Abstract

To evaluate the relationship between the position of uterus on transvaginal ultrasound (TVS) and the presence of deep infiltrating endometriosis (DIE) at laparoscopy. Multi-centre prospective study (Jan 09-Feb 13). Women with chronic pelvic pain (CPP) +/− history of endometriosis and due for laparoscopy were included. All women underwent detailed TVS to assess the position of the uterus: anteverted, anteverted/retroflexed, axial, retroverted or retroverted/anteflexed. These women then underwent laparoscopic surgery to determine the location of DIE nodules (rectosigmoid, anterior rectum, retrocervical, rectovaginal, vaginal, right/left uterosacral ligament). The relationship between the position of the uterus and the presence of DIE nodule was then assessed. 189 women underwent TVS and laparoscopy, however three were lost to follow up. Uterine positions were: 139/189 (73.5%) anteverted, 3/189 (1.6%) anteverted/retroflexed, 41/189 (21.7%) retroverted, 1/189 (0.5%) retroverted/anteflexed and 2/189 (1.1%) axial. There were no statistically significant relationships between the position of the uterus and the presence of DIE nodules. Uterine position does not seem to correlate with the presence of DIE nodules in the posterior compartment.
