However, the absence of strict regulations on YouTube questions the reliability of the available content. Hence, the main objective of this study is to evaluate the scientific accuracy of YouTube videos addressing TMJ arthrocentesis. Top of Form MATERIALS AND METHODS This study analyzed50 videos related to TMJ arthrocentesis by three Oral and Maxillofacial surgeons. Data on the video source, duration, upload date, time elapsed since upload, total views, likes, dislikes, comments, interaction index, and viewing rate were collected and analyzed. Video quality was assessed using the Global Quality Scale (GQS) and the Quality Criteria for Consumer Health Information (DISCERN). RESULTS Out of the 50 videos, 10 used the single-needle technique, while the remaining 40 used the 2-needle technique. Following joint irrigation, medications such as steroids were used in 10 videos, hyaluronic acid in 5, and IPRF in one. The mean total number of video views was 137,419.16, with a mean interaction index of 1.51 and a mean viewing rate of 2,803.28. The mean GQS and DISCERN scores were 2.25, and 1.94 respectively. CONCLUSION YouTube can be a valuable resource for learning, it should not replace formal education or hands-on training if you plan to perform TMJ arthrocentesis as a medical professional. Always consult with experts and follow best practices when performing medical procedures. TMJ Arthrocentesis YouTube Internet GQS DISCERN INTRODUCTION The temporomandibular joint (TMJ) arthrocentesis procedure, first introduced by Nitzan et al[ 1 ] in 1991, has evolved over the last three decades for treating various temporomandibular joint disorders (TMDs). TMJ arthrocentesis is considered the minimally invasive intervention for patients unresponsive to conservative treatments like drugs, physical therapy, or splint therapy[ 2 ]. This procedure comprises lavage of upper joint space with copious amounts of fluids like ringer lactate or normal saline through needles or cannulas via single or double puncture technique, under local or general anesthesia or intravenous sedation[ 3 , 4 ]. The rationale of this technique is to lyse adhesions and flush out inflammatory mediators improving the functional outcomes in the conditions like anchored disc phenomenon, non-reducing disc displacement, osteoarthritis, and TMJ arthralgia[ 5 ]. In recent years, YouTube has emerged as a prominent platform for sharing and accessing medical information[ 6 ]. There has been a significant rise in the number of videos dedicated to the arthrocentesis of TMJ available on this platform. These videos provide information about procedures enabling the viewers to improve their understanding of technique and its benefits. Healthcare professionals also can utilize these videos as supplementary educational resources to enhance their knowledge regarding the latest techniques. Also, by watching real-life experiences from individuals who had gone through the procedure firsthand, viewers can gain insights into what to expect, potential outcomes, and post-operative care. Nevertheless, YouTube videos on oral surgery differ widely in terms of their credibility, and informative content,[ 7 ] and were found inconsistent [ 8 ]. In recent years there has been a remarkable growth in video uploads regarding medical information on YouTube but the videos being uploaded are free of cost and are uncensored for their content, quality, or reliability. Thus, it becomes quite confusing for the patients to separate as helpful or useless, making the situation misleading. To clear this obscurity, the study aims to conduct scientific research on the quality and reliability of YouTube videos on the arthrocentesis of TMJ. MATERIALS AND METHODS STUDY DESIGN The cross-sectional observational study was structured by maxillofacial surgeons. Our research utilized the keywords "TMJ arthrocentesis," "TMJ lavage," and "TMJ click," which were identified through the Google Trends application( www.trends.google.com ). It is essential to highlight that our study, which relied solely on publicly accessible data, did not need approval from the ethics committee. DATA COLLECTION On August 22, 2023, we individually entered these keywords into the YouTube search bar and employed the "relevance" filter to ensure the retrieval of the most pertinent results. Recognizing the potential variability in search results over time, we established a playlist to preserve the initial search findings. We compiled a total of 500 videos that were deemed highly relevant to the specified search terms. Within this selection of videos, we excluded those with a duration of less than 20 seconds, those containing advertisements, videos intended for entertainment purposes, repetitive content, and those not in English. Following this screening process, we retained 50 videos for inclusion in our study. For each of these 50 videos, we conducted a comprehensive assessment, documenting various general parameters, including the number of views, likes, dislikes, comments, days since upload, video length in minutes technique of the procedure, and irrigating material used for the study. We evaluated viewer engagement by calculating the interaction index and viewing rate. EVALUATION OF THE VIDEOS The videos that were incorporated into our study underwent assessment based on two quality measurement tools: DISCERN and GQS. The survey comprised five questions, with a scoring system where "no" responses were assigned 0 points, and "yes" responses were awarded 1 point. We calculated a reliability score by summing up these points. To ensure the reliability of our evaluations, all videos were also subjected to review by three examiners (HA, VN, AS), and experts in maxillofacial surgery. STATISTICAL ANALYSIS Statistical Package for the Social Sciences (SPSS) software program (version 21) was used for statistical evaluations. The normality of the data distribution was evaluated using the Shapiro-Wilk test. Pearson correlation coefficients were calculated to evaluate possible correlations between three reviewers of GQS and DISCERN. Statistical significance was evaluated at the P- 0.05 level. RESULTS Descriptive statistics for the evaluated videos are shown in Table 1 . The mean total number of video views was 137419.16. The mean number of comments was 33.25, and the mean number of likes was 2682.95. The mean duration of the video was 152.16 sec. The mean interaction index was 1.51, and the mean viewing rate was 2803.28. The mean GQS score was 2.25 and the mean DISCERN score was 1.94. Out of 50 videos, the single needle technique was used in 10, and in 40 videos 2 needle technique was used. In 37 videos normal saline was used, 7 videos used Ringer lactate whereas in 3 videos not specify which material was used. After irrigating the joints various medicaments were used like steroids in 10 videos, hyaluronic acid in 5, and IPRF in one (Table 2 ). There was strong positive correlation between Examiner 1 and Examiner 2 (r = 0.732), Strong positive correlation between Examiner 2 and Examiner 3 (r = 0.704), Moderately strong positive correlation between Examiner 1 and 3 (r = 0.489). (Table 3 & 4 ). There was no correlation between the GQS and DISCERN score of any examiner with the corresponding Interaction index and View rate (Table 5 ). TMJ arthrocentesis, a leading minimally invasive intervention for patients unresponsive to conservative management, entails irrigating the joint. The objective of this procedure is to enhance functional outcomes by dismantling adhesions and eliminating inflammatory mediators. YouTube, the foremost video-sharing platform, serves as a hub for diverse content created by individuals on various subjects. It serves as a convenient resource for acquiring both theoretical and practical knowledge, offering a swift means to review or supplement their understanding of the procedure. Although there have been previous studies assessing health-related videos on YouTube, no specific research focusing on the quality and reliability of videos related to TMJ arthrocentesis was found. Consequently, this is the first study undertaken to evaluate the educational value of YouTube videos pertaining to TMJ arthrocentesis. The study underscored the pros and cons of YouTube as a free and easily accessible platform for patients and medical students seeking information on TMJ arthrocentesis. From an initial pool of 500 YouTube videos, a focused analysis was conducted on 50 videos deemed most relevant to TMJ arthrocentesis. The average total video views among these selected videos stood at 137,419.16, with an average of 33.25 comments and 2,682.95 likes. The videos had an average duration of 152.16 seconds. The mean interaction index and viewing rate were 1.51 and 2803.28, respectively, while the average GQS score was 2.25 and the DISCERN score for these videos was 1.94. Even though we discovered a few videos with commendable educational content, most YouTube videos focusing on TMJ Arthrocentesis were found to be of inadequate quality and unsuitable for educational purposes. In the videos considered unhelpful, a common shortcoming was the absence of information on risks, contraindications, and synovial fluid analysis. Additionally, among the numerous excluded videos, some were patient-recorded or lacked any supplementary explanation. Despite the poor quality of YouTube videos focused on TMJ Arthrocentesis for educational purposes in the present study, patients considering or undergoing the procedure can still find comfort and guidance in these videos. Through witnessing actual demonstrations and hearing personal narratives from individuals who have undergone the procedure, patients can gain valuable insights into what to expect, potential outcomes, and postoperative care. Many authors in the Maxillofacial surgical domain have previously evaluated the Calibre of online information about surgical procedures such as orthognathic surgery, temporomandibular joint disorders, dental implants, and head and neck cancer [ 9 – 11 ]. The common conclusion was that these sources of information were generally of poor quality. An evaluation of 97 top YouTube videos on oral implantology revealed a prevalent commercial bias and limited educational value, even though they often showcase high-quality footage[ 12 ]. Contemporary medical students favor digital learning over traditional lecture-based classes, citing its user-friendly interface, accessibility, superior medical image quality, and the opportunity for repeated practice as key advantages [ 13 , 14 ]. A randomized study on ultrasound-guided vascular access training also noted that web-based courses were determined to be just as effective as conventional methods in teaching procedural skills[ 15 ]. In contrast, supervised training on cadavers has been identified as critically important for enhancing the confidence of medical students in procedural skills related to Arthrocentesis [ 16 ]. An evaluation conducted to appraise the educational efficacy of YouTube videos about knee Arthrocentesis similarly indicates a low-average score for overall educational value [ 17 ]. The findings of the present study are consistent with these comparable results. The primary constraint of this study stemmed from the data collection process, which occurred within a specific and limited time frame. Additionally, the dynamic nature of digital data posed a challenge, as it was observed to undergo rapid changes. Despite these acknowledged limitations, we contend that the findings reasonably reflect the quality of information available on YouTube. We have identified a discrete number of YouTube videos on TMJ Arthrocentesis that are suitable for application in digital learning for medical students, fellows, and residents. Regrettably, even when posted by health professionals, we have considered YouTube content to be of insufficient and inconsistent educational quality. CONCLUSION Despite a few commendable educational videos, most YouTube content on TMJ Arthrocentesis is of inadequate quality. It is essential not to substitute formal education or hands-on training with YouTube, especially for medical professionals. Consult experts and adhere to best practices during medical procedures. In conclusion, caution is needed when relying on YouTube for TMJ Arthrocentesis education; formal training remains irreplaceable. Data on the video source, duration, upload date, time elapsed since upload, total views, likes, dislikes, comments, interaction index, and viewing rate were collected and analyzed. Video quality was assessed using the Global Quality Scale (GQS) and the Quality Criteria for Consumer Health Information (DISCERN).\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eRESULTS\u003c/strong\u003e\u003c/p\u003e\n\u003cp\u003eOut of the 50 videos, 10 used the single-needle technique, while the remaining 40 used the 2-needle technique. Following joint irrigation, medications such as steroids were used in 10 videos, hyaluronic acid in 5, and IPRF in one. The mean total number of video views was 137,419.16, with a mean interaction index of 1.51 and a mean viewing rate of 2,803.28.
