Despite the high expectations and interest in adopting and using electronic medical record in Ethiopia, its overall adoption rate is low. Therefore, this study aims to assess attitude and associated factors towards implementation of electronic medical record among health professionals in Ethiopian, 2023 . Methods: Multicenter cross-sectional study design was conducted. Systematic random sampling technique was used to select a total of 397 samples. Data was entered to Epi data 4.6 and analyzed by SPSS version 25. A binary logistic regression was performed, and variables with p-values <0.25 were analyzed with multivariable logistic regression. A p-value of <0.05 and adjusted odds ratio with 95% confidence interval was used to declare the associated factors. Result: In this study, favorable attitude towards implementation of electronic medical record system was 184 (48.2%). Age 29 years (AOR: 3.05; 95% CI: 1.58, 5.9), computer literacy (AOR: 2.66; 95% CI: 1.16-6.09), training on the EMR system (AOR: 2.87; 95% CI: 1.80-4.56)) and health professionals’ knowledge on EMR (AOR: 1.8; 95% CI: 1.10-2.96) were significantly associated with attitude. Conclusion: In this study, the attitude towards the implementation of electronic medical record is relatively low. Age, computer literacy, knowledge, and training access to EMR were factors significantly associated with the attitude towards electronic medical record system. Therefore, I would recommend the hospitals and ministry of health to enhance health professionals ‘attitude by improving their computer literacy and knowledge on EMR to scale up EMR use. Electronic Medical Record Attitude towards Electronic medical record implementation Ethiopia Background Electronic medical records (EMR) are far more than a simple alternative to paper medical records, as they allow for far better communication and coordination amongst different healthcare professionals and departments to help provide the best possible service ( 1 ). EMR usually contain basic information about your medical history, including treatment data and past medical history that has been collected by your medical practice. An EMR system acts as a central interface for all physicians to communicate and facilitate care. Nowadays, EMRs are becoming standard; in fact, they’re slowly but surely becoming a part of a new government requirement that is centered on the safe collection, storage and access of patients’ medical data ( 2 ). Despite the high expectations and interest in adopting and using EMR systems worldwide, its overall adoption rate is relatively low, especially in the resource-limited countries where high diseases prevalence and incidence rates are predominant( 3 ). As indicated by various studies, the adoption and use of EMR systems in developing countries is in its early stage for several reasons ( 4 ). The available evidence indicated that there were many factors that hindered the widespread adoption of an EMR in sub-Saharan Africa. These were high costs of procurement and maintenance of the EMR system, lack of financial incentives and priorities, poor electricity supply and internet connectivity, poor health professionals’ attitude towards the technology and primary user’s limited computer skills. Wide adoption of an Electronic Health Record (EHR) in sub-Saharan Africa region requires a lot more effort than what is assumed because of the current poor level of technological development, lack of required computer skills, and limited resources ( 5 ). Although work has been undertaken by institutions or countries on some form of a computerized patient healthcare information system, as yet not many hospitals have successfully introduced an electronic health record with clinical data entry at the point of care ( 1 ). Various health facilities in developing countries are using EMR systems in varying degrees due to several reasons ( 2 , 6 ). In Ethiopia, though most frequently identified EMR barriers are lack of funding, lack of capacity, infrastructure constraints, legal aspects, resistance to computer technology, computer systems literacy, strong resistance to change by many healthcare professionals, the issue of privacy and confidentiality, and inadequate knowledge and attitude of healthcare professionals on EMR system ( 7 ); still the health professionals’ attitude towards the implementation of EMR in those fully EMR implementing hospitals is a big challenge. Even though digitalizing the health care system is the key priority that World Health Organization (WHO) and Federal Ministry of Health (MOH) working through, still there are a lot of challenges towards the implementation of EMR; of which health professionals attitude towards EMR implementation is one of the major challenge ( 1 , 3 , 8 ). Therefore, this study will have paramount importance for healthcare professionals, the health institutions, policy maker, EMR software owners, further researchers and the community at large by assessing the health professionals’ attitude status and associated factors towards the implementation of EMR. The Ministry of Health has been working on the development and cascading of health information systems and digital health-related national documents. These include the Information Revolution Roadmap II (2020–2029), Information Revolution Strategic Plan (2018–2025) and ICT Policy and Digital Health Strategy (2020-24) ( 7 ). So the finding from this research will help for the accomplishment of the settled objectives. Methods and Materials Study design and setting A multicenter based cross-sectional study design was conducted in north Shewa Zone, Amhara region and Addis Ababa Ethiopia from June 6, 2022 to July 6, 2022. Three governmental hospitals that implemented full EMR (Debre Berhan University Hakim Gizaw Hospital, North Shoa, Amhara region; Yekatit 12 Hospital Medical College and Tirunesh Bejing General Hospital, Addis Ababa), are included in to the study. Population and eligibility criteria The source population was all health professionals working in all full EMR implemented Ethiopian hospitals. The study populations are health professionals working in Yekatit 12, Tirunesh Bejing and Debre Berhan University Hakim Gizaw Hospitals. The study units are each health professionals selected by systematic random sampling method from the above three hospitals. All health professionals who had work experience greater than 6 months were included into the study. Health professionals who were on maternal leave, annual leave, working as free service and critically ill with sick leave were excluded. Sample size and sampling procedure Single population proportion formula was used to determine sample size. The sample size at 95% confidence level (Zα/2 = 1.96) and absolute precision or margin of error 5% (d = 0.05) is the one calculated from 56.1% ( 9 ). Considering 5% non-response rate the final sampled size calculated was 397. There are three fully EMR piloted hospitals (Yekatit 12 Hospital, Tirunesh Bejing Hospital and Debre Berhan University Hakim Gizaw Hospital) which are the only government hospitals that are currently exercising fully automated EMR despite a lot of challenges. Systematic random sampling technique was used to select the study participants. First the total sample size was divided for three hospitals based on the allocation formula. Based on this Yekatit 12 hospital has 1265 health professionals which account 56% of the sample size that is 221 samples. In similar way Tirunesh Bejing Hospital has 611 health professionals which account 27% of the sample size, that is 108 samples and Hakim Gizaw Hospital has 380 health professionals which account 17% of the sample size, which is 68 samples. Finally, the study unit was selected using systematic random sampling from each profession based on the proportion calculated. From yekatit 12 hospital medical college; specialist and above (26), General practitioners (34), Nurse/Midwifes (110), Laboratory ( 15 ), Pharmacy ( 14 ), Radiology ( 7 ) and Anesthesia ( 15 ), from Tirunesh Bejing general hospital; Specialist and above ( 11 ), General practitioners ( 14 ), Nurse/Midwifes (57), Laboratory ( 8 ), Pharmacy ( 8 ), Radiology ( 3 ) and Anesthesia ( 7 ) and from Debre Berhan University Hakim Gizaw Hospital; Specialist and above ( 9 ), General practitioners ( 7 ), Nurse/Midwifes (26), Laboratory ( 9 ), Pharmacy ( 9 ), Radiology ( 3 ) and Anesthesia ( 5 ) were included in the sample. Study variables The dependent variable is the attitude towards EMR implementation. The independent variables includes; socio-demographic variables: organizational factors; technology related factors; and personal factors. Definition of terms Level of knowledge : This is the understanding level of health professionals about EMR and its benefits, as compared to paper based medical record system. Based on total score, knowledge level on EMR was categorized into good knowledge and poor knowledge. Good knowledge: Those who answered mean and above from 12 knowledge related yes/no type of questions were classified as knowledgeable and poor knowledge those participants whose response were less than the mean from questions that were asked in knowledge. Attitude Is the intention of participants towards EMR implementation. Attitude for EMR implementation was assessed through 18 questions. Those who answered above the median were labeled as having favorable attitude and those who answered less than median were labeled as having unfavorable attitude. Computer literacy .Respondents that can perform at least basic Microsoft office applications (MS Word, PowerPoint, Excel, Access and Internet services) were considered as having computer literacy. Income Low income (first quartile), medium income (second quartile) and high income (third quartile) was used to classify average income level in this study. EMR training If refreshment training was given to health professionals for one day at least two times per year. Management support if there is EMR committee and conduct regular monitoring and evaluation, take immediate action whenever problems happened. Electric power interruption If there is electric power interruption at least once per day. Data measurement and tool Data collection was conducted by using a pretested, structured and self-administered questionnaire after taking consent from study subjects. The data were collected by 5 (3 BSc nurses) trained health professionals. There were two trained General Practitioners who supervised the data collection process. Data quality management To assure quality of data, a standard questionnaire obtained from different published articles (( 9 – 13 ) were used for this study. A two hours training was given to data collectors on the objective of the study, the questionnaire and extent of explanations and the way of keeping the privacy and confidentiality of the study subjects. The tool was prepared and distributed in English language. Pretest was conducted on a similar setting at St, Paulos hospital, Addis Ababa Ethiopia before the actual data collection time on 5% of the sample size in order to check the reliability of the instrument and to modify the questionnaire accordingly. During the actual data collection process, supervisors (General Practitioners) were cross checked the data collectors on randomly every day for questionnaire completeness. Data processing and analysis The collected data was cleaned, coded and entered to Epi data software (4.6) which secured further data quality by reducing errors made while data entry. Then, entered data was exported to SPSS (version 25) for further data cleaning and data analysis. In order to identify determinants of dependent variable, first chi-square test was done for each independent variable then those variable which was statistically significant in x2 test (P-value < 0.05) was analyzed by bivariate analysis; the variables with P < 0.25 during bivariate analysis were a candidate for multivariate analysis and variables with P < 0.05 in multivariate analysis were the final determinants of dependent variables. And the degrees of association between dependent and independent variables was assessed using AOR at 95% CI. Model fitness was checked by using Hosmer and Lemeshow test, and it was fitted since the p-value was above 0.05 (0.80). Independency assumption was also checked by facility level using Intra Class Correlation Coefficient and it was fulfilled, since the ICC value was below 10%, which was 0.5% Result Socio-demographic characteristics of study participants A total of 397 study participants participated in the study with response rate of 96.2%. The majority (61.8%), of the respondents was male. The mean age of the respondents was 30.04 (± 4.43 SD) years. Nearly two-third of the participants (64.4%), had less than 6 years of work experience and 69.6% of the respondents were BSC holders. Majority of the study participants (88.7%) had computer literacy (Table 1 ). The majority of respondents are aware of the potential functions of EMR in handling patient data, review patient problems, obtain test result, allow private pass ward, need local network and updating patient information. About three-fourths of respondents know that EMR allows requesting laboratory orders, prescriptions, and has disease code and data backup systems (Table 2 ). Among the study participants, they strongly agree that EMR can save cost (30.6%), easy to enter data (26%), increase timely decision (24.3%), needs training (43.5%), decrease medical error (26.2%), increase productivity (27.7%) and good for audit trial (28.8%). About 40.8% and 39.5% of the respondents were strongly agreed and agreed to encourage other hospitals to implement EMR respectively (Table 3 ). In the bivariate analysis those having P-value of ≤ 0.25 were a candidate to be analyzed by multivariate analysis. After controlling for other variables, the multi variable logistic regression analysis showed that, respondents with age group ≤ 29 years old were three times more likely to have good attitude towards EMR implantation compared to age group greater than ≥ 35 year (AOR: 3.05; 95% CI: 1.58, 5.9). In addition, respondents who took an EMR training were 2.8 times more likely to have positive attitude towards EMR implementation as compared to those who didn’t took the training (AOR: 2.87; 95% CI: 1.80–4.56). Health professionals who had computer literacy, had 2.7 times more favorable attitude to EMR implementation as compared to those who had no computer literacy (AOR: 2.66; 95% CI: 1.16–6.09) and having good knowledge for EMR had 1.8 times more likelihood to have favorable attitude to EMR as compared to having poor knowledge (AOR: 1.8; 95% CI: 1.10–2.96) (Table 5 ). Discussion This study assessed health professionals’ attitudes and associated factors about electronic medical record systems at EMR implementing Ethiopian hospitals. This study revealed that nearly half of the respondents 48.2% (CI: .431-.532) had favorable attitude towards EMR implementation. The finding is lower than the studies done in USA 97% ( 14 ), Saudi 70% ( 15 ) and South Africa 67.2% ( 16 ). The difference might be health professionals who are working at high-resource countries and who have computer experience in their day-to-day life might understand the relative advantage of electronic medical record systems in health care. The current finding in attitude status, also lower than the study done in Northern Ethiopia, 50.6% ( 9 ), St. Paulos hospital 52.1% ( 13 ), Amhara region hospitals 58.3% ( 10 ) and Eastern Ethiopia 72.8% ( 11 ); the difference might be health professionals who participated in the current study were from hospitals where full EMR is implementing and where as other studies were conducted in partially and none EMR implementing hospitals. From this study, the predictor variables for health professionals’ which had statistically significant association with attitude towards electronic medical record systems were; EMR training, computer literacy, respondents age less than thirty and good knowledge in EMR. This finding is supported by studies conducted in Sub Sahara ( 5 ), Eastern Ethiopia ( 11 ) and Northern Ethiopia ( 17 ); but in Canada; the motivation of physicians, user satisfaction, coaching and peer mentoring, EMR experience, gender, physician perception, team-based care, adequate technical support and training, sharing resources, financial incentives, and policies to increase EMR use all had a favorable attitude on health professionals to use advanced EMR ( 18 ). This difference might be due to the setting difference where computer literacy and knowledge were not a problem. In the current study, variables like work experience, educational status, type of profession, management support, computer access, presence of EMR manual, presence of ICT and focal person for EMR had no significant association with health professionals’ attitude while in other studies like Sub-Sahara; poor network, high cost and maintenance ( 19 ); work experience ≤ 6 years, meeting on EMR, presence of EMR manual ( 20 ); computer access ( 10 ); hospital lack of management commitment and poor network had significant association with attitude status of health professionals towards EMR implementation. The difference might be due to management concern and commitment difference, budget difference among health institutions. As a strength, this study includes all hospitals that were fully implementing EMR in Ethiopia. However, since setups are different the study doesn’t include non-governmental hospitals, so it may be generalized for this setups. Conclusion This study revealed that attitude of health professionals towards the implementation of EMR was relatively low. Health professionals’ age, knowledge on EMR, computer literacy and training on EMR were significantly associated with attitude towards EMR system. Therefore, I would recommend the hospitals and ministry of health to enhance health professionals‘ attitude by improving their computer literacy and knowledge on EMR to scale up EMR use. After controlling for other variables, the multi variable logistic regression analysis showed that, respondents with age group\u0026thinsp;\u0026le;\u0026thinsp;29 years old were three times more likely to have good attitude towards EMR implantation compared to age group greater than \u0026ge;\u0026thinsp;35 year (AOR: 3.05; 95% CI: 1.58, 5.9). In addition, respondents who took an EMR training were 2.8 times more likely to have positive attitude towards EMR implementation as compared to those who didn\u0026rsquo;t took the training (AOR: 2.87; 95% CI: 1.80\u0026ndash;4.56). Health professionals who had computer literacy, had 2.7 times more favorable attitude to EMR implementation as compared to those who had no computer literacy (AOR: 2.66; 95% CI: 1.16\u0026ndash;6.09) and having good knowledge for EMR had 1.8 times more likelihood to have favorable attitude to EMR as compared to having poor knowledge (AOR: 1.8; 95% CI: 1.10\u0026ndash;2.96) A p-value of \u0026lt;0.05 and adjusted odds ratio with 95% confidence interval was used to declare the associated factors.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResult:\u003c/strong\u003e In this study, favorable attitude towards implementation of electronic medical record system was 184 (48.2%). Age \u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;\u0026nbsp;29 years (AOR: 3.05; 95% CI: 1.58, 5.9), computer literacy (AOR: 2.66; 95% CI: 1.16-6.09), training on the EMR system (AOR: 2.87; 95% CI: 1.80-4.56)) and health professionals’ knowledge on EMR (AOR: 1.8; 95% CI: 1.10-2.96) were significantly associated with attitude.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eConclusion: \u003c/strong\u003eIn this study, the attitude towards the implementation of electronic medical record is relatively low.
