OC09.05: Ultrasound findings and associated symptoms in endometriosis

Abstract

To determine the statistical association between patient-reported symptoms and ultrasound findings for endometriosis. This was a retrospective observational study based on data collected at a University Hospital with a gynecologic ultrasound unit between 2018 and 2022. The study included women diagnosed with endometriosis through laparoscopy, with the purpose of evaluating the symptoms reported by patients during gynecology consultations, such as dyschezia, dyspareunia, pelvic pain, dysmenorrhea, urinary symptoms, and abnormal uterine bleeding, as well as the findings encountered in transvaginal ultrasound with the International Deep Endometriosis Analysis (IDEA) group protocol. The study's outcome was to determine which symptoms were associated with the findings identified through the IDEA protocol. We perform a chi-square analysis. In total 446 cases were included, of which 317 met the study inclusion criteria. We assessed the symptoms and their correlation with each anatomical compartment to evaluate their involvement. For the anterior component, pelvic pain was related to the finding of hydronephrosis (p > 0.02), and abnormal uterine bleeding was associated with anterior compartment involvement (p > 0.00). For the middle compartment, abnormal uterine bleeding was assessed with the finding of adenomyosis (p > 0.002), dyspareunia with the finding of adenomyosis (p > 0.04), and in the posterior compartment, dysmenorrhea was found to be associated with bowel involvement (p > 0.05). Our results allow us to associate symptoms with the findings evidenced by ultrasound in each of the anatomical compartments of patients with endometriosis.
