EP31.20: Endometriosis and frozen pelvis: an ultrasound study

Abstract

To describe the frequency of ultrasound findings in patients diagnosed with laparoscopic sealed pelvis. This was a retrospective observational study based on data collected at a University Hospital with an advanced gynecologic ultrasound unit between 2018 and 2022. Women with a diagnosis of endometriosis and frozen pelvis by laparoscopy were included. Findings identified on transvaginal ultrasound with the International Deep Endometriosis Analysis (IDEA) group protocol were assessed. A descriptive analysis was performed using absolute frequencies. 317 cases of patients with endometriosis were evaluated, of which 100% (12) cases had a diagnosis of advanced endometriosis with pelvic adhesions by laparoscopy. 66% (8/12) of the patients had ultrasound findings for the anterior compartment, with 8% (1/12) showing involvement in the urinary tract. For the middle compartment, 100% (12/12) had ultrasound findings, with adenomyosis present in 66% (8/12) and endometrioma in 50% (6/12) of the cases. Involvement in the posterior compartment was observed in 83% (10/12) of the cases, with lesions in the parametrium in 83% (10/12) of the patients. In patients with endometriosis involving the middle and posterior compartments, there is a tendency to find a frozen pelvis. Therefore, it is suggested to consider these findings for adequate surgical planning.
