This study is the first family-based epidemiological survey of H. pylori in the province, aiming to provide new insights into the status of family-based H. pylori infections in Yunnan Province as well as in the country as a whole. Methods Between 2021 and 2023, 1,176 families with 2,947 family members were tested for H. pylori in six cities in Yunnan Province with different geographic locations and economic status, to assess the status of H. pylori infection, and to clarify the related factors and modes of transmission by means of a questionnaire survey. Results In Yunnan Province, the H. pylori infection rate was 31.46% (927/2947) in people and 54.59% (642/1176) in families.On the individual side, living outside of southern Yunnan (eg, Central OR 1.41, 95% CI 1.10–1.81), having a family member living together for ≥ 1 year (OR 2.43, 95% CI 1.49–3.95), being ≥ 18 years old (eg, 18–44 years old: OR 3.51, 95% CI 1.98–6.23), and gastrointestinal discomfort within the last 1 year (OR 1.25, 95% CI 1.04–1.50) were independent risk factors for H. pylori infection in the study population in Yunnan Province, whereas an education level of college/university and above (OR 0.62, 95% CI 0.48–0.79), and a gastroscopy within 5 years (OR 0.58, 95% CI 0.46–0.73) were independent protective factors for their H. pylori infection. In terms of family, living outside of southern Yunnan (eg, Central OR 1.52, 95% CI 1.07–2.18), and having ≥ 3 people living together in the family (eg, 3–4 people: OR 2.04, 95% CI 1.52–2.72) were independent risk factors for H. pylori infection in study families in Yunnan Province, whereas an annual household income of ≥ 100,000 ( OR 0.68, 95% CI 0.51–0.90), total household living area ≥ 60 m2 (OR 0.62, 95% CI 0.39–0.97), the use of separate meals for intra-household gatherings (OR 0.56, 95% CI 0.38–0.85), and the use of communal spoons and chopsticks (OR 0.38, 95% CI 0.28–0.52) were the independent protective factorse. Conclusion The risk factors of Family-Based H. pylori infection in Yunnan province were closely related to demographic characteristics, personal and family living habits. Close contact with infected family members may be the main cause of transmission. Family-based control of H. pylori infection should be regarded in clinical practice. Pylori is a gram-negative pathogenic bacterium that is specifically localised to the gastric epithelium,In the 1980s, Drs. Marshall and Warren discovered the bacteria through ongoing research [ 1 ] .Since then, research has consistently confirmed that H. pylori chronic infection not only can lead to gastrointestinal diseases such as chronic gastritis, peptic ulcer,intestinal metaplasia, gastric adenocarcinoma and gastric mucosa-associated lymphoid tissue lymphoma [ 2 ] , but also is closely associated with a number of extra-gastrointestinal diseases [ 3 , 4 ] . H. pylori infection is a major risk factor for the development of gastric cancer [ 5 ] , and among the cancers caused by various pathogens, gastric cancer caused by H. pylori infection is the first one, surpassing liver cancer caused by hepatitis viruses and cervical cancer caused by human papillomaviruses [ 6 ] . In 1994, the World Health Organization (WHO) and the International Agency for Research on Cancer (IARC) defined H. pylori as a class I carcinogen [ 7 ] , and so far, H. pylori is the only bacterium with this risk level. It is clear that H. pylori infection-associated diseases are a health threat to families and society. All H. pylori infections should be considered pathogenic and should be eradicated [ 8 ] . H. pylori infection is an infectious disease that can be transmitted from person to person, especially among family members [ 9 , 10 ] . The latest studies show that the rate of H. pylori infection in households in China ranges from 50.27–85.06%, with an average infection rate of 71.21%, and the rate of H. pylori infection among individuals in China ranges from 25.33–59.61%, with an average rate of 40.66%,both show regional variability [ 10 ] . Yunnan Province is located in the border area, where ethnic minorities live, and has unique dietary and living habits. In recent years, because of the favourable climate, more and more families have chosen to visit or even settle in Yunnan, and the health and hygiene conditions in Yunnan are getting more and more attention. There has not been any epidemiological survey of H. pylori in Yunnan Province based on households, and the status of household H. pylori infections, related factors, and modes of transmission have not yet been evaluated. Therefore, in the face of such a severe situation of H. pylori infection prevention and treatment, it is important to understand the current situation of H. pylori infection in households in Yunnan Province, which is of great significance for the prevention and treatment of H. pylori in the province. Materials and methods Study population According to the formula: n = z 2 p(1-p)/d 2 (n, z, p, and d are sample size, z value, positive rate, and acceptable error, respectively), the sample size was calculated, and the prevalence rate of households was about 50%, the test level was 0.05, the confidence level was 0.95, and the permissible error was 3%, and we found out that the Z value was 1.96 by consulting the table of the standard normal distribution, and a total of 1,067 households needed to be included.Considering the additional bias of the convenience sampling method, we finally included 1176 households.The study was conducted from August 2021 to November 2023 in six prefectures of Yunnan Province, including Kunming, Baoshan, Lincang, Qujing, Xishuangbanna Dai Autonomous Prefecture, and Honghe Hani and Yi Autonomous Prefecture(Fig. 1 ), in which 1,176 families and 2,947 eligible volunteers were randomly selected and enrolled in this study. The screening and registration process in each prefecture was carried out by postgraduate students from the Department of Gastroenterology at the First Affiliated Hospital of Kunming Medical University, medical staff from local tertiary hospitals, and trained volunteers.Households with ≥ 2 household members (living continuously for more than 10 months per year) were invited to participate in the survey. Families were excluded from the study if they had a family member who had taken antibiotics, bismuth, or proton pump inhibitors within 4 weeks, a history of major gastric resection, active bleeding from the upper gastrointestinal tract within 1 week, and women who were preparing for pregnancy, were pregnant, or were breastfeeding.Households with ≥ 1 household member infected with H. pylori are recognised as positive households.The study was approved by the Ethics Committee of the First Affiliated Hospital of Kunming Medical University(No. 148, 2022), and all enrolled participants or their guardians complied with the voluntary principle and signed the relevant informed consent forms. Questionnaire All participating family members or their guardians filled out a questionnaire [ 10 ] . The questionnaire mainly included demographic characteristics (e.g., gender, age, occupation, etc.), socio-economic situation (e.g., annual household income and education level, etc.), personal and family living habits (e.g., hand hygiene, family dietary habits, etc.), and past medical history. The questionnaires were completed both electronically and on paper, and specially trained enumerators administered the questionnaires on-site to all participants or their guardians on a one-to-one basis, carefully collecting data and excluding invalid questionnaires. Helicobacter pylori test In this study, H. pylori testing was performed on all included members according to the instructions of the HUBT-20A H. pylori detector and urea [14C] breath test kit (Shenzhen Zhongguo Haidui Biotechnology Co., Ltd.), and the HY-IREXB Carbon 13 Breathalyzer and Urea [13C] Breath Test Diagnostic Kit (Beijing Huagananbang Science and Technology Co., Ltd.).The sensitivity and specificity of the two tests were similar and both had high accuracy. Statistical analysis Statistical analyses were performed using SPSS 25.0 software. Continuous variables were expressed as mean ± standard deviation, and categorical variables were expressed as numbers and percentages. Continuous variables were compared using the t-test or Fisher's exact probability method, and categorical variables were compared using the chi-square test. Risk factor analyses were performed using multifactor logistic regression modelling, and all variables were included in the model(stepwise, sls = 0.10,sle = 0.05). Contrast ratios (OR) and 95% confidence intervals (CI) were calculated, and differences were considered statistically significant at P < 0.05. Result H. pylori infection status of individuals A total of 2947 subjects were examined, 927 were H. pylori -positive and 2020 were H. pylori -negative, giving an overall infection rate of 31.46%.Among them, the infection rate of minors(less than 18 years old) was 11.54% (15/130), and the infection rate of adults was 32.37% (912/2817), with statistically significant differences in the H. pylori infection rates of individuals of different ages (P < 0.05), showing a trend of increasing with age(Fig. 2 ). The infection rate of the southern region of Yunnan was the lowest at 21.24% (100/467), and that of the western region of Yunnan was the The highest 46.10% (248/538), the difference of H. pylori infection rate among individuals in different geographical locations was statistically significant (P < 0.05)(Fig. 4 A). In univariate analysis, the risk of H. pylori infection was higher among those who lived outside of southern Yunnan, had family members living together for ≥ 1 year, were adults, and were married (P < 0.05). In contrast, the risk of H. pylori infection was lower among those with education level of college or above, occupation other than farmer, frequent eating out, gastroscopy within 5 years, and previous H. pylori examination (P < 0.05) (Table 1 ). Of the 1176 households, 45.41% (534/1176) had no member infected, 35.54% (418/1176) had one member infected and 11.14% (131/1176) had all members infected.The lowest infection rate was 43.02% (77/179) in southern Yunnan, and the highest rate was 69.50% (139/200) in western Yunnan, with statistically significant differences in the rate of H. pylori infection in households in different geographic locations (P < 0.05)(Fig. 4 B). Household infection rates increase with the number of family members(Fig. 3 ). In univariate analysis, the risk of H. pylori infection was higher (P 2 persons living together in the household, and drinking heated tap water. In contrast, the risk of H. pylori infection was lower (P < 0.05) in households with an annual household income ≥ 100,000, a total household living area ≥ 60 m², a split meal system for family meals, and the use of communal chopsticks and spoons for family mealse (Table 2 ). The traditional "Test and treat" and "Screen and treat" have long been the strategies for treating H. pylori infection [ 4 ] , however, in recent years, with the advancement of clinical practice and public awareness, H. pylori infection in the family has been more widely noticed, and more and more studies and guidelines have pointed out that H. pylori can be transmitted between people, especially among family members, and that H. pylori infection has an obvious family aggregation [ 4 , 9 ] . Therefore, preventing and controlling infections among family members is a practical way to block the chain of transmission and reduce the rate of infection in society. Starting from the new important strategy of "family-based H. pylori infection control and management" [ 11 ] , Yunnan Province conducted the first family-based H. pylori epidemiological survey in the province, which provided a new understanding of the status of family-based H. pylori infections in Yunnan Province as well as in the whole country. The results of this study showed that the H. pylori infection rate in Yunnan Province was 31.46% (927/2947) in individuals and 54.59% (642/1176) in families, with more than half of the families infected with H. pylori , and the rate of H. pylori infection in families was significantly higher than that in individuals, which is in line with previous studies [ 12 ] . In terms of the regional distribution of infections, there were significant differences in the prevalence of H. pylori infections among the four different geographic locations, namely, southern Yunnan, central Yunnan, northeastern Yunnan, and western Yunnan, both in terms of individuals and families.It can be seen that there are regional differences in H. pylori infections, which can be caused by a variety of factors such as socio-economic conditions, cultural customs, and living and dietary habits, etc. Therefore, it is very important that prevention, control, and treatment strategies be tailored to the needs of the local context, and, of course, the artificial interventions adopted in the prevention and control of H. pylori infections in different countries and regions are very important. Of course, the human intervention in the prevention and control of H. pylori infection in different countries and regions is also an influencing factor of the local epidemiological trend.However, there was no significant difference in the prevalence of H. pylori infections between different regions in Hainan Province [ 9 ] . In terms of changes in infection rates, a continued decline in H. pylori infection rates has been reported in many parts of the world, including South Korea [ 13 , 14 ] , China [ 15 ] , Austria [ 16 ] , Iran [ 17 ] , and Malaysia [ 18 ] .Meta-analyses in recent years have shown that the global H. pylori infection rate has declined from 58.2% during the period 1980–1990 to 43.1% during the period 2011–2022 [ 19 ] , and that the H. pylori infection rate in our country has declined by 0.9% per year during the period 1983–2018, from 63.8% in 1983–1994 to 2006–2018 as 46.7% [ 20 ] . A Japanese study predicts a further decline in H. pylori prevalence by 2050 [ 21 ] .The continuous decline in infection rates at home and abroad is related to improvements in education, the economy, and sanitation, as well as increasing public health awareness. In recent years, H. pylori health education has been continuously carried out in our province to standardise the diagnosis and treatment of H. pylori , and has been improved in clinical practice. In this study, the overall personal infection rate (31.46%) and the personal infection rate (21.41%) in the southern Yunnan region of our province were lower than the survey data of Wang Lan in 2018, 45.29% and 43.01%,and the personal infection rate (28.8%) in the central Yunnan region was also lower than the survey data of Jia Demei et al. in 2013–2015, 33.5%; in addition, the household infection rate, In addition, the household infection rate (54.59%) and individual infection rate (31.46%) are lower than the national household infection rate (71.21%) and individual infection rate (40.66%) in 2022, which shows that the prevention and treatment effect is remarkable. Yunnan Province is located in the southwestern border of China, dominated by mountains and plateaus, with great terrain undulations, obvious climatic differences in different places, and the concentration of ethnic minorities in the territory, creating unique dietary habits and living conditions in different places. In this study, the H. pylori infection rate of families and people in southern Yunnan, such as Xishuangbanna and Honghe, was significantly lower than that of people in central Yunnan, northeastern Yunnan, and western Yunnan (P < 0.001), which may be related to the fact that people in southern Yunnan like to eat spicy and stimulating foods, and pepper oil, chilli pepper, and garlic are added to their dipping sauces, which were found to have an antimicrobial or bacteriostatic effect on Helicobacter pylori in the stomach in previous studies [ 22 – 25 ] . H. pylori is mainly transmitted within the family through the oral-oral, faecal-oral, and shared utensil routes and is difficult to self-clean after infection [ 11 ] .Longer time of family members living together means that the exposure time and frequency of certain risk factors increase, leading to the formation of oral-oral or faecal-oral chain of transmission of H. pylori within the family through some medium, which results in infections.National studies have shown that when couples cohabit from 5 years to 30 years, the The prevalence of infection increased from 17–22%-24% [ 10 ] , and in this study it was also shown that the prevalence of H. pylori infection was 2.43 times higher in those whose family members had been living together for ≥ 1 year than in those who had been living together for < 1 year (p < 0.001), which in some sense proves the clustering of infection in the family. Regarding the relationship between age and H. pylori infection, different studies have different views. Alarid-Escudero et al. [ 26 ] conducted a mathematical model to assess the potential risk of H. pylori infection in Mexico, and this model estimated that H. pylori infection force is highest at birth and decreases to zero with age.In this study, the H. pylori infection rate increased with age, and the old age group (≥ 60 years old) had the highest infection rate, which is consistent with the study of Danni Liu et al. [ 9 ] in Hainan, and the H. pylori infection rate of the middle-aged group (45–59 years old) and the old age group (≥ 60 years old) exceeded the average infection rate of Yunan Province, which is a priority for the prevention and control of H. pylori infection in these two age groups. The relationship between H. pylori infection and ethnicity has been reported differently in different studies. The ethnic structure of Malaysia consists of three main Asian races including Malays, Chinese and Indians. Indians had the highest prevalence of H. pylori infection (> 50%), followed by Chinese (40%-50%), while Malays had a relatively low prevalence of about 10%-20% [ 18 ] , suggesting that H. pylori spreads predominantly within ethnicity. In contrast, there was no significant difference in the prevalence of H. pylori infection between the Han nationality and ethnic minority populations in the study in Hainan Province [ 9 ] , which is consistent with this study. The residence status of all ethnic groups in China is "big mixed residence, small settlement", and this form of distribution is conducive to the interaction of all ethnic groups, and the free intermarriage between different ethnic groups may make the influence of host genetic factors and the distribution of strains among ethnic groups less and less. Higher educated people have good hygiene and dietary habits, and have higher acceptance and compliance to health education information, and this study shows that higher educated people have lower H. pylori infection rate, which is consistent with related research reports [ 10 , 27 ] . Traditionally, many people believe that eating out is a hygiene hazard and that it is easy to "spread disease from mouth to mouth". Previous studies have shown that people who eat out more frequently are at higher risk of H. pylori infection [ 10 ] . Interestingly, the results of this study showed that the risk of H. pylori infection was significantly lower in those who ate out more frequently (more than 2 days per week) than those who rarely ate out. People in Yunnan often use fermented foods for flavouring dishes, especially in the catering industry, such as tempeh and pickles, in which the probiotics may inhibit the growth of H. pylori . In recent years, with socio-economic development and greatly improved hygiene conditions, the catering industry has stricter hygiene standards, and the technology and frequency of disinfection of tableware even exceeds that of some families with low hygiene concepts. After the tableware has been sterilised and divided, the risk of infection is reduced. In addition, when eating out, separate meals and the use of communal chopsticks are more common, which reduces the chance of cross-infection brought about by sharing tableware. Dining out offers a wide variety of food choices, and the intake of vegetables and fruits is rich in antioxidants, which may help to suppress H. pylori. Moreover, people who eat out more often are usually better off and have access to better medical services and health protection. These factors above are not absolute and are subject to further research. H. pylori infected people present with varying degrees of gastrointestinal symptoms, and the prevalence of H. pylori infection was higher among people with gastrointestinal symptoms in the past 1 year in this study, so it is more important for symptomatic people to be screened for H. pylori in time to prevent further transmission and the development of related diseases. However, this was not found in previous studies. Currently, advanced endoscopic technology allows for a high degree of accuracy in the diagnosis of gastrointestinal lesions, and most gastroenterologists will ask patients to complete an H. pylori test if gastroscopy reveals a lesion, and will recommend eradication treatment if there is an infection with no contraindications to eradication, and if there is no infection, some patients will also undergo outpatient counselling or Internet consultation, which will increase their awareness of H. pylori , and then undergo H. pylori screening. In this study, the H. pylori infection rate was lower in those who had undergone gastroscopy within 5 years compared to those who had not, which was considered to be related to the above factors. With a large family size, the living environment is relatively crowded, with poor environmental hygiene, close contact between family members, and the superposition of risk factors, which provide favourable conditions for the spread of H. pylori within the family, and the present study found that families with a large number of people living together had a high rate of H. pylori infection, which is consistent with previous studies [ 10 ] . Annual household income and household living area represent the socio-economic status of a family. Families with high annual household income and large total living area also have relatively better family hygiene, living conditions, education level and working environment, and the risk of H. pylori infection is lower. Firstly, better-off families can afford better healthcare services, including H. pylori testing and treatment, which can reduce infection rates. Secondly, higher-income families usually have larger family living space, better living and hygienic conditions, such as better ventilation and lighting conditions, clean drinking water and food, clean kitchens and bathrooms, and regular cleaning of furnished environments, which can help to reduce bacterial growth and transmission. Lastly, people with a high level of education usually have a better sense of hygiene and are able to take effective preventive measures, such as washing hands frequently and using communal chopsticks, etc. They are also more inclined to undergo regular medical check-ups and seek timely medical treatment, which can help in early detection and treatment, thereby reducing the infection rate. All these factors tend to interact with each other. For example, people with a higher level of education usually have a higher income and are able to improve their living conditions, further reducing the infection rate. Our findings also confirm that households with higher socioeconomic levels have lower H. pylori infection rates, consistent with previous studies [ 28 – 30 ] . In addition to humans, some animals, such as cattle and sheep, may also be hosts for H. pylori [ 31 ] , although no correlation was found between having animals in the home and H. pylori in the home in this study. In Bogotá, Colombia, long-term sampling of water sources showed that H. pylori DNA was found in 31% and 38.7% of raw water and drinking water samples, respectively [ 32 ] . In Peru, the intake of unboiled water (93.9%) and watercress (79.6%) were found to be the most important factors associated with H. pylori infection in children [ 33 ] . In the United Arab Emirates, the prevalence of H. pylori was significantly higher among tap water drinkers than among bottled water drinkers [ 34 ] . In the present study, the risk of H. pylori infection was higher in households consuming heated tap water compared to those consuming bottled water. Therefore, untreated tap water should not be used as a source of drinking water in the home. The oral cavity is the first gateway for H. pylori to enter the human body, and many scholars have applied molecular biology and immunological methods to detect oral H. pylori in dental plaque, saliva, tongue, and dental pulp [ 35 – 38 ] , and H. pylori DNA has even been detected on chopsticks [ 39 ] .In Chinese families, the use of chopsticks to hold food when sharing a meal with family and friends has been a part of the Chinese culture for centuries, and the practice of mouth-to-mouth feeding of young children is a common phenomenon in families. The feeding of young children with mouth-to-mouth is a frequent occurrence in families, which inadvertently transmits pathogens through the oral component.Z. Ding et al. [ 40 ] found that eating alone was an independent protective factor against H. pylori infection.Using of communal chopsticks and spoons reduced the risk of H. pylori infection in the family [ 10 ] .In the present study, the low rate of H. pylori infection in families that used communal chopsticks and spoons and a split-meal diet is in line with previous studies, and provides evidence for the oral-oral route of transmission, and emphasises the importance of implementing home-based control and management of H. pylori infection in clinical practice. To raise public awareness of the prevention of H. pylori infection, good hygiene practices should be further promoted through community health activities, media campaigns, school education, etc., taking into account the local conditions. Because the sample size was large enough, the common method of performing univariate analyses was used and then all the variables were included in the multivariate model and stepwise regression was used for the control of variable inclusion. Also for categorical variables such as geographic location, education level, etc., they were converted to dummy variables to view the stratification effect, and age as a continuous variable was also grouped. Confounding factors in this study have been controlled as much as possible. There are some limitations to this study. First, the survey used a convenience sampling method, which may limit generalisations and bias the overall selection, but because of the large sample size, the impact was small. Second, the detection method used only the urea breath test, which can only represent a presenting infection and can be made more accurate by combining it with serological or faecal antigen testing. Third, this work was conducted in Yunnan Province and the definition of geographical areas is not precise in terms of latitude and longitude, the results may not be applicable to other regions. Fourth, The 50% prevalence assumption for sample size calculation lacks previous epidemiological surveys and may not reflect the true baseline rate. We acknowledge any bias in data collection or reporting. Conclusions The overall rate of H. pylori infection in Yunnan Province is lower than the national average, with variations by region.There is family aggregation of H. pylori infection, and the risk factors for infection are closely related to demographic and sociological characteristics, family characteristics, and individual and family living habits. Therefore, the prevention and control of H. pylori infection should be tailored to local conditions, and comprehensive measures should be developed on a family basis.
