Acute right heart failure after repair of subarterial ventricular septal defect: Iatrogenic right ventricular outflow tract obstruction

Abstract

We report a case of a 22-year-old man with subtruncal ventricular septal defect associated with a moderate reversal of aortic valve repair. Transesophageal echocardiography (TEE) revealed acute dilatation and insufficiency of the right ventricle with severe tricuspid regurgitation and iatrogenic right ventricular outflow tract obstruction. After emergency operation to clear the right ventricular outflow tract, TEE confirmed that the obstruction was relieved, and the right heart function and hemodynamics returned to normal quickly. This case suggests that acute right heart failure after cardiac surgery should be vigilant against mechanical obstructive factors, and TEE is a key tool for rapid diagnosis and guidance of treatment.
