers and other worker-farmers were marginally less likely to experience toxicity. Toxicity symptoms were 2.7 odd times more likely to be experienced by smokers rather than non-smokers. Further, farmers who never chewed tobacco were less likely to be at risk of any toxicity than farmers who consumed any tobacco. Although education did not show significant association, there were considerable variation in any toxicity in different levels of education.\u003c/p\u003e \u003cp\u003eCaste and occupation are the only variables significantly associated with skin rash. Skin rashes became more prevalent as persons aged. Compared to farmers aged 15\u0026ndash;30 years, those aged 30\u0026ndash;44 years had an increased risk of skin rash by 9%, and among farmers aged 45\u0026ndash;60 years the odds of reporting skin rash was 53% higher. The odds of skin rash among male farmers were twice compared to female farmers. Compared to cultivators, agricultural labourers had a 56 percent lower risk of developing skin rash, whereas, other farmers had a 44 percent lower risk. In comparison to non-smokers, farmers who smoke were substantially more likely to experience skin rash symptoms. There was no significant relationship between any characteristics and vomiting in the current study. However, with advancement of age the odds of reporting vomiting increased. Male farmers reported 1.7 odd times higher prevalence of experiencing vomiting than females. On the other, farmers with relatively low level of education, lack/adequate training and knowledge, attitude and practice (KAP) related the use of agricultural practices. Farmers use huge amount of inorganic pesticides and/or fertilizers (agrochemicals) without any training and proper personal safety to fulfil the increased food demand, resulting into health problems. Therefore, the current study aims to assess the prevalence of toxicity symptoms among the farmers due to use of agrochemicals in the north-eastern state of Tripura, India.\u003c/p\u003e\u003ch2\u003eMethods\u003c/h2\u003e \u003cp\u003eTo fulfil the objective of the study, primary data was collected from 995 farmers of Dhalai district using multistage stratified random sampling. A semi-structured questionnaire was developed and pre-tested. Bi-variate analysis was used to assess the pattern of toxicity and Chi-square test was applied to test the significance of association with the factors under consideration. Finally, logistic regression was applied to examine the factors independently associated with different toxicities.\u003c/p\u003e\u003ch2\u003eFindings\u003c/h2\u003e \u003cp\u003e: Above one-third (39%) of the farmers reported toxicity symptoms while using agrochemicals. Prevalence of symptoms of any toxicities was much higher among males than females. The major toxicity symptoms were skin-rash (16%), vomiting (12%), skin redness (10%), headache (7%), and excessive sweating (6%). Age, sex, occupation, smoking and drinking behaviour were significantly associated with the symptoms of toxicities.\u003c/p\u003e\u003ch2\u003eConclusions\u003c/h2\u003e \u003cp\u003eThere is need for increased awareness level and training to accustom the farmers with safe professional practices and personal hygiene.
