This case report describes a 23-year-old woman admitted with a new, tender, non-reducible left groin bulge initially evaluated as an indirect inguinal hernia, using physical exam plus ultrasound and computed tomography to identify a tubular cystic structure tracking along the left inguinal canal and expected round ligament path. Intraoperatively, a 6-cm dark purple cystic structure was found within the hernia sac, dissected and opened to reveal clear fluid, followed by left indirect inguinal hernia repair with absorbable mesh and excision of a round ligament hydrocele. Final pathology showed a hernia sac lined by fibromuscular tissue and mesothelium, and a hydrocele with concordant endometriosis, with an uneventful postoperative course and discharge on postoperative day 1. As a single-patient report, it provides limited generalizability and does not include systematic diagnostic or therapeutic comparisons. This paper is centrally about endometriosis — it reports isolated left-sided inguinal endometriosis contained within an inguinal hernia sac along the round ligament.
