The aim of this study was to determine the empowerment status of pregnant women in an Iranian setting. Methods: This analytic cross-sectional study was carried out on 200 eligible pregnant women attending Ardabil health centers in 2023, selected through random cluster sampling. Data was collected using a demographic questionnaire and a Persian version of the Kameda questionnaire on empowerment of pregnant women, which was completed through interviews. The overall empowerment score ranged from 27 to 108, and is classified as poor to excellent. Descriptive statistics, logistic regression, Pearson's correlation and t-tests were utilized for data analysis using SPSS version 17. Results: The mean age of the pregnant women was 26.8± 4.67 years, with the majority having a college education (60%) and being housewives (76%). The overall empowerment score of the pregnant women was 54.58, which indicates a moderate level of empowerment. Among the subscales of empowerment, the highest mean score (64) was related to the joy of adding a person to the family, and the lowest score (49) was related to the subscale of self-esteem. There was a significant inverse correlation between the age of pregnant women and their future image (P=0.019). Higher education and employment were linked to higher self-esteem. There were no statistically significant relationships between empowerment status in pregnant women and housing type, income, husband's education, gravida, parity, number of abortions or number of living children. Conclusion: The results of this study could be used in conducting effective interventions for maternal health and promoting the empowerment of pregnant women. Empowerment Pregnant Women Iran Background Pregnancy is one of the most important and sensitive periods in a woman's life( 1 , 2 ). Women's health during pregnancy and childbirth is considered as the cornerstone of population health at the international level. Improving maternal health and reducing infant mortality have been two crucial components of the third Millennium Development Goals ( 3 – 5 ) and remain among the 17 key goals of Sustainable Development, 2030( 6 ). Empowerment is one of the objectives of the World Health Organization aimed at improving health( 7 ). However, defining and understanding the concept of empowerment remains complicated( 8 ). Gibson (1991)stated that women’s empowerment is the process of recognizing and enhancing women’s ability to meet health needs, solve health problems, and take action to feel in control of their health and lives( 9 ). Kameda defined empowerment during pregnancy as a sense of satisfaction, happiness, self-actualization and increasing independence through interacting with the environment and other people. This ultimately leads to a spontaneous increase in psychological energy for pregnancy, successful childbirth, and better adaptation to physical and psychological changes in the perinatal period ( 10 ). In general, empowerment in pregnancy was redefined as the capability to make strategic life decisions( 11 ). Pregnant women must obtain the necessary knowledge and skills to navigate this period and play an active and effective role during their pregnancy and childbirth. They should be able to have a natural childbirth without fear or worry, and with increased self-confidence( 12 ). On the other hand, the transition to parenthood is a significant life change that necessitates adjustments in physical, psychological and social aspects. This transition can be accompanied by considerable distress( 13 ). In addition to the rapid physical changes, future mothers also experience the process of developing maternal identity ( 14 ). Therefore, the empowering pregnant women on issues during pregnancy, safe delivery and postpartum care is one of the key components of reproductive health. It can play an important role in maternal and child health, while also positively influencing the likelihood of reporting instances of pregnancy discrimination ( 12 , 15 – 17 ). WHO indicates that empowering pregnant mothers leads to increased use of contraceptive methods after childbirth, decreased Illegal abortions and infant mortality, improved of perinatal care, complete vaccination and overall maternal and child health ( 18 , 19 ). Considering the importance of empowerment during pregnancy and the lack of adequate research in Iran, conducting the present study to determine the empowerment status of pregnant women in an Iranian setting, was deemed necessary to implement appropriate interventions to enhance maternal and child health. Methods This analytic cross-sectional study was conducted on 200 pregnant women who were referred to Ardabil health centers to receive care, in 2023. Random cluster sampling was utilized in this study. The inclusion criteria were; being literate, having a health record in the integrated health system, not having high-risk pregnancy conditions, no history of chronic diseases, not taking any drugs, alcohol, narcotics, immune-suppressive drugs, or chemotherapy. Exclusion criteria included incomplete questionnaires and unwillingness to continue cooperation. Ardabil city has 34 health care centers in the five regions of the city (North, South, Center, East and West), two health centers were randomly selected from each of the regions, totaling 10 centers. Women were recruited into the study using convenience sampling. The number of pregnant women in each center was listed, and the ratio of the number of pregnant women in each center to the total number of pregnant women was determined. The sample from each center was then selected randomly. The data collection tool was a checklist containing demographic information and a standard questionnaire on empowerment in pregnant women (Kameda)( 10 ), which was completed through interviews by researcher. The questionnaire was translated and localized by Hajipour et al. (2016). They translated the questionnaire from English to Persian twice. Then, the final version was reviewed by a group of experts to determine content validity and very minor changes were made. Next, the questionnaire was distributed to 15 pregnant women to check their reading and comprehension ability and minor changes were made. In this way, the validity and reliability of the instrument were confirmed. By the way, the Cronbach's alpha coefficient for all questions in this study was 0.89 ( 20 ). This questionnaire consists of 27 items and five subscales, namely self-efficacy (six items), imagining the future (six items), self-esteem (seven items), understanding support from others (four items), and the joy of adding a person to the family (four items). Content validity and face validity were used to determine the scientific validity of the questionnaire. Participants answered the items using a four-point Likert scale, scoring from one to four points: strongly disagree ( 1 ), disagree ( 2 ), agree ( 3 ), strongly agree ( 4 ). The scores for each subscale ranged from 6 to 24 points for self-efficacy and imagining the future, 7 to 28 points for self-esteem, 4 to 16 points for understanding support from others and the joy of adding a person to the family. The empowerment score for pregnant women was calculated by summing the scores from each subscale, with a total score ranging from 27 to 108. Finally, this raw score is classified as poor (0–50), average or moderate (50.1–66), good (66.1–84) and excellent (84.1–100) on a scale of 0-100. Descriptive statistics (frequency, mean, and standard deviation) were used for demographic variables. The correlation between quantitative variables (age, Gravida, Parity, number of abortions, and number of live children) with the empowerment status of pregnant women were analyzed using the Pearson's correlation test, and the relationship between qualitative variables (Job, education, income, type of housing and spouse's education) were analyzed using the chi-square test in SPSS version 17. Ethical considerations The study was approved by the research ethics committee at IR.ARUMS.REC.1402.037. The study protocol adheres to the ethical guidelines outlined in the 1975 Declaration of Helsinki, including considerations such as voluntary participation and obtaining written informed consent. Women were assured of the privacy of their data and were informed that all data would be analyzed anonymously. Results According to the findings of this study, the mean age of pregnant women was 26.8 ± 4.67 years. 75% of women were in the second trimester of pregnancy. Most of them had a college education (60%), 76% were housewives and 72% declared their family income to be at an average level (Table 1 ). Among the subscales of empowerment, the highest mean score (64) was related to the joy of adding a person to the family, and the lowest score (49) was related to the subscale of self-esteem. The mean empowerment scores of pregnant women in support and assurance of others and future image subscales were 55, and the self-efficacy was 53.9 (Table 2 ). Therefore, as pregnant women get older, the future image is not good. Discussion The focus of this study was to determine the empowerment status of pregnant women. The results of the overall scores indicated a moderate level of empowerment in this study, which was consistent with the study of Hajipour et al.( 20 ), but lower than the study of Borghei et al( 21 ). Among the subscales of empowerment, the highest score was related to the subscale of the joy of adding a person to the family, and the lowest score was related to the subscale of self-esteem, in line with the results of Borghei et al. ( 21 ). The low score of support from others in our study indicates insufficient support from pregnant women's relatives, spouses, and even health-care providers, which should receive more attention in this field. Supporting pregnant women not only has a positive effect on the mental health and stress reduction of women, but can also improve mother-infant interactions and subsequently, a child's growth, leading to an increase in self-esteem and empowerment ( 22 – 24 ). Another finding of this study was the high score in relation to the joy of adding a new person to the family. This result could be interesting despite the decrease in mothers' desire to have more children and the increase in families' preference for having only one child in recent years. This result emphasizes the fact that one of the components of women's empowerment in Iranian society has always been the number of family members and children( 25 ). In Ardabil province, 24% of families have opted to have only one child( 26 ). This finding, in turn, may be the result of new efforts and incentives for childbearing, including economic and social support, etc. in the country. Regarding factors related to the empowerment of pregnant women, the results of our study showed that; there was no significant relationship between the overall level of empowerment and the age of pregnant women. However, there was a significant inverse correlation between the age of mothers and the image of the future. As pregnant women age, their image of the future tends to be less positive. As a woman's age increases, her rights and duties also change( 27 ). In this regard, Tavananezhad et al., reported that the empowerment of pregnant women weakens with ageing( 28 ), but this finding is inconsistent with other studies that suggest aging is associated with an increase in women's empowerment( 21 , 27 , 29 , 30 ) A common explanation offered by most of these researchers is that older women spend more time with their husbands and children, leading to better communication and understanding of family issues ultimately resulting in increased empowerment. Therefore, these differences may be due to cultural variations in other societies. The results of this study show a statistically significant relationship between a mother's education and empowerment, indicating that mothers with a university education have higher self-esteem. Education is directly correlated with learning and skills( 28 ). The importance of women's education in today's society is increasing, and the positive role of education in empowering women has been supported by numerous studies ( 31 – 34 ). Our study’s findings confirm these contents. Another finding of this study is that working women have higher self-esteem compared to housewives. Occupation plays a crucial role in one's beliefs, abilities and self-esteem( 35 ). Entrepreneurship and financial independence empower women( 36 , 37 ). Akter et al., in their study in Bangladesh, concluded that having a job and exposure to communication were positively correlated with women's empowerment ( 38 ). In the study by Aboagye et al(2024), the highest odds of adequate prenatal care and empowerment belonged to women with higher levels of education, employment, and wealth( 34 ). Hajipour et al., reported the mean empowerment score of working women was higher than that of housewives, although not statistically significant( 20 ). The employment of pregnant women may also be associated with risks, Burgers(2024) reported that 43% of pregnant employees in the Dutch labor market experienced pregnancy discrimination between 2007 and 2019( 15 ). However, in this study, being employed had a positive effect on the self-esteem of pregnant women, possibly due to socio-cultural issues or financial independence. In this study, no statistically significant relationship was found between the empowerment of pregnant women with housing type, income, spouse's education, Gravida, Parity, number of abortions, and number of living children. However, many studies have reported a significant relationship between them. Sabit et al. (2024), reported that most women who had spontaneous abortions had low levels of self-esteem( 39 ). Zare et al, revealed that infertile women had lower self-esteem than fertile women and there is a significant relationship between self-esteem and women’s income, and housing ( 40 ). Silva also reported that pregnant women’s empowerment is influenced in it’s subscales, by the previous experience of pregnancy and birth and by social support( 41 ). Samari investigated the impact of childbearing on women’s empowerment trajectories in Egypt. She discovered that, for a young woman, giving birth is associated with increased empowerment; the first birth and each subsequent birth predicted improvements in all measures of empowerment except the financial autonomy ( 42 ). Ndola Prata et al, has been found that group antenatal care to be effective in improving the empowerment of pregnant women. Pregnant women who feel empowered through better coping skills before giving birth are less likely to suffer from postpartum depression ( 43 ). These differences can be due to differences in the environment or cultural differences. Empowerment should be understood within the socio-cultural-economic-political perspective of women( 44 ). Women's external and internal characteristics must be considered in the broader socio-cultural-economic-political perspective of each woman, along with a woman's belief in herself and her meaningful connection with her caregivers ( 45 ). This study is one of the few studies have been conducted on the empowerment status of pregnant women in an Iranian setting. One limitation of this study was the low participation desire of some women and their reluctance to answer questions, which was addressed through cooperation of health care providers who were trusted by the pregnant women. Another limitation was that it was self-reported, so not all answers may be correct. The other limitation was that only urban women were studied, who might have different socio-cultural conditions compared to rural women. Therefore, it is recommended to also study the empowerment status of rural women. Conclusion The results of this study examined the empowerment status of pregnant women in an Iranian setting and confirmed that; a high level of education and being employed; can increase the self-esteem of pregnant women. However, aging does not always result in women feeling more empowered and can be associated with a negative outlook on the future. Therefore, it is recommended that policymakers pay more attention to pregnant women’s empowerment and provide the necessary support facilities for them. The importance of women's education in today's society is increasing, and the positive role of education in empowering women has been supported by numerous studies (\u003cspan additionalcitationids=\"CR32 CR33\" citationid=\"CR31\" class=\"CitationRef\"\u003e31\u003c/span\u003e\u0026ndash;\u003cspan citationid=\"CR34\" class=\"CitationRef\"\u003e34\u003c/span\u003e). Our study\u0026rsquo;s findings confirm these contents.\u003c/p\u003e\u003cp\u003eAnother finding of this study is that working women have higher self-esteem compared to housewives. Occupation plays a crucial role in one's beliefs, abilities and self-esteem(\u003cspan citationid=\"CR35\" class=\"CitationRef\"\u003e35\u003c/span\u003e). Akter et al., in their study in Bangladesh, concluded that having a job and exposure to communication were positively correlated with women's empowerment (\u003cspan citationid=\"CR38\" class=\"CitationRef\"\u003e38\u003c/span\u003e). In the study by Aboagye et al(2024), the highest odds of adequate prenatal care and empowerment belonged to women with higher levels of education, employment, and wealth(\u003cspan citationid=\"CR34\" class=\"CitationRef\"\u003e34\u003c/span\u003e). Data was collected using a demographic questionnaire and a Persian version of the Kameda questionnaire on empowerment of pregnant women, which was completed through interviews. The overall empowerment score ranged from 27 to 108, and is classified as poor to excellent. Descriptive statistics, logistic regression, Pearson's correlation and t-tests were utilized for data analysis using SPSS version 17.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults:\u003c/strong\u003e The mean age of the pregnant women was 26.8± 4.67 years, with the majority having a college education (60%) and being housewives (76%). The overall empowerment score of the pregnant women was 54.58, which indicates a moderate level of empowerment. Among the subscales of empowerment, the highest mean score (64) was related to the joy of adding a person to the family, and the lowest score (49) was related to the subscale of self-esteem. There was a significant inverse correlation between the age of pregnant women and their future image (P=0.019). Higher education and employment were linked to higher self-esteem.
