Abstract
This study was attempted to analyse 415 consecutive abdominal hysterectomies, especially concerned with its indication and complication, from January 1, 1978 to December 31, 1982 at the department of Obstetrics and Gynecology of the Inje medical college, Seoul Paik hospital. The results were summarized as follows: 1. Age distribution was between 25 to 70 years with 42.2 year of mean age, and the group of 40 to 49 year revealed highest incidence. 2. There couldn`t be found any relationship between the indication of surgery and gravity or parity. 3. Irregular vaginal bleeding was the most common symptoms, and followed by menorrhagia and dysmenorrhea. 4. Over the 30% of all cases was anemic in preoperative check of hemoglobin levels, and the distribution was between 2.6 and 15.6gm%. 5. The major indications for operation were uterine myoma, ovarian tumor and adenomyosis, and the cervical carcinoma below the stage Ib was also found in 7.7% of all cases. 6. Majority of cases were received abdominal hysterectomies, including 3 cases of radical hysterectomies with pelvic lymphadenectomy and 2 cases of cesarean hysterectomies. One cases of ureteral anastomosis was performed during operation because of surgical injury. 7. Postoperative pathologic examination revealed uterine myoma in 53%, ovarian tumor in 26.7% and adenomyosis in 22.7% of all cases. Malignancy in the uterine cervix, uterine endometrium and ovary was also found in 8.7% of all cases. 8. Overall complication rate of abdominal hysterectomy was 26%, which included febrile morbidity of 13.5%, hemorrhage requiring transfusion of 8.8% and mortality rate of 0.5%.
