Patients' satisfaction with these services is an essential aspect of healthcare quality. Even though several studies have been conducted on patient satisfaction in different health services, studies on radiology services are rare in Ethiopia. This study assessed patient satisfaction with radiological service and associated factors at Adama Hospital Medical College. Methods A facility-based cross-sectional study was conducted among patients visiting the radiology department from November 01–30, 2023. A systematic sampling technique was used, using the point of exit as a frame, and every k-interval at each unit was interviewed. Data were collected by using an interviewer-administered questionnaire on exit. The collected data were cleaned, entered by Epi-Info 7.2, and analyzed using SPSS version 25. Descriptive statistics were used to present the results. Binary logistic regression was used to get the candidate variables for multivariable logistic regression analysis in which variables with p < 0.25 were selected as candidate variables for multivariable logistic regression analysis. Multivariable binary logistic regressions with odds ratios and 95% confidence intervals were used to identify the association between dependent and independent variables. Statistical significance was declared at p < 0.05. Result A total of 322 patients participated in the study, making a 98.8% response rate. In this survey, 280(87%) patients expressed satisfaction with radiological services overall. Waiting for 6–12 hours (AOR = 0.289, 95%CI: 0.09–0.926), having negative experiences (AOR = 0.191, 95%CI: 0.074–0.494), and having a negative perception about the overall radiology services (AOR = 0.408, 95%CI: 0.197–0.847) were significantly associated with patients' satisfaction towards overall radiology services. Conclusions This study found that the majority of respondents were satisfied with the radiological services, which was higher when compared to other studies' findings in Ethiopia. Waiting for diagnostic results, perception, and experiences about the overall radiology services were associated with patients’ satisfaction. Attempts should be made to reduce waiting time, and staff should work on boosting patients’ perception and attitude towards the radiology services to further improve the satisfaction rate to a higher level.
