The established risk factors for postoperative cardiac arrest includehypovolemia, acidosis, and electrolyte imbalances. Case presentation: We present the case of a 67-year-old female with a history of type 1 diabetes mellitus and hypertension who underwent a Whipple procedure under general anesthesia. Preoperative serum potassium levels, which were measured three days prior, were within normal limits (5.28 mmol/L). However, profound hypokalemia (2.71 mmol/L) was detected before the skin incision, and the anesthesiologisttreated the patient with 2 g (26.8 mEq) of intravenous potassium chloride under cardiac output monitoring. The posttreatment serum potassium level at 16:41 was 3.28 mmol/L, indicating persistent hypokalemia. Following the procedure, the patient's vital signs stabilized, meeting the criteria for transfer to the general ward. However, within 30 minutes of ward arrival, the patient experienced a sudden episode of hypertension and tachycardia, rapidly progressing to pulseless electrical activity (PEA) cardiac arrest. Immediate resuscitation efforts, including continuous cardiopulmonary resuscitation (CPR) and epinephrine administration, were initiated. After spontaneous circulation returned, the patient was transferred to the intensive care unit (ICU) and subsequently diagnosed with hypoxic-ischemicencephalopathy (HIE), multiple organ dysfunction syndrome (MODS), or sepsis. Ultimately, the patient succumbed to these complications on day 14 of ICU admission. Conclusions: This critical event underscores the vital importance of meticulous perioperative potassium management in diabetic patients undergoing major abdominal surgery. We advocate for mandatory reevaluation of electrolyte levels within 24 hours preoperatively, particularly following bowel preparation and dietary restriction, in conjunction with prophylactic potassium supplementation and careful insulin administration, as crucial measures to prevent such devastating outcomes. Background The Whipple procedure presents considerable technical challenges due to the intricate retroperitoneal anatomy surrounding the pancreas and duodenum, including proximity to major vasculature and biliary‒pancreatic ducts [ 1 ] . Elevated rates of mortality and postoperative myocardial infarction have been reported following Whipple surgery in elderly patients [ 2 ] . While advancements in preoperative assessment and intraoperative techniques have improved safety, perioperative cardiac arrest remains a rare but devastating complication. These events are frequently multifactorial, resulting from the complex interplay of surgical stress, hemodynamic instability, and acute metabolic derangements. Among modifiable risk factors, profound hypokalemia represents a critical and potentially preventable trigger for malignant arrhythmias and subsequent cardiac arrest [ 3 ] . This risk is particularly relevant in insulin-treated diabetic patients, where potassium homeostasis is often precarious. Insulin administration promotes intracellular potassium shifts, whereas common preoperative practices such as mechanical bowel preparation can induce significant potassium depletion. Despite routine electrolyte monitoring, rapid and undetected hypokalemia can develop subclinically within a short period. While the literature emphasizes the arrhythmogenic potential of hypokalemia, comprehensive documentation of its precipitous perioperative evolution in high-risk diabetic patients, particularly those who are undergoing major abdominal surgery, remains limited. Few reports detail the synergistic mechanisms (insulin effect and bowel preparation) and kinetic progression from normokalemia to life-threatening levels before cardiac arrest. This report presents a compelling case of unaddressed preoperative hypokalemia in an insulin-dependent diabetic patient undergoing elective Whipple surgery. Within 72 hours, a cascade of bowel preparation-induced losses and insulin-mediated transcellular shifts precipitated a decline from normokalemia (5.28 mmol/L) to critical hypokalemia (2.71 mmol/L), undetected by standard monitoring intervals. This rapid electrolyte imbalance culminated in postoperative cardiac arrest, resulting in HIE and MODS. Underscoring the necessity for intensified perioperative electrolyte monitoring in high-risk cohorts. 2. Case Presentation A 67-year-old female (ASA II) with well-controlled hypertension and type 1 diabetes mellitus was admitted for management of an ampullary tumor and scheduled for laparoscopic pancreaticoduodenectomy. General anesthesia was induced at 08:05 on May 29 with endotracheal intubation utilizing sufentanil, cipepofol‌, and rocuronium. Mechanical ventilation was initiated with 100% oxygen in pressure‒volume control mode, targeting a tidal volume of 7 ml/kg. Standard noninvasive monitoring and invasive arterial blood pressure monitoring methods were established. A serum potassium level measured at 08:50 revealed critical hypokalemia (2.71 mmol/L). Consequently, 2 g (26.8 mEq) of potassium chloride was administered intravenously via a central venous line over 6 hours. Remifentanil, cipepofol, sevoflurane and rocuronium were used for intraoperative anesthesia maintenance. The patient underwent a laparoscopic Whipple procedure with resection of the pancreatic head, duodenum, distal stomach, common bile duct, and gallbladder. Intraoperative blood loss was minimal (100 mL). During the 6-hour 40-minute procedure (10:20), the serum potassium level measured at 16:41 was 3.28 mmol/L. The procedure ended at 17:00. After tracheal extubation at 17:25 in the postanesthesia care unit (PACU), the patient remained stable and was transferred to the ward at 17:50. At 18:19, the patient acutely developed hypertension, tachycardia, and new-onset arrhythmias. The rapid decline in potassium levels directly precipitated refractory ventricular arrhythmias and cardiac arrest. Cardiac arrest occurred at 18:35, prompting immediate CPR with intravenous epinephrine. Intensive care physicians performed endotracheal intubation and manual ventilation. Defibrillator monitoring revealed PEA, a nonshockable rhythm. Following continued CPR, spontaneous circulation returned at 18:58, with sinus tachycardia (150–170 bpm), hypertension (167/121 mmHg), and a peripheral oxygen saturation of 98%. Bilateral pupillary dilation (5.5 mm) with an absent light reflex was noted. The patient was transferred to the ICU for mechanical ventilation and hemodynamic support. Comprehensive ICU care included potassium repletion, mechanical ventilation, neuroprotection (hypothermia, osmotherapy), hemodynamic support (vasodilators, vasopressors, inotropes), infection control (antibiotics), organ-specific therapies (corticosteroids, cardioprotectives, neurotrophic factors, renal preservation), nutrition, and homeostasis maintenance. Subsequent delays in identifying and correcting concurrent hypocalcemia likely compounded myocardial injury. The changes in the serum potassium and ionized calcium levels on the day of cardiac arrest and after surgery are shown in Fig. 1A-B. The patient underwent neurosurgical intervention, with a subsequent diagnosis of HIE complicated by concurrent cardiac, hepatic, and renal dysfunction. Despite aggressive multidisciplinary management (involving cardiology, neurosurgery, and the intensive care unit (ICU)), the patient failed to regain full consciousness and progressed to deep coma. Sulcal effacement in the frontal regions, consistent with cerebral edema, was observed via cranial computed tomography (CT) (Fig. 1C). Figure 1. A notable aspect of this case was the presence of refractory hypokalemia, requiring substantially higher doses of potassium supplementation to maintain unstable potassium levels. A total of 33.5 g (448.9 mEq) of potassium chloride (including 2 g used in the operating room) and 34.2 g (200 mEq) of potassium aspartate injection were administered in the ICU. The changes in refractory hypokalemia from the beginning of anesthesia to death are shown in Table 1. Persistent elevation of myoglobin (MYO) and high-sensitivity cardiac troponin I (hs-cTnI) following resuscitation indicated ongoing myocardial injury and necrosis. Moreover, the sustained increase in N-terminal pro-B-type natriuretic peptide (NT-proBNP) suggested significant cardiac dysfunction and ventricular wall stress. Although initial therapeutic interventions transiently reduced biomarker levels, the subsequent rebound and refractoriness to intensified management indicated progressive myocardial damage and worsening cardiac function. The patient also presented with intermittent seizures and nonsustained ventricular tachycardia [ 4 ] , presented with fixed and dilated pupils (5 mm bilaterally) without light or corneal reflexes, and remained ventilator dependent. By day 14 postcardiac arrest, vital signs remained unstable (heart rate persistently > 130 bpm, systolic blood pressure < 90 mmHg, diastolic < 60 mmHg). Following discussion, the family opted for the withdrawal of life-sustaining therapies, and the patient expired. The sequence of events and management of cardiac arrest are summarized in Fig. 2 . Discussion and conclusions This case illustrates fatal cardiac arrest precipitated by hypokalemia during the perioperative period. Despite aggressive resuscitation efforts, the patient succumbed to MODS, HIE, and sepsis, a cascade initiated by multifactorial pathophysiological processes, including perioperative electrolyte derangements, ischemic-reperfusion injury, and metabolic dysregulation. The primary trigger for postoperative cardiac arrest was unaddressed perioperative hypokalemia. More than two enemas throughout the body are considered risk factors for hypokalemia [ 5 ] . This rapid decline was synergistically driven by insulin-mediated transcellular potassium shifts and substantial gastrointestinal potassium losses from preoperative mechanical bowel preparation. Furthermore, dietary restriction combined with sustained insulin therapy markedly potentiated this effect. Postarrest hypocalcemia exacerbates myocardial dysfunction and cerebral injury. Calcium dysregulation in cerebral ischemia‒reperfusion activates calpain-caspase apoptotic pathways, contributing to irreversible neurological injury. Exaggerated inflammatory responses, coupled with catecholamine hyperactivity, are recognized as drivers of hypocalcemia [ 6 ] . While the 7-hour operative duration was within the expected limits for Whipple procedures, prolonged anesthesia and surgical trauma precipitated a systemic inflammatory response, occult hypoperfusion, insulin resistance, and stress hyperglycemia (29.4 mmol/L at the time of arrest). Hyperglycemia potentiates oxidative stress-mediated brain injury, particularly in diabetic patients [ 7 ] . Nonetheless, we acknowledge the limitations of this report. First, regarding hypokalemia, electrolytes were not re-evaluated within 24 hours preceding the operation. Moreover, severe hypokalemia was identified intraoperatively but was not adequately corrected during the perioperative period, and delayed intervention may have contributed to arrhythmia. Second, the patient's preexisting hypertension and diabetes, although managed with medication, may have led to unrecognized microvascular dysfunction, potentially exacerbating ischemic injury following cardiac arrest [ 8 ] . Hypertension requiring medication is independently associated with cardiovascular complications after Whipple procedures and may need to be considered in preoperative risk stratification [ 9 ] . Third, although HIE was diagnosed, advanced neurological monitoring (such as electroencephalography) was not conducted, which may have resulted in underestimation of the severity of brain damage. These limitations highlight the need for prospective studies incorporating rigorous intraoperative monitoring and standardized post arrest care pathways in high-risk surgeries. In conclusion, this case report clearly illustrates the catastrophic consequences of inadequately managed perioperative hypokalemia, particularly in insulin-treated diabetic patients undergoing major abdominal surgery, such as the Whipple procedure. This fatal event underscores critical lessons. First, high-risk patients (insulin-treated diabetic patients undergoing major surgery) require serial perioperative potassium monitoring: postbowel preparation, preoperatively on the morning of surgery, immediately upon arrival at the operating room, and intraoperatively every 60 minutes for procedures exceeding 5 hours. Proactive repletion should target a preoperative potassium > 4.0 mmol/L. If necessary, surgery should be postponed until the internal environment is optimized. Second, therapeutic hypothermia represents one of the most widely implemented neuroprotective strategies following cardiac arrest [ 10 ] . The implementation of targeted temperature management (TTM) should be considered, and hyperbaric oxygen therapy may also be considered within 6 hours postarrest to mitigate HIE progression. The fatal trajectory of persistent HIE, irreversible MODS, and sepsis underscores the devastating potential of perioperative electrolyte instability in elderly patients with diminished physiological reserves. Future research must prioritize the development of validated risk stratification tools and the implementation of rigorous, protocol-driven perioperative electrolyte management strategies, especially for high-risk populations undergoing complex surgical procedures. Preoperative serum potassium levels, which were measured three days prior, were within normal limits (5.28 mmol/L). However, profound hypokalemia (2.71 mmol/L) was detected before the skin incision, and the anesthesiologisttreated the patient with 2 g (26.8 mEq) of intravenous potassium chloride under cardiac output monitoring. The posttreatment serum potassium level at 16:41 was 3.28 mmol/L, indicating persistent hypokalemia. Following the procedure, the patient's vital signs stabilized, meeting the criteria for transfer to the general ward. However, within 30 minutes of ward arrival, the patient experienced a sudden episode of hypertension and tachycardia, rapidly progressing to pulseless electrical activity (PEA) cardiac arrest. Immediate resuscitation efforts, including continuous cardiopulmonary resuscitation (CPR) and epinephrine administration, were initiated. After spontaneous circulation returned, the patient was transferred to the intensive care unit (ICU) and subsequently diagnosed with hypoxic-ischemicencephalopathy (HIE), multiple organ dysfunction syndrome (MODS), or sepsis.
