ENDOMETRIOSE: AVANÇOS NO DIAGNÓSTICO, MANEJO DA DOR E IMPACTOS SOBRE A FERTILIDADE E QUALIDADE DE VIDA

A endometriose é uma doença inflamatória crônica, estrogênio-dependente, caracterizada pela presença de tecido semelhante ao endométrio fora da cavidade uterina e associada a dor pélvica, dismenorreia, dispareunia, infertilidade e prejuízo relevante da qualidade de vida. Esta revisão narrativa discute avanços no diagnóstico, manejo multimodal da dor, repercussões reprodutivas e estratégias de cuidado centrado na paciente, incluindo o papel complementar da nutrição. A evolução da ultrassonografia especializada e da ressonância magnética ampliou a possibilidade de diagnóstico clínico-radiológico, reduzindo a dependência da laparoscopia diagnóstica isolada. O tratamento deve ser individualizado conforme sintomas, desejo reprodutivo, extensão da doença, tratamentos prévios e preferências. Terapias hormonais, analgésicas, cirurgia e reprodução assistida permanecem pilares do manejo. Intervenções nutricionais são de interesse crescente, sobretudo por possíveis efeitos sobre inflamação, sintomas gastrointestinais e dor; contudo, a evidência ainda é heterogênea e não sustenta dietas restritivas universais nem substituição do tratamento médico. Revisões recentes de ensaios randomizados sugerem possível redução de dismenorreia com alguns compostos nutricionais, mas com risco de viés e heterogeneidade importantes. Conclui-se que o cuidado ideal é multidisciplinar, integrando ginecologia, reprodução humana, manejo da dor, saúde mental e, quando indicado, acompanhamento nutricional individualizado.
