Clinical significance of laparoscopy in management of ovarian cysts,smaller than 5 cm in diameter

Abstract

【Objective】 To evaluate the clinical significance of laparoscopy in management of ovarian cysts, smaller than 5 cm in diameter. 【Methods】119 patients with unilateral ovarian cysts underwent laparoscopy. 32 of the 119 patients had ovarian cysts smaller than 5cm in diameter and were classified into Groupe A, who with symp- tom or stable ovarian cysts over 4 months period or high lever of CA125. The other 87 patients with cysts larger than 5 cm in diameter were classified into Group B. The clinical dates were reviewed retrospectively. The clinical patho- logical characteristics were compared between the two groups. 【Results】87.34% cysts in Group A needed opera- tion. More teratoma and endometriosis were found in grope A. The rate of intra-operative cysts rupture, operation time, the rate of blood loss ≤20 mL and the rate of electric coagulation in group A were significantly lower than those in group B (P 0.05). The study showed no statistical difference in the rates of true tumor, intra-operative ad- hensions, post-operative fever and complications between the 2 groups (P 0.05). 【Conclusion】Laparoscopy has significant shorter operative time, less blood loss, less chance of cyst rupture and protects the ovarian function. It′s a safe and necessary treatment for patients with symptomatic ovarian cysts 5 cm or stable ovarian cysts over 4 months period or high lever of CA125, especially beneficial for those who have strong desires to preserve fertility.
