Abstract

Emerging clinical observations and limited vascular research suggest that pelvic venous hypertension and reflux—particularly from syndromes such as Nutcracker Syndrome (left renal vein compression) and May–Thurner Syndrome (left iliac vein compression)—may play an under-recognized role in the dissemination and progression of endometriosis beyond the pelvis. This paper presents a structured hypothesis linking venous compression → pelvic congestion → extrapelvic implantation of endometrial tissue. It proposes an early-screening paradigm for young women with endometriosis or infertility and calls for collaboration between gynecology and vascular medicine. This version serves as a timestamped draft establishing authorship for future clinical development and peer review.
