Case Summary: A 61-year-old man had off-pump CABG using great saphenous vein conduits from both legs due to idiopathic pulmonary fibrosis-pattern ILD and triple-vessel coronary artery disease (TVD). Significant epistaxis while on dual antiplatelet treatment, hypoalbuminemia, a protracted respiratory recovery, and sternal dehiscence necessitating rewiring within 9 days after CABG all added complications to his postoperative course. Final healing and release from the hospital occurred on the 24th postoperative day as a result of thorough interdisciplinary care. While coronary artery bypass grafting (CABG) can help ILD patients avoid lung damage from cardiopulmonary bypass, it comes with its own set of postoperative and technical issues, such as the possibility of bleeding, wound healing complications, and graft patency.
