Abstract
To evaluate the intraobserver reproducibility of sonographic signs of adenomyosis using standard transvaginal ultrasound and validated 2D sonographic signs easily available in any setting without the need for 3D technology. A non-selective population attending a medical screening facility underwent 2D transvaginal scanning using a Voluson P8 system with a 2D 7.5-MHz probe. The following sonographic signs were documented: asymmetrical myometrial thickening, linear striations, myometrial cysts, hyperechoic islands, irregular endometrial-myometrial junction (EMJ), parallel shadowing, and localised adenomyomas. Cineloops and still images were stored and analysed at a later date by the same operator with blinding to the original analysis. Results from both evaluations were then compared using SPSS (p < 0.05). 111 women were included with median age 47 (35–77), BMI 23.8 (16.9-35.3), parity 3 (0–6). The intraobserver reproducibility is seen in table 1. Intraobserver reproducibility of sonographic signs of adenomyosis using standard transvaginal ultrasound is good to very good. The use of multiple signs for diagnosis may increase reproducibility significantly. 2D transvaginal ultrasound may be sufficient for diagnosis of adenomyosis in the absence of 3D capabilities or MRI.
