Abstract

In ovarian cancers, no valuable prognostic factors are available so far for predicting biological behaviour and clinical outcome. During the last years, overexpression of c-erbB-2-oncogene has been reported to be a prognostic factor in various human carcinomas. Its prognostic value in ovarian cancer is still a matter of controversy. The records and histological specimens of 243 patients with primary ovarian cancer were reviewed. Overexpression of c-erbB-2-oncogene encoded transmembrane protein p185 was determined immunohistochemically using polyclonal and monoclonal antibodies. Of 243 investigated tumours, 45 (19%) were positive. We examined the relationship between p185 and clinical stage, histological type, grading and prognosis. p185 overexpression differs within the histological subtypes of tumours. No correlation was found between p185 overexpression and stage or histological grade. The follow-up data demonstrate that patients with positive staining for p185 have a significantly worse prognosis (p = 0.001) than those with negative staining. These results suggest that overexpression of c-erbB-2 oncogene is associated with higher biological aggressiveness and unfavourable course of disease.

In den letzten Jahren wurde über die prognostische Bedeutung der Amplifikation respektive Überexpression des Onkogens c-erbB-2 bei verschiedenen humanen Karzinomen berichtet, wobei die Bedeutung für das Ovarialkarzinom bis dato unterschiedlich bewertet und kontrovers diskutiert wurde. In der vorliegenden Studie wurde das Karzinomgewebe von 243 Patientinnen, die wegen eines primären Ovarialkarzinoms in der Zeit zwischen 1982 und 1991 in der UFK Göttingen onkologisch behandelt wurden, untersucht und die Überexpression des c-erbB-2-codierten Transmembranproteins immunhistochemisch bestimmt. In 19% (45 Fälle) der 243 analysierten Karzinome ließ sich eine p185-Überexpression nachweisen. Der prozentuale Anteil p185-positiver Befunde variierte innerhalb der verschiedenen histologischen Typen. Eine Korrelation zum Tumorstadium und den histologischen Differenzierungsgraden konnte nicht nachgewiesen werden. Patientinnen mit p185-positiven Tumoren hatten eine signifikant schlechtere Prognose (p = 0,001) mit einer medianen Überlebenszeit von 19 Monaten im Vergleich zu 32 Monaten für p185-negative Tumoren. Die vorliegenden Resultate erlauben die Schlußfolgerung, daß die Überexpression des Onkogens c-erbB-2 beim Ovarialkarzinom ein Kollektiv mit besonders ungünstiger Tumorbiologie und einer signifikant schlechteren Prognose charakterisiert.
