We introduce a novel framework, the Equivariant Spatiotemporal Transformer with MDL-Guided Feature Selection (EST-MDL), which integrates group-theoretic symmetries, Kolmogorov complexity, and Minimum Description Length (MDL) principles. By enforcing spatial and temporal symmetries (e.g., translations and rotations) and leveraging MDL for robust feature selection, our model achieves improved generalization and interpretability. Evaluated on three realworld PET datasetsโLUNG-PET, BRAIN-PET, and BREAST-PETโour approach achieves AUCs of 0.94, 0.92, and 0.95, respectively, outperforming CNNs, Vision Transformers (ViTs), and Graph Neural Networks (GNNs) in AUC, sensitivity, specificity, and computational efficiency. This framework offers a robust, interpretable solution for malignancy detection in clinical settings. I. INTRODUCTION Dynamic Positron Emission Tomography (PET) scans capture detailed spatiotemporal patterns of radiotracer uptake, enabling early and accurate malignancy detection critical for improving patient outcomes [ 1 ]. However, the high cost of PET imaging, limited sample sizes, and the risk of false positives in noisy, high-dimensional data necessitate sophisticated modeling approaches. Traditional deep learning models like Convolutional Neural Networks (CNNs) and Vision Transformers (ViTs) often struggle with these challenges, as CNNs lack global context and ViTs are prone to overfitting due to insufficient inductive biases for geometric transformations [ 2 ]. To address these issues, we propose the Equivariant Spatiotemporal Transformer with MDL-Guided Feature Selection (EST-MDL). Our approach combines group-theoretic equivariance to enforce robustness to spatial and temporal trans-formations, Kolmogorov complexity for model simplicity, and MDL principles for principled feature selection. This results in a model that is both accurate and interpretable, suitable for clinical decision support. Figure 1 illustrates the proposed architecture. Our contributions are: A novel equivariant transformer architecture tailored to the symmetries of dynamic PET data. An MDL-guided feature selection method using Kolmogorov complexity to optimize feature sets. Extensive evaluation on three real-world PET datasets, achieving superior performance over state-of-the-art models. II. BACKGROUND A. Group Theory in Neural Networks Group theory provides a mathematical framework to model symmetries in data. A group G is a set with a binary operation satisfying closure, associativity, identity, and invertibility. For PET scans, we consider the group G = โ 3 โ SO (3) ร โ, combining 3D translations (โ 3 ), rotations ( SO (3)), and temporal shifts (โ). A group action g . x transforms input data x โ โ d (e.g., a PET scan voxel) by g โ G . A neural network f : โ d โ โ m is equivariant if: where g ยท f ( x ) denotes the action of g on the output. This ensures that transformations like rotating a PET scan produce correspondingly transformed predictions, reducing parameters and improving generalization [ 3 ]. For example, in 3D CT segmentation, equivariant networks have improved robustness to patient orientation [ 4 ]. To illustrate, consider a 3D rotation g โ SO (3) applied to a PET scan x . A standard CNN might learn rotation-specific features, requiring more parameters, whereas an equivariant network shares weights across rotations, ensuring: This property is critical for PET scans, where patient positioning or scanner alignment may vary. B. Kolmogorov Complexity and MDL Kolmogorov complexity K ( x ) of a data string x is defined as the length of the shortest program that outputs x on a universal Turing machine [ 5 ]. Formally: Where | p | is the program length, and U is a universal Turing machine. For example, a repeating pattern like x = 101010 has low K ( x ) (e.g., a short loop program), while random noise has high K ( x ) due to its lack of structure. In practice, K ( x ) is incomputable, so we approximate it using encoding lengths. The Minimum Description Length (MDL) principle extends this idea to model selection [ 6 ]. For a model M and data D , MDL seeks to minimize: where L ( M ) is the description length of the model (e.g., bits to encode parameters), and L ( D | M ) is the length of encoding D given M (e.g., negative log-likelihood). For feature selection, MDL balances the complexity of selected features with their ability to explain the data, aligning with Occamโs Razor. For PET scans, MDL helps select features that capture malignancy patterns (e.g., radiotracer uptake) without overfitting to noise. If ๐ฎ is a feature subset, we approximate: where | โฑ | is the total number of features, and M is the feature dimensionality. The data term L ( D | ๐ฎ) is typically the negative log-likelihood under a logistic model: where y i is the label (malignant/benign), and p ( y i | ๐ฎ, x i ) is the predicted probability. C. Dynamic PET Scans Dynamic PET scans produce a sequence of 3D volumes over time, represented as X โ โ T ร H ร W ร D , where T is the number of time frames, and H, W, D are spatial dimensions. The goal is to classify each scan as malignant ( y = 1) or benign ( y = 0) based on spatiotemporal patterns of radiotracer uptake [ 1 ]. The high dimensionality ( T ยท H ยท W ยท D ) and noise necessitate models that exploit symmetries and select informative features. III. METHODOLOGY A. Equivariant Spatiotemporal Transformer Our EST-MDL model extends the Vision Transformer (ViT) [ 7 ] to handle dynamic PET scans while enforcing equivariance to spatial and temporal transformations. We represent a PET scan X โ โ T ร H ร W ร D as a sequence of spatiotemporal patches X = { x 1 , โฆ, x N };, where each patch x i โ โ P ร P ร P ร C is a 3D cube with P voxels per spatial dimension and C channels (e.g., radiotracer intensity over time). Patches are extracted after spatial downsampling by a factor of s to reduce dimensionality, such that . 1) Equivariant Attention Design The transformer encoder is designed to be equivariant to the group G = โ 3 โ SO (3) ร โ, which includes 3D translations, rotations, and temporal shifts. For a group element g = ( t, R, s ) โ G , where t โ โ 3 is a translation, R โ SO (3) is a rotation, and s โ โ is a temporal shift, the action on a patch x i is: where v โ โ 3 is a spatial coordinate, and ฯ โ โ is a time point. To ensure equivariance, we use steerable filters [ 3 ] to compute patch embeddings. A steerable filter ฯ : โ 3 โ โ C transforms under R โ SO (3) as: where D ( R ) is a representation matrix (e.g., a rotation matrix for vector fields). The embedding of patch x i is: which is equivariant since: These embeddings are fed into the transformer, where the self-attention mechanism is modified to preserve equivariance. For query Q , key K , and value V matrices derived from embeddings E = { e 1 , โฆ, e N };, the attention is: where Q, K, V are transformed under g โ G using group convolutions. Specifically, for a group element g , the attention weights satisfy: ensuring that the attention map transforms consistently with the input. This is achieved by projecting Q, K, V onto irreducible representations of G , as in [ 3 ]. The output embeddings Z = { z 1 , โฆ, z N }; are aggregated via global attention: where W is a learned weight matrix, and z โ โ d is the scanlevel representation. Figure 1 illustrates this pipeline. B. MDL-Guided Feature Selection From the transformer output Z , we extract a feature set โฑ = { f 1 , โฆ, f M };, where each f i โ โ d is a feature vector. The goal is to select a subset ๐ฎ โ โฑ that minimizes: where L (๐ฎ) is the description length of the feature set, and L ( D | ๐ฎ) is the data encoding length. We approximate L (๐ฎ) as: Where counts the ways to choose| ๐ฎ |features from M , and log 2 d accounts for encoding each featureโs dimensionality. The data term is: where ฯ is the sigmoid function, and W ๐ฎ , b are parameters of a logistic regression model over ๐ฎ. The optimization problem is: Since exact optimization is NP-hard, we use a greedy algorithm, iteratively adding the feature f i that most reduces L (๐ฎ, D ). The runtime complexity is O ( M ยท| D | k ), where k is the number of iterations (empirically, k โ 10โ20 for convergence). Greedy MDL has theoretical guarantees of nearoptimality under certain conditions [ 6 ]. Model Training The selected features ๐ฎ are fed into a classification head: where W c , b c are learned parameters. The model is trained end-to-end with cross-entropy loss: We optimize using gradient descent, with the loss gradient: This ensures the model learns to prioritize features that distinguish malignant from benign scans. IV. EXPERIMENTS A. Datasets We evaluate on three real-world dynamic PET datasets: LUNG-PET : 500 scans (300 benign, 200 malignant), 20 time frames, 128 ร 128 ร 64 voxels. BRAIN-PET : 400 scans (250 benign, 150 malignant), 15 time frames, 96 ร 96 ร 48 voxels. BREAST-PET : 600 scans (350 benign, 250 malignant), 25 time frames, 128 ร 128 ร 80 voxels. B. Baselines We compare against: CNN : 3D ResNet-50 [ 8 ]. ViT : Standard Vision Transformer [ 7 ]. GNN : Graph Neural Network with spatiotemporal graphs [ 9 ]. Random Selection : ViT with random feature selection (for ablation). C. Evaluation Metrics We report Area Under the Curve (AUC), sensitivity, specificity, and F1-score on a 20% test set, with 95% confidence intervals (CIs) computed via bootstrapping. Statistical significance is assessed using the Wilcoxon signed-rank test ( p < 0.05). D. Implementation Details Implemented in PyTorch, trained on an NVIDIA A100 GPU with batch size 16, Adam optimizer, learning rate 10 โข 4 , patch size P = 16, and 100 epochs with early stopping. V. RESULTS Table I shows that EST-MDL outperforms baselines across all datasets, achieving AUCs of 0.94 (LUNG-PET), 0.92 (BRAIN-PET), and 0.95 (BREAST-PET) with p < 0.05. The F1-score is particularly strong on the imbalanced BRAIN-PET dataset, reflecting robust handling of class imbalance. Figure 2 shows how AUC varies with the number of selected features, peaking at an optimal subset size. Figure 7 shows the trade-off between AUC and parameter count, with EST-MDL achieving high performance with fewer parameters. Figures 3 , 4 , and 5 provide additional insights into model performance and efficiency. Figure 6 visualizes the energy landscape in phase space with overlaid feature evolution trajectories. DISCUSSION EST-MDLโs strong performance stems from its ability to capture spatiotemporal patterns while selecting compact, meaningful features, as visualized in Figure 1 . The equivariant transformer leverages long-range dependencies, unlike CNNs, which focus on local patterns, and ViTs, which lack geometric priors. Mathematically, the equivariance property ensures that: reducing the need for data augmentation and improving robustness to patient positioning. The MDL-guided feature selection, detailed in Figure 2 , optimizes the trade-off between model complexity and data fit, selecting features that align with physiological biomarkers, such as regions of high radiotracer uptake. For example, in LUNG-PET, MDL prioritized features corresponding to tumor boundary dynamics, consistent with clinical indicators of malignancy [ 1 ]. However, limitations exist. The computational cost of 3D rotation-equivariance, involving matrix operations over SO (3), scales as O ( n 3 ) for n -dimensional representations, though mitigated by downsampling. Large labeled datasets are required, which may limit applicability in rare cancers. Future work could explore self-supervised pretraining [ 10 ] to reduce data needs or extend EST-MDL to multimodal imaging (e.g., PET-MRI) [ 11 ]. VII. CONCLUSION We introduced EST-MDL, a framework for malignancy detection in dynamic PET scans, combining equivariant transformers with MDL-guided feature selection. By respecting data symmetries ( Figure 1 ) and prioritizing simplicity ( Figure 2 ), it achieves AUCs of 0.94โ0.95 across three datasets, surpassing CNNs, ViTs, and GNNs, as shown in Figures 3 , 4 , 5 , and 6 . This approach offers a practical, robust tool for oncology, with potential for broader imaging applications.
