EP24.25: Prospective validation of preoperative and surgical endometriosis staging systems using mobile device interface

Abstract
With the ESHRE 2022 endometriosis guideline outlining the ‘gold standard’ for endometriosis diagnosis is now imaging, the prior mantra of “see and treat” at time of laparoscopy now has a new meaning. The rASRM, AAGL, UBESS, #ENZIAN and EFI staging systems, among others, have been used preoperatively. We aim to prospectively validate preoperative staging and surgical staging by contemporaneously using a mobile A. This prospective multi-centre observational study (December 2023 and ongoing), recruited symptomatic patients undergoing ultrasound according to IDEA methodology scheduled for surgery. Mapping of deep and superficial endometriosis on ultrasound (US) resulted in real time calculation of ultrasound staging (ASRM-US, AAGL-US, UBESS, EFI, #ENZIAN) and at the completion of surgery real time calculation of surgical stage (ASRM, AAGL, UBESS, EFI, ENZIAN). These outcomes were then compared. At time of analysis after two months recruitment the cohort includes 10 subjects that have undergone clinic review, imaging and surgery (see table 1). In the early recruitment stages, we are demonstrating the feasibility of our ongoing project in assessing the accuracy of multiple staging systems in predicting surgical complexity.
