The Parkland Cholecystitis Scale (PCS) is a tool for staging the severity of gallbladder disease using intraoperative imaging. The main objective of this study is to describe the clinical-epidemiological characteristics of patients undergoing laparoscopic cholecystectomy (LC) in the period from March 2018 to December 2021, with the secondary objective being to investigate whether there is a relationship between the presence of presurgical leukocytosis and grades 3, 4 and 5 in the PCS. Methods: This is a cross-sectional and single center study, which exhaustively analyze patients undergoing LC at the Institute of Security and Social Services of State Workers (ISSSTE by its acronym in Spanish) León Regional Hospital, in the period from March 2018 to December 2021. Results: A total of 289 patients were included in the study of which 214 (74%) were female and 75 (26%) were male, with a mean age of 50 ± 5.7 years. It was identified that being male, type 2 diabetes mellitus, as well as the presurgical presence of leukocytosis (≥ 10x 10 9 /L), hyperbilirubinemia (total bilirubin ≥ 1.2 mg/dl), thickened gallbladder wall reported by ultrasound (≥ 4 mm), and bile duct dilation (> 6 mm) showed a statistically significant association with difficult cholecystectomies. Conclusion: It is essential to use different rescue tools or even consider converting to open surgery when faced with a patient with these characteristics to avoid an unfavorable outcome in patients with difficult cholecystectomy. Figures Figure 1 Introduction Biliary stone disease is the most common condition that affects the gallbladder and biliary tree. Since the gallbladder concentrates bile, the concentration of solutes in it differs from the rest of the bile duct. LC is the most frequently performed surgery by the general surgeon. The rate of bile duct injury has always been a concern, based on the higher risk of biliary injury in LC (0.1%-1.5%) when compared to the open approach (0.1%-0.2%), in contrast to the multiple benefits that the first represents with respect to the second, which drives the implementation of different safety methods, such as critical safety vision [1–2]. Today the Tokyo criteria are recommended for the diagnosis, classification and management of acute cholecystitis [3]. It has been shown that increasing degrees of gallbladder inflammation lead to higher conversion rates and iatrogenic injury [4]. Among the classifications designed to predict the level of difficulty for LC, we found the PCS proposed by Madni TD et al., a tool for accurately stage the severity of gallbladder disease using intraoperative imaging, it is an image scale of the gallbladder at the initial moment of laparoscopic surgery that assigns a grade based on the visual characteristics found, resulting in five easily implementable grades [5]. This system is based on the anatomy and inflammatory changes observed during surgery. The risk factors for complications in LC have been well identified, and these are generally related to age, urgent surgery, obesity and concomitant diseases, which generate difficulties in identifying the structures of Calot's triangle [6]. Multiple strategies have been sought with preventive purposes to accurately predict the difficulty of LC. In a retrospective study in Istanbul published in 2015, whose objective was to evaluate C-reactive protein (CRP) levels at different cut-off points to predict the severity of cholecystitis, it was found that CRP can be accepted as a strong predictor to classify cholecystitis severity [7]. Another indirect marker of the degree of severity that is being studied to predict a difficult cholecystectomy is procalcitonin; in a study that was carried out in 2018 and published by the Department of Hepatopancreatobiliary Surgery at Shenzhen People's Hospital in Guangdong, China in which the predictive value of procalcitonin for difficult cholecystectomy was evaluated, it was found that it has a higher positive predictive value than CRP to diagnose difficult cholecystectomy in acute cholecystitis, considering > 1.50 ng/mL as a cut-off point [8–10]. In a study carried out in Indonesia and published in 2019, the correlation between leukocytosis and gallbladder adhesions in cholecystectomized patients was analyzed, ruling out the presence of an association [11]. Siada S et al. through the American Association for the Surgery of Trauma introduced a scale for acute cholecystitis based on anatomical variables, which uses imaging, intraoperative and pathological criteria to quantify the severity of the acute cholecystitis, with grade 1 being acute cholecystitis, grade 2 gangrenous or emphysematous cholecystitis, grades 3–5 describes it as perforation with local contamination, abscess or fistula and the presence of peritonitis [12]. The primary objective of this study was to pre-surgically identify certain clinical-epidemiological characteristics of the patients to predict more complex conditions at the time of LC. The secondary objective was to determine the relationship of a pre-surgical leukocyte count greater than or equal to 10x10 9 /L as an indirect marker related to the severity and the PCS (grades 3, 4 and 5). Materials & Methods This is a cross-sectional and single-center study carried out in ISSSTE Regional Hospital of León, Guanajuato. Records were analyzed from SIMEF and the ISSSTE Laboratory and Imaging Studies database. Inclusion criteria consisted of patients over 18 years of age who were hospitalized with a diagnosis of cholecystitis according to Tokyo Guidelines for Acute Cholecystitis 2018 [3], using ultrasound of the liver and bile ducts as an imaging test, having blood count, liver function tests and pre-surgical ultrasound of the liver and bile ducts, who underwent LC as initial approach during the period from March 2018 to December 2021. The exclusion criteria were made up of patients under 18 years of age, in whom an open approach was initially performed, having an active malignant neoplasm, or incomplete clinical records. A finite population of 568 patients was obtained, where quantitative probability sampling was carried out (Fig. 1 ). The variables considered were the following: PCS classification, difficult cholecystectomy (Parkland grades 3, 4 and 5), age, sex, gender, diabetes mellitus II, obesity, arterial hypertension, leukocytosis, previous hospitalizations, period of hospital stay, surgical time, ASA (American Society of Anesthesiologists) classification [13], hyperbilirubinemia, thickened gallbladder wall and bile duct dilation (Table 1 ). Qualitative variables are reported in rates, frequencies, and percentages. Quantitative variables are reported as mean and standard deviation if the data distribution is normal, as well as in median and interquartile range if it is abnormal distribution. A comparison between the Parkland 1 and 2 groups vs Parkland 3, 4 and 5 was made. The Chi-square test was used for the qualitative variables, depending on the data in the contingency tables, and the Student's t-test was used for the quantitative variables. The significance level was a 95% confidence interval (p-value ≤ 0.05). Results A total of 289 patients were included in the study, with a mean age of 50 ± 5.7 years (36–62), of which 214 (74%) were female and 75 (26%) were male. According to the World Health Organization criteria, only 67 patients (23.2%) suffered from obesity (BMI ≥ 30). The average length of hospitalization of the patients undergoing the study was 1.12 days. Analyzing the hospital stay in PCS grades classified as 3, 4 and 5, an average hospital stay of 1.25 ± 0.25 days is observed, while in grades 1 and 2, it was 1.01 ± 0.12. The distribution according to the PCS was as follows: 79 (27.3%) correspond to grade 1, 79 (27.3%) to grade 2, 88 (30.4%) to grade 3, 34 (11.8%) to grade 4 and finally 9 (3.1%) to grade 5. According to the biochemical variables, the median value of total bilirubin was 0.7 mg/dL, with an interquartile range of 0.1 to 7.3 mg/dL. The average leukocyte count was 9.2 x 10 9 /L. The patients were divided into two groups according to whether the leukocyte count was greater than or equal to 10x 10 9 /L, as the presence of leukocytosis or lack thereof, showing the following distribution: 99 (34.3%) of the patients had presurgical leukocytosis and 190 (65.7%) absence of such characteristic. Similarly, the distribution of the PCS was analyzed, taking those classified in grades 3, 4, and 5 as difficult cholecystectomy. The following distribution was observed: 158 (54.7%) were classified as not difficult cholecystectomy, and therefore 131 (45.3%) were classified as difficult cholecystectomy. Cases classified as difficult cholecystectomy and certain clinical-epidemiological characteristics of the patients were analyzed (Table 2 ). Discussion Recognizing the clinical-epidemiological characteristics which represent essential indicators for predicting severe cholecystitis and hence the type of outcome that the patient may present during medical care allows surgeons to predict this scenario and therefore address it in the best possible way. The development of secondary changes in the setting of acute cholecystitis such as gangrene, xanthogranulomatous inflammation, hemorrhage and emphysema defines severe acute cholecystitis [14]. This is the case of our study in which we sought to identify if determined clinical-epidemiological characteristics represent a greater risk of predisposing to a more complex condition at the time of performing the cholecystectomy and, therefore, the possibility of facing any potential complication ranging from an increase of hospital stay, until the presence of bile duct injuries, which unquestionably translate into a decrease in the patient's quality of life. The present study analyzed the records of 289 patients with an average age of 50 ± 5.7 years. The prevalence in women vs man (74% vs 26%) is essentially the same as reported in the literature, and the majority of these patients present some type of comorbidity, such as obesity, hypertension or diabetes mellitus (73%) [14–16]. When analyzing the distribution according to the PCS, we identified a higher prevalence of Parkland 3 cholecystectomies (88%), followed by grades 1 and 2 with a number of 79, respectively (27.33%). The length of stay was analyzed according to the PCS distribution, and we observed a slightly longer average hospitalization in cases of difficult cholecystectomy (1.25 days ± 0.25 vs 1.01 ± 0.12 days). When analyzing the OR of the clinical-epidemiological characteristics in which we found statistical significance, we identified that the patients who were classified within the difficult cholecystectomy group, having a diagnosis of type 2 diabetes mellitus, showed an OR of 2.15 (CI 1.2–3.7), which accords with the mean OR of 2.92 (CI 1.02–7.16) reported in a systematic review in 2020 by Khan et al. [13]. Likewise, we identified within the biochemical parameters that patients with presurgical hyperbilirubinemia have an OR of 3.45 (CI 1.6 to 7.0). Continuing with the analysis of the biochemical variables, we found that presurgical leukocytosis has an OR of 4.42 (CI 2.6 to 7.4), which translates to the fact that presurgical leukocytosis represents a significant risk factor for the presence of difficult cholecystectomy. Likewise, this concurs with Khan et al. who reported a median OR of 2.8 (CI 1.44–15.7), taken from the analyses of five studies [14]. This result is of vital importance to us, since in addition to being relevant to our secondary objective, it represents a relatively easy element to measure in a hospital environment and provides great information to assess whether it is likely that we will encounter the scenario of a difficult cholecystectomy. Other variables that were shown to be significant predictors of difficult cholecystectomy were the presence of thickened gallbladder wall reported by ultrasound and common bile duct dilation, with an OR of 2.22 (CI 1.1 to 4.19) and 10.2 (CI 1.26–82.7), respectively. It is worth emphasizing that as well Khan et al. reported gallbladder wall edema in ultrasound as a predictor for severe cholecystitis, being a median OR of 2.2 (CI 1.097-4.9), besides gallbladder wall thickening with a median OR of 1.44 (CI 0.24–4.13) is also mentioned [14]. In 2022, Yoshida et al. reported the same results that we found regarding bile duct dilation [4]. It is important in this context to mention the results found by Lee et. al. who compared the presence of leukocytosis (using our same cut-off point), as well as the presence of total bilirubin ≥ 1.2 mg/dl in acute cholecystitis vs chronic cholecystitis, finding out that it was more probable to find these laboratory alterations in acute cholecystitis, with an OR of 8.483 (CI 5.646–12.745), p-value 0.000 and an OR of 4.279 (CI 2.874–6.373), p-value 0.000, respectively [15]. Once we are faced with a difficult cholecystectomy, early recognition of the situation is essential, as well as the evaluation of the scenery and correct decision-making. All of this with the aim of avoiding a bile duct or vascular injury, among other potential complications [2]. In 2015, Strasberg et al. defined the term of subtotal cholecystectomy, which is divided into "fenestrating" and "reconstituting" types, as "bailout" procedures [17]. These strategies, among others, have shown a significant reduction in postcholecystectomy complications [18]. It should be mentioned that this study has some limitations among which are the fact that it was carried out only with patients in whom a cholecystectomy was performed through a laparoscopic and non-open approach initially which makes it susceptible to bias, besides it is unable to investigate the temporal relation between the measured variables due to the nature of our study (cross-sectional), so it would be interesting in the future to see the results of prospective, randomized and controlled trials with a larger sample. Conclusions At the time of facing cholecystitis, the fact of having type 2 diabetes mellitus, hyperbilirubinemia, leukocytosis, and imaging data obtained by ultrasound such as thickened gallbladder wall and dilation of the bile duct represent risk factors to encounter a more complex scenery while performing a laparoscopic cholecystectomy. It is essential to use different rescue tools or even consider converting to open surgery when faced with a difficult cholecystectomy to avoid unfavorable outcomes, and therein lies the importance of being able to identify this condition as early as possible. Since the gallbladder concentrates bile, the concentration of solutes in it differs from the rest of the bile duct. LC is the most frequently performed surgery by the general surgeon. The rate of bile duct injury has always been a concern, based on the higher risk of biliary injury in LC (0.1%-1.5%) when compared to the open approach (0.1%-0.2%), in contrast to the multiple benefits that the first represents with respect to the second, which drives the implementation of different safety methods, such as critical safety vision [1\u0026ndash;2]. Today the Tokyo criteria are recommended for the diagnosis, classification and management of acute cholecystitis [3].\u003c/p\u003e \u003cp\u003eIt has been shown that increasing degrees of gallbladder inflammation lead to higher conversion rates and iatrogenic injury [4]. Among the classifications designed to predict the level of difficulty for LC, we found the PCS proposed by Madni TD et al., a tool for accurately stage the severity of gallbladder disease using intraoperative imaging, it is an image scale of the gallbladder at the initial moment of laparoscopic surgery that assigns a grade based on the visual characteristics found, resulting in five easily implementable grades [5]. This system is based on the anatomy and inflammatory changes observed during surgery.\u003c/p\u003e \u003cp\u003eThe risk factors for complications in LC have been well identified, and these are generally related to age, urgent surgery, obesity and concomitant diseases, which generate difficulties in identifying the structures of Calot's triangle [6]. The development of secondary changes in the setting of acute cholecystitis such as gangrene, xanthogranulomatous inflammation, hemorrhage and emphysema defines severe acute cholecystitis [14].\u003c/p\u003e \u003cp\u003eThis is the case of our study in which we sought to identify if determined clinical-epidemiological characteristics represent a greater risk of predisposing to a more complex condition at the time of performing the cholecystectomy and, therefore, the possibility of facing any potential complication ranging from an increase of hospital stay, until the presence of bile duct injuries, which unquestionably translate into a decrease in the patient's quality of life.\u003c/p\u003e \u003cp\u003eThe present study analyzed the records of 289 patients with an average age of 50\u0026thinsp;\u0026plusmn;\u0026thinsp;5.7 years. Continuing with the analysis of the biochemical variables, we found that presurgical leukocytosis has an OR of 4.42 (CI 2.6 to 7.4), which translates to the fact that presurgical leukocytosis represents a significant risk factor for the presence of difficult cholecystectomy. The Parkland Cholecystitis Scale (PCS) is a tool for staging the severity of gallbladder disease using intraoperative imaging.\u003c/p\u003e\n\u003cp\u003eThe main objective of this study is to describe the clinical-epidemiological characteristics of patients undergoing laparoscopic cholecystectomy (LC) in the period from March 2018 to December 2021, with the secondary objective being to investigate whether there is a relationship between the presence of presurgical leukocytosis and grades 3, 4 and 5 in the PCS.\u003c/p\u003e\n\u003cp\u003eMethods: This is a cross-sectional and single center study, which exhaustively analyze patients undergoing LC at the Institute of Security and Social Services of State Workers (ISSSTE by its acronym in Spanish) León Regional Hospital, in the period from March 2018 to December 2021.\u003c/p\u003e\n\u003cp\u003eResults: A total of 289 patients were included in the study of which 214 (74%) were female and 75 (26%) were male, with a mean age of 50 ± 5.7 years.
