A real-world simulation of the fertility trend among women with endometriosis in Italy

Abstract

OBJECTIVES: to model a real-world probability curve of fertility rates in non-endometriotic and endometriotic women using demographic data and information from Italy. DESIGN: probabilistic simulation built from available data of Italian population. SETTING AND PARTICIPANTS: data from the Italian population of the years 2020-2023 were collected on: 1. the actual desire for pregnancy; 2. the infertility rate and outcomes of assisted reproductive technologies (ART); 3. the age at which endometriosis is diagnosed and the impact of endometriosis treatments on fertility; 4. the infertility rate in endometriotic patients and outcomes of ART; 5. the natural decline in fertility with age in women. Data for points 1 to 3 were obtained from Italian institutional websites, while points 4 and 5 were estimated from scientific literature. Normalized estimates of cumulative pregnancy rates and live birth rates in Italian non-endometriotic and endometriotic women were calculated based on Italian family planning behaviour and endometriosis medical care issues. These estimates were combined and plotted by female aging to observe trends. MAIN OUTCOME MEASURES: age-specific fertility trends in endometriotic and non-endometriotic Italian women. RESULTS: treated endometriotic patients show a reduction in fertility potential compared to non-endometriotic and untreated endometriotic patients. CONCLUSIONS: the decline of fertility in Italian endometriotic women is mainly iatrogenic. Women with endometriosis should consider conceiving at a young age.

Abstract

Obiettivi: modellare una curva di probabilità real-world di frequenza di fertilità in donne non endometriosiche ed endometriosiche utilizzando dati e informazioni demografiche italiani. Disegno: simulazione probabilistica costruita a partire dai dati della popolazione italiana. Setting e partecipaneti: sono stati raccolti dati dalla popolazione italiana per gli anni anni 2020-2023 su: 1. il desiderio effettivo di gravidanza; 2. il tasso di infertilità e gli esiti delle tecniche di riproduzione assistita (ART); 3. l’età alla diagnosi di endometriosi e l’impatto dei trattamenti per l’endometriosi sulla fertilità; 4. il tasso di infertilità nelle pazienti endometriosiche e gli esiti delle ART; 5. il naturale declino della fertilità con l’età nelle donne. I dati per i punti da 1 a 3 sono stati ottenuti da siti web istituzionali italiani, mentre i punti 4 e 5 sono stati stimati dalla letteratura scientifica. Le stime normalizzate delle frequenze di gravidanza cumulative e delle frequenze di nati vivi da donne italiane non endometriosiche ed endometriosiche sono state calcolate sulla base del comportamento di pianificazione familiare italiano e dei problemi di assistenza medica per l’endometriosi. Queste stime sono state combinate e rappresentate graficamente in base all’invecchiamento femminile per osservarne le tendenze. Principali misure di outcome: costruzione di curve di tendenza di fertilità nelle donne italiane con e senza endometriosi. Risultati: le pazienti endometriosiche trattate mostrano una riduzione del potenziale di fertilità rispetto alle donne endometriosiche non trattate e a quelle senza endometriosi. Conclusioni: il declino della fertilità nelle pazienti endometriosiche italiane è principalmente iatrogeno. Le pazienti endometriosiche dovrebbero mirare al concepimento il prima possibile.
