Background Autism Spectrum Disorder (ASD) is a group of neurodevelopmental disorders characterized primarily by "impaired interpersonal communication" and "limited and repetitive behavioral patterns"[ 1 ]. This heterogeneity ranges across the full range of IQ and language functioning, diverse communication, social, and behavioral deficits [ 2 ]. Autistic adolescents may have poor adaptive behaviors even when they have normal cognitive abilities [ 3 – 7 ]. Therefore, even cases of high cognitive ability require diagnosis and support [ 8 ]. Stigma refers to prejudice against "undesirable kinds of attributes" [ 9 ]. Treatment stigma, in particular, refers to prejudice and discrimination against receiving treatment and support [ 10 ] and is a factor that inhibits help-seeking from treatment and support agencies [ 11 ]. The longer treatment stigma exists and extended treatment and support are refused, the worse the outcome is likely to be [ 12 ]. In a study on the relationship between autism and stigma, researchers in Japan developed and implemented an awareness program to reduce public stigma about disability; the program resulted in reduced stigma and improved knowledge [ 13 ]. Online classes on ASD improved college students' knowledge of ASD and reduced stigma against it [ 14 , 15 ]. However, these studies were solely about public stigma of ASD. Therefore, it is worthwhile to examine the impact of treatment stigma. Cognitive Behavioral Therapy (CBT) has been shown to be effective for both children and adults with ASD [ 16 – 18 ]. For example, it has been suggested that CBT reduces anxiety symptoms in young people and those with ASD [ 19 – 21 ], decreases symptoms of depression and stress [ 22 ], and is effective in treating comorbid Obsessive Compulsive Disorder in youth and adults with ASD [ 23 ]. The Program for Education and Enrichment of Relationship Skills (PEERS) for Young Adults led to an improvement in the social skills of adolescents and young adults [ 24 – 26 ]. Psychoeducation about disability has been shown to increase knowledge and positive attitudes toward disability [ 27 ]. Psychoeducational programs that teach Autistic adolescents about their disability increase knowledge and awareness of ASD [ 28 ]. The stigma around ASD undermines the mental health and quality of life of the guardians of Autistic adolescents as well [ 29 – 31 ]. A decrease in guardianship treatment stigma also contributes to improved parental mental health [ 32 , 33 ]. Following such research, we developed Aware and Care for my Autistic Traits (ACAT), a program that integrates CBT and psychoeducation to promote understanding of ASD for diagnosed adolescents and their guardians, and implemented it for two years starting in 2018 [ 34 ]. The results showed that the intervention group of children with ASD had significantly improved awareness and mental health, and reduced treatment stigma. ASD awareness significantly improved in the guardian intervention group as well, compared to the control group (Oshima et al., in submission). The purpose of this study is to determine whether the treatment stigma of Autistic adolescents and their guardians who participated in the ACAT clinical trial as the intervention group affects the effectiveness of the intervention in the trial. Two hypotheses were formulated in this study. (1) The higher the treatment stigma of autistic adolescents, the less CBT can improve AS traits and adaptive behavior. (2) The higher the treatment stigma of the guardians, the less the effects of CBT on the guardians of the autistic adolescents can be improved. Methods This study used data from the ACAT clinical trial (Oshima et al., in submission) with six weeks psychosocial education program by CBT at Chiba University. The intervention group in the randomization consisted of Autistic adolescents, n = 23 (16 boys, 7 girls; mean age 12.8 ± 2.2 years) and their guardians, n = 23 (one male, 22 females; mean age 44.8 ± 6.1 years). Eligibility and exclusion criteria were as follows. Demographic characteristics of participants are summarized in Table 1 and Table 2 . [Table 1 near here] [Table 2 near here] Relevant standards Autistic adolescents: 1) Those who have been diagnosed and notified by a psychiatrist that their primary diagnosis is ASD and who also meet the criteria for ASD on either the Autism Diagnostic Observation Schedule (ADOS-2) or Autism Diagnostic Interview-Revised (ADI-R); 2) A verbal IQ of 90 or higher on the Wechsler Intelligence Test; 3) The Strengths and Difficulties Questionnaires (SDQ) with a score of moderate (Some need) or higher; 4) The applicant must be between 10 and 17 years of age at the time of obtaining consent, and one parent or guardian must be able to participate in the CBT. Guardians of Autistic adolescents: Those whose guardians did not meet the diagnostic criteria for mental illness in the Japanese version of the Brief Structured Interview for Mental Illness. Exclusion Criteria: Organic brain disorder (including dementia), psychotic disorder, bipolar disorder, drug dependence, or other mental severe disorder complicated by psychosis, imminent suicide risk, antisocial behavior, or significant progressive physical illness. Endpoints used in this study The Autism Knowledge Quiz-Child (AKQ-C) and Autism Knowledge Quiz-Guardians/Parents (AKQ-P) [ 28 ], which is a five-item structured interview that measures understanding of ASD characteristics and 15 sections of the item knowledge quiz. Both child self-administered and guardian self-administered questions were asked to be answered. The Barriers to Access to Care Evaluation scale version 3 (BACEv3) [ 10 ] is a 30-item questionnaire that measures treatment stigma related to mental health and has a four-point scale ranging from "not at all" to "quite a bit." Children and guardians were each asked to respond. The Depression Self-Rating Scale for Children (DSRS-C) [ 35 ] is an 18-item self-assessment questionnaire for depression in children. The children were asked to rate their level of depression by answering "always," "sometimes," and "never.” The Strengths and Difficulties Questionnaire (SDQ) [ 36 ] is a self-administered questionnaire designed to comprehensively assess adjustment and mental health status from early childhood through adolescence. The questionnaire consists of 25 items, each of which requires a response of "yes," "somewhat yes," or "no." The rating is divided into three levels: “low need,” “some need,” and “high need.” Guardians were asked to fill in the form. In the Vineland Adaptive Behavior Scales Second Edition (Vineland-II) [ 37 ], children’s adaptive behavior is graded in four domains: "communication domain," "daily skill domain," "sociality domain," and "motor skills.” Three domains were graded except for "motor skills," where the evaluation target was up to 6 years old. A semi-structured interview was conducted with the guardians. The 12-item General Health Questionnaire (GHQ-12) [ 38 ] is a self-administered questionnaire that measures the presence of mental and physical problems in the health of the child from the first day of the child's life to the present and consists of 12 items to be answered using a four- point scale: "less than usual", "no more than usual", "rather more than usual", "or much more than usual". Guardians were asked to respond. The Parenting Resilience Elements Questionnaire (PREQ) [ 39 ] of guardians with developmental disabilities is a self-administered questionnaire that measures the strength of nurturing resilience, with 16 items answered using a seven-point scale ranging from "not at all true" to "very true." Guardians were asked to respond. Analysis method For hypothesis (1), the change in BACEv3 at pretest and the change in other tests at posttest in Autistic adolescents was calculated by Spearman rank correlation coefficients for the intervention group and the control group were calculated. For hypothesis (2), the change in guardians' BACEv3 at Pre and the change in other tests at Post were calculated by Spearman rank correlation coefficients were calculated for the intervention and control groups, respectively. All these analyses were performed using SPSS Statistics Version 27.0 (IBM Armonk, New York, USA). The p-value was set at .05 to determine significance. Results As a result of the hypothesis analysis (1), BACEv3 of Autistic adolescents in the intervention group was not correlated with the amount of change in any scale. As a result of the analysis of hypothesis (2), BACEv3 of guardians in the intervention group was significantly correlated with the AS traits knowledge of ASD and communication of adaptive behavior. There was a positive (r = 0.681, p < 0.01) and moderate correlation between baseline guardians' BACEv3 and the amount of change in AKQ-C "knowledge." There was a negative (r = 0.534, p < 0.05) and moderate correlation between baseline guardians' BACEv3 and the amount of change in Vineland II "communication" (receptive/expressive language, reading and writing) (Table 3 ). [Table 3 near here] Discussion The purpose of this study was to determine whether the treatment stigma of Autistic adolescents and their guardians affected the effectiveness of the ACAT intervention. The Spearman's rank correlation coefficient analysis showed that the BACEv3 of Autistic adolescents did not correlate with the change in any of the scales. Guardians' BACEv3 was significantly correlated with Autistic adolescents’ knowledge of ASD characteristics and communication of adaptive behavior. The study examined two hypotheses. The first hypothesis is that the higher the treatment stigma of autistic adolescents, the less CBT can improve autistic traits and adaptive behavior. Based on previous studies by Corrigan et al. (2004) and Gulliver et al. (2010), we predicted that high treatment stigma in Autistic adolescents would reduce the effect of CBT intervention, but in the present study, high treatment stigma in Autistic adolescents was uncorrelated with the amount of change in intervention. Therefore, this hypothesis was rejected. Thus, at least for Autistic adolescents, the individual's treatment stigma was not expected to affect treatment effectiveness. Secondly, the higher the treatment stigma of the guardians, the less the effects of CBT on the guardians of the autistic adolescents can be improved. Although there were changes in measures related to Autistic adolescents among guardians in the intervention group, there were no significant differences in the amount of change in measures related to guardians. This suggests that guardianship treatment stigma did not affect the guardians' intervention. Contrary to the prediction that high guardianship treatment stigma would reduce CBT effectiveness for guardians and Autistic adolescents, Autistic adolescents in the intervention group showed an increase in knowledge of ASD. This may be due to the high treatment stigma of the guardians, which may have led to a less open discussion of ASD between guardians and children. Furthermore, the Autistic adolescents’ knowledge at the pre-intervention time point may have been low to begin with. Additionally, there was a decrease in communication (receptive/expressive language, reading, and writing) of the Autistic adolescents. This may be because the guardians’ lack of knowledge prior to the intervention may have led them to overestimate their assessment of the Autistic adolescents and evaluate them as if they were neurotypical children. One factor that may have contributed to the lack of association between guardians' treatment stigma and changes in the intervention was that the BACEv3 is a measure of self-treatment stigma, and therefore the child's intervention, the ACAT, may not have been correctly reflected on the ACAT. A recently reported study has shown that treatment stigma prevents people from seeking help [ 11 ]; however, this study suggests that treatment stigma does not have an impact. Notably, people already undergoing treatment participated in the original study [ 34 ]. Therefore, it can be said that the study showed that high treatment stigma does not affect the intervention only for those who are undergoing treatment. An environment where treatment and support are appropriately and easily connected is important for children with ASD and their parents, who have high treatment stigma. It would be good if there were a mechanism or system that allows everyone to easily connect to welfare and clinical services for support and treatment. If the findings from the past and future research can be part of the construction of such a system, even those with high treatment stigma who are not receiving support may be able to access support. Limitations The participants in this study were Autistic adolescents and guardians who actively avoided or did not receive treatment or support, as the study dealt with treatment stigma while participating in an intervention trial. Regarding guardianship treatment stigma, we did not measure guardians' treatment stigma. It may be necessary to examine treatment stigma for Autistic adolescents and their guardians who are not receiving assistance in the future. In addition, because we used data from an intervention trial, the number of samples was relatively small, and there is a need to reanalyze the results with a more significant number of samples. Conclusions Even when Autistic adolescents had treatment stigma, there was no change between high treatment stigma and the effect of the intervention, suggesting that treatment stigma had no effect. Even when guardians' treatment stigma was high, CBT interventions for Autistic adolescents increased knowledge, suggesting that it also did not reduce the effectiveness of the intervention for Autistic adolescents. The changes in the Barriers to Access to Care Evaluation scale version 3 (BACEv3) before the intervention and other tests after the intervention were analyzed using Spearman's rank correlation coefficient.\u003c/p\u003e\u003cp\u003eResults\u003c/p\u003e\u003cp\u003eThe Spearman's rank correlation coefficient analysis showed that the BACEv3 of Autistic adolescents did not correlate with the change in any of the scales. Guardians' BACEv3 was significantly correlated with Autistic adolescents’ Autistic traits knowledge of Autism Spectrum Disorder characteristics and communication of adaptive behavior.\u003c/p\u003e\u003cp\u003eConclusions\u003c/p\u003e\u003cp\u003eThe results suggest that the presence of treatment stigma in Autistic adolescents does not affect CBT interventions.
