Analysis of outcomes and the related factors of the expectant pregnancy in the patients with endometriosis-associated infertility after laparoscopic treatment

Abstract

Objective:To investigate the expectant pregnancy in patients with endometriosis-associated infertility after laparoscopic treatment.Methods:The patients who underwent the laparoscopic treatment for endometriosis during Jan.2004 to Dec.2006 in this hospital were followed up to assess fertility outcome.The relationship between the duration after the surgery and the expected pregnancy was analyzed.Logistic regression of pregnancy rate versus age,GnRH-a treatment and the staging of patients by EM r-AFS classification system was used to explore the factors related to fertility outcome within 12 months.Results:Ninety-three in 231 patients were found pregnant,with 76 cases during 1 to 12 months,15 during 13 to 24 months,and 2 during 25 to 36 months post-operation.The pregnant rate in 12 months,13 to 24 months,and 24 to 36 months was 32.9%,13.51%,and 3.33% respectively.The differences of the pregnant rates were statistically significant.P value of Wald test in Logistic regress model for the variables of age,administration of GnRH-a and the staging of endometriosis was 0.422,0.917 and 0.219 respectively,which showed that,age,administration of GnRH-a or the staging of endometriosis had no relationship with the fertility outcome within 12 months post-operation. Conclusions:The patients with endometriosis-associated infertility have the natural pregnancy chance after laparoscopic treatment if the salpinx works properly.The expectant time for pregnancy may not be set as longer than 12 months.If the ovary's function is normal,age is not an important factor.Using GnRH-a after surgery does no good for the pregnancy rate,and the EM r-AFS staging has the limited value in predicting the pregnancy chance.
