Lifesaving interventionscan be used for mothers and babies when vaginal delivery is contraindicated. If not medically indicated or if performed under suboptimal conditions, CD can cause maternal and fetal complications. Objective: This study aimed to assess the indication and outcome of CD and associated factors for cesarean delivery in Addis Ababa, Ethiopia. Methods: An institutional-based cross-sectional study was performed. A total of 422 CDs from Yekatit 12 Hospital Medical College, Janmeda Health Center and Kotebe Health Center were included in the study. Mothers and newborns were followed for the first seven days of life, at the time of discharge, or at the time of death; whichever came first was considered the last time of data collection. The statistical analysis was carried out using SPSS version 25. Descriptive statistics are presented as frequencies. A chi-square test was performed to evaluate the relationships between the dependent and independent variables. A P value of less than 0.05 was considered to indicate statistical significance. Results and Conclusion: Emergency CDaccounts for 75.4% of the total cesarean deliveries. From clinical indications, a nonreassuring fetal heart rate pattern was the most common indication, followed by cephalopelvic disproportion. 5.9% of mothers had maternal complications. Repeat CD, emergency CD and mothers with antepartum hemorrhage were shown to be associated with maternal complications. The neonatal death rate was 3%. Nulliparity, hypertensive disorder during pregnancy, gestational age, the APGAR score and the need for immediate neonatal resuscitation were found to be significantly associated with neonatal death. delivery cesarean delivery outcome indication maternal complication neonatal death Figures Figure 1 Figure 2 INTRODUCTION In a woman's life cycle, parturition and pregnancy are significant milestones. Pregnant women may deliver their children via normal spontaneous vaginal delivery or through CD. One of the key indicators of a country’s health status is the magnitude of maternal and perinatal mortality and morbidity. This is reflected in the quality of obstetric care. Although giving birth is a physiological process, there is a large risk to the life and wellbeing of both mothers and newborns. Approximately 10% of all deliveries are considered high risk, some of which require cesarean section.( 1 , 2 ) CD was defined as the birth of a fetus, placenta or membrane from the uterus through an abdominal incision after 28 weeks of gestation. The terms cesarean delivery or cesarean birth are preferred over cesarean section (CS). Primary cesarean refers to a CD performed on a woman without a prior cesarean birth, whereas repeat cesarean refers to a CD performed on a woman who had a cesarean birth during a previous pregnancy.( 3 – 5 ) Global CS rates have risen dramatically over time, from approximately 7% in 1990 to 21% today, exceeding the WHO’s recommended acceptable rate of between 10% and 15%. These trends are expected to continue during the current decade, with overuse and unmet needs coexisting at a projected global rate of 29% by 2030. The primary cesarean section rate has also increased globally due to modern obstetrics practices for medical, social, economic, and legal reasons.( 2 , 6 ) When medically justified, a CD can effectively prevent maternal and perinatal mortality and morbidity.( 7 – 9 ) However, there is no evidence showing the benefits of CD for women or infants who do not require the procedure.( 10 – 13 ) If not medically indicated or if performed under suboptimal conditions, CD can cause maternal and fetal complications, including death.( 14 ) These risks are greater for women with limited access to comprehensive obstetric care.( 10 ) There is currently no internationally accepted classification system for CD that allows meaningful and relevant comparisons of CD rates across different facilities, cities or regions.( 5 , 11 ) Ethiopia, a nation in sub-Saharan Africa, significantly contributes to maternal and newborn morbidity and mortality. To lower this statistic, the country is making significant efforts in multiple direction. This includes the availability of compressive obstetric care in health institutions and the need to perform cesarean section in those health institutions for scientific reasons based on the WHO recommendation.( 1 , 8 , 15 ) Owing to the dearth of local data on this topic, this study aimed to identify the common indications for CD, fill in the lack of local data on the indications and outcomes of CD, and provide valuable contributions to national data. Recognition of the factors that affect maternal and neonatal outcomes of cesarean section will help health professionals and pregnant women prepare themselves.( 8 ) METHODS AND MATERIALS Study area An institution-based cross-sectional study was performed to assess the indication and outcome of CD and associated factors for maternal and neonatal outcomes of CD in Addis Ababa, Ethiopia. In Addis Ababa, CD is diagnosed at hospitals and health centers, which provide comprehensive obstetric maternal and neonatal care. The study area was selected based on the case burden. Yekatit 12 Hospital Medical College was selected as the hospital medical college. The annual delivery rate at the college hospital was estimated to be 8000-10,000, with a CD rate ranging from 35–45%. Among the health centers, the Janmeda health center and Kotebe health center were selected. The annual delivery of each health center was estimated to be 2000–2500, with a CD rate ranging from 25–30%. Mothers who delivered a CD in the study area and who provided consent were included. Sample size and sampling The sample size was calculated with the assumptions of a 50% proportion, 5% level of significance, 5% margin of error, and 10% nonresponse rate. The total sample size was calculated as 422 (CD). Systemic random sampling was used to select the study population. Samples were proportionally allocated for each selected health facility, and 322, 50, and 50 samples were drawn from Y12HMC, the Janmeda Health Center and the Kotebe Health Center, respectively. Data collection tool and process The study was approved by the institutional review board of Y12HMC. Prior to the main study, 5 days of training was given for the data collectors. A pilot study was performed at Y12HMC using 5% of the sample. Then, sociodemographic data, obstetric history and maternal and neonatal outcomes were collected using a structured questionnaire. Data analysis The data were double entered into the SPSS version 25 statistical package. Descriptive statistics Frequencies (mean, medians and proportions) were used to describe some variables. A chi-square test was performed to evaluate the relationships between the dependent and independent variables. Two-tailed P values were calculated, and a value of less than 0.05 was used for statistical significance. Tables and graphs were used to present the results. Variables Maternal outcome, neonatal outcome, and indication of CD were the dependent variables. The independent variables were maternal age, parity, antenatal care, prior cesarean delivery, number of pregnancies, mode of anesthesia, gestational age, birth weight and APGAR score. OPERATIONAL DEFINITIONS DELIVERY was defined as a CD attended at Yekatit 12 Hospital Medical College, Janmeda Health Center or Kotebe Health Center at or above 28 weeks of gestation. MATERNAL OUTCOME- Any medical or surgical complication occurring during or after surgery and before discharge that requires therapeutic antibiotic use, therapeutic anticoagulant use, blood transfusion, relaparotomy, or maternal death. (yes or no). ( 11 ) NEONATAL OUTCOME- The death of the neonate in the first 7 days of life and before discharge. (yes or no). ( 11 ) RESULTS The mean maternal age of the study participants was 26.98 years (SD = 4.74). The majority of the study participants were urban dwellers (n = 399; 94.5%) and were married (n = 412; 97.6%). Analysis of educational status showed that 188 (44.5%) of the study participants had attended secondary school, followed by 123 (29.1%). Approximately 410 (97.2%) of the study participants had at least one antenatal contact (ANC) for their current pregnancy; however, 12 mothers (2.8%) had no ANC and were unbooked. Only 23 (5.5%) mothers had a bad obstetric history. Thirteen mothers had a history of stillborn birth, and 10 mothers had early neonatal death. There was spontaneous onset of labor in 191 (45.3%) mothers. However, CD was performed on 101 (23.9%) mothers after labor induction, and 130 (30.8%) mothers had direct cesarean section for different obstetrics or medical indications. Approximately 104 mothers (24.6%) had elective CD, which was performed after scheduling for at least 8 hours. However, 318 mothers (75.4%) had emergency CD for different obstetric indications without scheduling. The nonreassuring fetal heart beat pattern (NRFHRP) emerged as the most common clinical indication for CD in our study and was detected for 170 mothers (40.3%). The second leading indication was cephalopelvic disproportion (CPD), which was found for 86 mothers (20.4%). Among the modes of anesthesia used, spinal anesthesia was the most common type of anesthesia used for 408 mothers in our study (96.7%). General anesthesia was used for 14 mothers (3.3%), all of whom were for emergency CD. Approximately 25 mothers (5.9%) had at least one maternal complication that was diagnosed before discharge. The most common complication was postpartum hemorrhage (PPH), which was diagnosed in 15 mothers (3.6%), of which 3 mothers required relaparotomy and 13 mothers required blood transfusion. Puerperal sepsis was the second most common complication and was diagnosed in 7 mothers (1.7%). All the patients were managed with therapeutic antibiotics. There was no maternal death in our study. On the 7th postoperative day, 415 mothers (98.3%) were discharged, with an average hospital stay of 2.27 days ( ± 0.776). Of all the deliveries, fifty-seven mothers (13.5%) did not recall LNMPs and did not have early-mile stones. Out of 422 deliveries, there were a total of 18 multiple gestations (all were twins). This process results in 440 total babies being delivered. According to the sex distribution of the newborns, 226 (51.4%) were female, and 214 (48.6%) were male. The majority of the newborns (93.2%) were found to be vigorous, with a 5-minute APGAR score of 8 or above. In 22 neonates (75.9%), neonatal hypothermia was diagnosed in the NICU, and 82% of the neonates’ temperature was corrected within 6 hours of admission. More than half of the newborns (51.7%) who were transferred to the NICU were managed with IV antibiotics (15 newborns). CPAP was given for 7 newborns (24.1%). Among the 7th -day newborns who were admitted to the NICU, 11 (37.9%) were discharged, 5 (17.3%) were still in the NICU, and 13 (44.8%) died. (Table 7 ) The neonatal death rate of all babies (440) was 3%. The test results revealed that mothers with a prior history of CD had greater maternal complications than did those who had primary surgery. Similarly, mothers who underwent emergency CD were more prone to significant complications than were those who underwent elective CD. Furthermore, maternal history of hypertension was also shown to be associated with neonatal survival (Table 9 ). However, neither NICU admission diagnosis nor hypothermia at admission was found to be associated with neonatal survival. A similar order of indications was found in a prior study performed in Ethiopia.( 16 ) Unlike in many other countries worldwide, in our study, the rate of repeat CD was lower (23.2%). ( 17 , 18 ) This finding is similar to most study findings in Ethiopia.( 8 , 19 ) This variation may be related to variation in trials of labor after cesarean delivery policy, which needs further study. In addition, similar to most of the studies in developing countries, our study found that the incidence of emergency CD was greater (75.4%) than that of elective CD.( 16 , 20 , 21 ) Maternal complications were found in 5.9% of mothers in the study group. The overall post-CD maternal morbidity was similar to that reported by St. Paul’s Hospital Millennium Medical College (5.2%).( 1 ) However, this rate was slightly lower than that reported by the Ayder Comprehensive Specialized Hospital (8.5%). ( 17 ) However, these findings are similar to most previous study results in other parts of Ethiopia, which ranged from 4.7–9.7%.( 15 ) The lower incidence of post-CD maternal complications in this study can be explained by the greater rate of ANC coverage in our study (97.2%), which was greater than that reported in 2016 by the National Health Survey (EDHS) (62%) but comparable to the antenatal coverage in Addis Ababa (97%).( 12 , 22 ) The morbidity and mortality associated with CD occur not only because of the surgical procedure used but also because of the anesthesia used. The choice of general or spinal anesthesia also has many controversies regarding maternal and neonatal safety.( 16 , 17 ) In our study, spinal anesthesia was the most common type of anesthesia and accounted for 96.7% of all surgeries, which is much greater than that reported by the Felegehiwot referral hospital (49.7%). ( 4 ) As mothers need to be scheduled for at least 8 hours during elective procedures, there will be time for medical and psychological preparation for both mothers and physicians. However, emergency procedures do not provide this opportunity. For this reason and by others, in our study, emergency CD was more strongly associated with maternal complications than mothers who underwent elective procedures. Similarly, high maternal complication rates associated with emergency CD were reported in Faisalabad. ( 20 , 21 ) The national proportion of neonatal deaths decreased from 4.8% in 2000 to 2.9% in 2016, but the proportion varies among administrative regions of Ethiopia. ( 10 ) The survival rate of newborns was 97%, and the death rate was 3%. The incidence of neonatal death was comparable to that reported by St. Paul’s hospital (2.8%) and by West Tigray (2.4%). ( 1 , 23 ) Obstetric complications correlated significantly with infant survival. One explanation could be that newborns who were born to mothers with obstetric complications were taken care of better than were those born to mothers of other types. These include administering dexamethasone and communicating early with NICU teams.( 9 ) Except for hypertensive disorders during pregnancy, which were found to be significantly associated with neonatal death, obstetrics and maternal complications were not found to be associated. Regarding neonatal variables, unlike other studies in which the female sex has continued to show an advantage over the male sex ( 7 , 14 ), sex was not found to be associated with the survival rate in our study. In our study, 57 mothers (13.5%) did not recall LNMPs or early-mile stones. Because of these limitations and the known discrepancy between antenatal and postnatal assessments of GA, excluding such mothers was found to be a more practical approach for determining the association between gestational age and survival. This problem has also been mentioned in similar papers from other developing countries.( 1 , 11 ) The remaining 365 women were analyzed for an association between gestational age and survival, and a statistically significant association was found between gestational age and survival rate. As evidenced by many studies, the immediate requirement for neonatal resuscitation is a predictor of poor neonatal outcomes. ( 11 , 24 ) Similar results were found in our study, as newborns who required immediate resuscitation had higher mortality. Improvements in intensive neonatal care units and medical technologies have played major roles in increasing the survival rate of infants in recent decades. ( 5 , 24 ) Out of the 440 newborns included in the study, 29 were admitted to the NICU. This resulted in a NICU admission rate of 6.6%. There were 13 neonatal deaths in our study. The NICU death rate was 44.8%. A report from St. Paul’s Hospital showed a high NICU admission rate (18.6%) but a lower NICU death rate (26.3%).( 1 ) However, neither NICU admission diagnosis nor management was found to be associated with neonatal outcome. In conclusion, the analysis of this study revealed that the leading indication for CD was NRFHRP, followed by CPD and previous CD. 5.9% of mothers had maternal complications; the leading complication was PPH. A prior history of CD, emergency CD and a mother with APH were found to be associated with maternal complications. There were 3% neonatal deaths on the first seven days. Nulliparity, hypertensive disorder during pregnancy, gestational age, birth weight, 5th minute APGAR score and requirement for immediate neonatal resuscitation after birth were found to be associated with the survival of newborns. The main limitations of the study were that the population included in the study was homogeneous with respect to several sociodemographic characteristics, and the results were limited by short-term inpatient stays. Moreover, the data were not fit for regression tests to measure the strength of the association between the dependent and independent variables. Therefore, further studies with longer study periods are recommended to determine the actual impact of CD on both maternal and neonatal outcomes and to determine the strength of the associations of different variables with these outcomes. Declarations Ethics Approval and Consent to Participate: This study was conducted in accordance with the ethical standards of the Declaration of Helsinki. Ethical clearance was obtained from the Institutional Review Board (IRB) of Yekatit 12 Hospital Medical College. A total of 422 CDs from Yekatit 12 Hospital Medical College, Janmeda Health Center and Kotebe Health Center were included in the study. Mothers and newborns were followed for the first seven days of life, at the time of discharge, or at the time of death; whichever came first was considered the last time of data collection. The statistical analysis was carried out using SPSS version 25. Descriptive statistics are presented as frequencies. A chi-square test was performed to evaluate the relationships between the dependent and independent variables. A \u003cem\u003eP \u003c/em\u003evalue of less than 0.05 was considered to indicate statistical significance.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults and Conclusion: \u003c/strong\u003eEmergency CDaccounts for 75.4% of the total cesarean deliveries. From clinical indications, a nonreassuring fetal heart rate pattern was the most common indication, followed by cephalopelvic disproportion. 5.9% of mothers had maternal complications. Repeat CD, emergency CD and mothers with antepartum hemorrhage were shown to be associated with maternal complications. The neonatal death rate was 3%.
