EP31.22: Endometriosis of the uterine Caesarean section scar: a case report

Abstract
The purpose of study is to describe the clinical history and ultrasound findings in woman with an endometriosis of the uterine Caesarean scar. Cases of scar endometriosis of the abdominal wall following various obstetrical and gynecological procedures have been reported in literature. However, endometriosis of a uterine scar is extremely rare and it is difficult to diagnose. No statistics are presents regarding its incidence and prevalence. The diagnosis is frequently made only after excision and histopathology of the lesion. We presented a case of a 34 years old patient with previous one lower segment Caesarean section, who was presented with a history of intermenstrual bleeding and pelvic pain during 1 year. Transvaginal ultrasound revealed an unilocular cystic structure with ground glass fluid content, arising from the lower uterine segment, measuring 37 × 28 × 45 mm. and communicating with the lower uterine cavity. The patient has underwent to a diagnostic laparoscopy, which revealed uterine scar endometriosis, without any endometriotic deposits elsewhere in pelvis. The laparoscopic excision of the Caesarean scar and myometrial repairing with the single layer of interrupted 2-0 Vycril sutures has been performed. The postoperative period was uneventful. Histopathology confirmed the diagnosis of endometriosis of the uterine scar. In data ultrasound is recommended for diagnosing endometriosis of the ovary, bladder, and rectum. According to our experience the ultrasound also extremely helpful for detection of the uterine Caesarean section scar endometriosis if it is performed in the late secretory phase of menstrual cycle.
