This study provides a theoretical basis for developing new teaching models. Method and result: A total of 30 general medicine resident supervisors from training bases were randomly divided into an experimental group and a control group, with 15 participants in each group. The experimental group was trained using the "Standardized Patient + Feedback-Based Real-Life Teaching + AI-Assisted System Teaching" model, while the control group followed traditional teaching methods. After six months of training and another six months of supervision, the supervisors’ abilities in standardized patient role-playing teaching, Mini-CEX (Mini-Clinical Evaluation Exercise) teaching, and feedback-based teaching were assessed. A self-designed satisfaction survey was used to evaluate the satisfaction of skill mentors, core members of the training base, and general medicine residents with the supervisors. The experimental group showed significantly better performance than the control group in standardized patient role-playing teaching ability (T=3.760, P<0.01), Mini-CEX assessment ability (T=3.299, P<0.02), and feedback-based teaching ability (T=3.866, P<0.01). The satisfaction levels of skill mentors (T=5.035, P<0.01), training base members (T=3.656, P<0.05), and general medicine residents (T=4.170, P<0.01) in the experimental group were also significantly higher than those in the control group. Conclusion: The "Standardized Patient + Feedback-Based Real-Life Teaching" combined with "AI-Assisted System Teaching" model effectively improves supervisors' multi-dimensional teaching abilities and general medicine residents' Mini-CEX evaluation skills, while also significantly enhancing teaching satisfaction. Standardized Patient Feedback-Based Teaching Mini-CEX General Medicine Resident Supervisors AI-Assisted System Figures Figure 1 Figure 2 Figure 3 Introduction Standardized Patient (SP) Teaching + Feedback-Based Teaching Model emphasizes a comprehensive approach, including SP role-playing, SP assessment, the construction of simulated clinical teaching environments for SPs, and timely feedback throughout the process. A standardized patient refers to a normal person or a mildly ill patient who can accurately simulate patient symptoms, medical history, and physical examination results [1]. Mini-CEX (Mini-Clinical Evaluation Exercise) is an effective tool for assessing general medicine residents' clinical practice abilities through simulated real-life clinical scenarios [2]. The evaluation focuses on clinical reception abilities (especially history taking and physical examination), doctor-patient communication empathy, and the ability to summarize and provide feedback. The feedback-based teaching model emphasizes process evaluation and personalized teaching, highlighting the subjectivity and participation of general medicine residents, with the goal of establishing a comprehensive, open feedback system [3]. From 2021 to 2023, the "Guidelines for Standardized Residency Training Activities" [4] have increasingly emphasized the practical effectiveness of training, multi-source feedback, timely feedback, process-oriented feedback evaluation, and resident feedback, aligning well with the new requirements of the guidelines. This study employs a standardized patient + feedback-based teaching method, using a human-machine collaborative model that integrates the advantages of both real-life human teaching and AI-assisted systems. The real-life SP model excels in cultivating advanced communication, empathy, humanistic care, and complex scenario handling, while the AI-assisted system is more advantageous for basic skills training, standardized assessments, reproducibility, and data feedback. By combining the objectivity and efficiency of AI with the depth and flexibility of human teaching, a more robust medical education system can be constructed. General medicine residents, as "gatekeepers" of human health, bear the significant responsibility of providing comprehensive and continuous health services to the community [5]. As educators and mentors for general medicine residents, the role of the supervising teachers is crucial. The AI-assisted system is a newly developed two-stage system by our team, focusing on healthcare communication education. The system consists of two modules: the patient simulation module and the communication scoring and feedback module. The entire system is built around a large language model, which achieves task-specific optimization by fine-tuning medical communication corpora. Users can engage in human-machine dialogue via a front-end training platform, and the system dynamically generates patient language based on the input context. After each interaction, the system provides a communication score and specific improvement suggestions[6]. This study aims to implement the standardized patient + feedback-based teaching model for general medicine resident supervisors at Xiangya Second Hospital of Central South University and explore the combined application of real-life teaching and AI-assisted system teaching in the training of general medicine resident supervisors. 1. Materials and Methods 1.1 Data Sources This study selected 30 newly appointed general medicine resident supervisors from the General Medicine Residency Training Base at Xiangya Second Hospital of Central South University, from February 2023 to January 2024, as research participants. All participants passed the teacher selection and qualification assessment. Among the 30 participants, 9 were male (30.0%) and 21 were female (70.0%). There was 1 chief physician (3.3%), 19 associate chief physicians (63.3%), and 10 attending physicians (33.3%). All participants held doctoral degrees, and 9 (30.0%) were master’s thesis advisors. The participants' ages ranged from 34 to 55 years, with a mean age of 42.93 ± 6.09 years. Their work experience ranged from 9 to 31 years, with a mean of 17.67 ± 7.17 years. Professional Background: The participants must hold a medical doctorate degree and have at least 9 years of clinical experience, along with a hospital-level general practice teaching certification. Communication Skills: The participants should possess good doctor-patient communication skills and teamwork abilities. Continuous Learning Ability: The participants should have high research levels and the ability to continuously learn, keeping up-to-date with the latest developments in general medicine. (2) Exclusion Criteria Participants who have previously been involved in teaching accidents. Participants lacking general medicine practice experience or those with improper learning attitudes. Participants who cannot effectively communicate or collaborate within a team. This study was approved by the Research Ethics Committee of Xiangya Second Hospital, Central South University [(2020) Ethics Review No. (Research 226)] and all participants provided informed consent.The human data research in this study complies with the provisions of the Helsinki Declaration. 1.2.2 Research Group Teaching: Integrated Four-Stage Model The research group adopted the "Standardized Patient + Feedback-Based Real-Life Teaching" combined with "AI-Assisted System Teaching" model, with the following implementation details: (1) Training Plan Development Based on the theoretical guidelines from the "Guiding Opinions on the Establishment of the Standardized Residency Training System" (National Health Science Education [2013] No. 56) [7], "Regulations on Standardized Residency Training (Trial)" (National Health Science Education [2014] No. 49) [8], and the "Standardized Residency Training Content and Standards (Trial)—General Practice Training Details" [9], a detailed training plan and teaching plan were developed. (2) Training Implementation The research group supervisors received 3 hours of concentrated training every Friday afternoon, which included 60 minutes of theoretical learning, 90 minutes of standardized patient and feedback-based teaching practice courses, and 30 minutes of feedback and summary. The class followed a closed-loop teaching method: theoretical learning → real-life demonstration → AI-based practice → feedback refinement. The two teaching models supported each other, complementing each other's strengths. 1) First Stage: a. Standardized Patient Theory Learning and Real-Life Demonstration (Concentrated Classroom Training) ① Theory Learning: Learning courses on disease knowledge, simulation skills, teaching techniques, communication skills, etc. [10], with MOOCs, micro-videos, and animated videos published on the CCMTV Cloud Platform. ② Real-Life SP Demonstration and Scenario Construction: Design typical and representative cases, such as complex doctor-patient communication, non-verbal cues (expressions, tone of voice, etc.), humanistic care, and advanced skills in dealing with emergencies. A script was written, and the scenario was set up [11]. ③ Role-Playing: Skill mentors were responsible for role-playing and evaluation teaching. The classroom employed real-life scenario simulations and case introductions to increase the class’s engagement [12]. ④ Observation and Evaluation: Using Mini-CEX, general practice resident physicians were evaluated on six abilities: history-taking, physical examination, clinical diagnosis, treatment planning, communication skills, and humanistic care [13]. ⑤ In-depth Feedback Summary: Skill mentors provided timely, in-depth, and warm qualitative feedback on-site, encouraging critical thinking, and teachers engaged in mutual discussions and self-reflection. b. Feedback-Based Teaching Theory Learning and Real-Life Demonstration (Concentrated Classroom Training) ① Clarifying Learning Objectives: The supervisors were tasked with improving six key feedback-based teaching abilities. ② Organizing Diversified Teaching Activities: Activities included small group discussions, case analysis, role-playing, flipped classrooms, special lectures, teaching rounds, outpatient supervision, annual assessments, graduation assessments [14], skill competitions, grassroots free clinics, and community faculty exchanges to improve the ability to solve on-site problems. After watching real-life simulations, supervisors could rehearse and review similar cases in the AI system. ③ Timely Feedback: Supervisors and general practice residents followed a "one-to-two" management system, with continuous tracking and management focusing on teaching progress, teaching techniques, and teaching satisfaction [15]. ④ Encouraging Self-Evaluation and Peer Evaluation: Supervisors adjusted teaching strategies in real-time to improve critical thinking and cooperation. ⑤ Real-Time Teaching Method Improvement: Regular exchanges on research topics, after-class Q&A, online interaction, and observation of teaching practices helped to identify problems and refine teaching methods. 2) Second Stage: Personalized AI Practice and Data-Driven Feedback (Post-Class Exercises) ① Seamless Integration: The case scenarios and teaching objectives from the classroom learning were synchronized to the AI-assisted system. ② Self-Strengthening Training: Supervisors could log into the AI-assisted system after class to engage in repetitive dialogue practice with a "virtual patient" using the same case. The AI system provided a consistent case background to ensure the alignment of training objectives. ③ Instant Data-Driven Scoring: After each human-machine dialogue, the system immediately generated a quantitative evaluation report, accurately identifying shortcomings in history-taking completeness, inquiry logic, and medical knowledge accuracy, and providing suggestions and improvement measures. This addressed the challenge of quantifying and replicating real-life feedback.[16]The specific operational technical roadmap of the AI-assisted system is shown in Figure 1(fig.1). 3) Third Stage: Data-Driven Real-Life Feedback and Refinement ① Feedback Upgrade: Skill mentors reviewed the practice data and records from the AI-assisted system during subsequent training sessions. ② Targeted Guidance: Based on the common issues and individual weaknesses identified by AI, skill mentors provided more targeted in-depth coaching. For example, real-life role-specific training was conducted to address areas where AI indicated low scores in "communication skills." ③ Spiral Ability Enhancement: A "real-life creation of advanced scenarios → AI solidification of basic skills and data provision → real-life enhancement based on data" spiral ability enhancement loop was formed. 4) Fourth Stage: Assessment and Evaluation A comprehensive evaluation system combining "AI-driven formative assessments + real-life teaching summative assessments" was established. AI data served as a key basis for process evaluation, while real-life assessments focused on evaluating comprehensive abilities and humanistic qualities. 1.2.3 Control Group Teaching The control group adopted the traditional lecture-based teaching model (LBL), centered on the supervisor. The supervisor provided collective theoretical lectures, teaching rounds, and demonstrations of basic clinical operations to general practice residents during their rotation. After learning the theoretical knowledge and practicing the operations, the residents were then uniformly assessed. 1.2.4 Evaluation Content and Standards (1) Supervisor Standardized Patient Role-Playing Teaching Ability Evaluation Skill mentors scored the supervisors based on their role-playing, interaction skills, and the quality of feedback they provided. The evaluation was conducted through observation, with a total score of 100 points, divided into five areas, each worth 20 points. See Table 2 . Each ability evaluation had a total score of 100 points: Teaching Design and Organization Ability Evaluation: Evaluation of goal-setting, content planning, teaching methods, and organization of the teaching process. Observation and Evaluation Ability Evaluation: Evaluation of the ability to observe student performance, student participation, learning progress, and feedback collection. Feedback and Guidance Ability Evaluation: Evaluation of timely feedback, clear learning direction, effective guidance, and personalized teaching. Communication and Interaction Ability Evaluation: Evaluation of pre-class preparation, classroom interaction, post-class communication, and teacher-student cooperation. Critical Thinking and Analysis Ability Evaluation: Evaluation of problem identification, analysis, problem-solving, and independent thinking abilities. Teaching Innovation and Self-Learning Ability Evaluation: Evaluation of new teaching methods, self-learning and continuous learning, innovation, and research capabilities. See Table 3 . Skill Mentors: 150 survey questionnaires were distributed and 150 were returned, with a 100% effective recovery rate. The results were averaged. Residency Base Members: 120 questionnaires were distributed and 120 were returned, with a 100% effective recovery rate. The results were averaged. General Practice Residents: The class leaders distributed 180 questionnaires, which were all returned with a 100% effective recovery rate. The average score of the six residents each supervisor had mentored was used for evaluation. See Tables 4 – 6 . Please refer to Appendix 1 for all supplementary supplementary file. Each group contained 30 residents. The research group used standardized patient teaching with Mini-CEX as the evaluation tool, while the control group used traditional teaching evaluation methods. After six months of training, the core members of the general practice residency base assessed the general practice residents’ Mini-CEX evaluation abilities. Seven evaluation categories, each with a score of 100 points, were used. See Table 7 . Categorical data were expressed as relative numbers. Normally distributed continuous data were expressed as mean ± standard deviation (± s). A t-test was used to compare continuous variables, and a χ² test was used to compare categorical data. A P-value of < 0.05 was considered statistically significant. 2. Results 2.1 Evaluation Results for Supervisors 2.1.1 Standardized Patient Role-Playing Evaluation The comparison between the two groups showed that the research group scored significantly better than the control group, especially in accurately role-playing patient clinical presentations, psychological states, accurate responses, and constructive feedback (P < 0.05). See Table 2 . 2.1.2 Feedback-Based Teaching Abilities Evaluation The research group scored significantly higher than the control group, particularly in teaching design and organization, feedback and guidance, communication and interaction, critical thinking and analysis, and teaching innovation and self-learning (P < 0.05). See Table 3 . 2.1.3 Satisfaction Survey Results for Supervisors 2.1.3.1 Skill Mentors' Satisfaction Scores The T-test results indicated that the research group had significantly higher satisfaction scores than the control group, particularly in areas like teaching model application, Mini-CEX evaluation application, standardized patient role-playing, feedback-based teaching abilities, personalized teaching, teaching content innovation, timely feedback and strategy adjustments, theoretical and practical teaching, and teaching progress and quality (P < 0.05). See Table 4 . 2.1.3.2 Residency Base Members' Satisfaction Scores The T-test results indicated that the research group had significantly higher satisfaction scores than the control group, particularly in the same areas as mentioned above (P < 0.05). See Table 5 . 2.1.3.3 General Practice Residents' Satisfaction Scores The T-test results indicated that the research group had significantly higher satisfaction scores than the control group, particularly in teaching model application, knowledge mastery and ability improvement, personalized teaching, receiving feedback and timely improvements, role-playing and feedback evaluation, the six teaching abilities, history-taking and physical examination, and fairness and impartiality (P < 0.05). See Table 6 . 2.1.4 Mini-CEX Evaluation Results for General Practice Residents The research group scored significantly higher than the control group in history-taking, physical examination, clinical diagnosis, communication skills, and feedback and summary evaluation (P < 0.05). See Table 7 . 3. Discussion 3.1 Current Status Domestically and Internationally As patients become more aware of privacy protection, the use of real patients in medical education has been restricted. The concept of standardized patients was first proposed by Barrows, the director of the American Academy of Neurology, in 1963. Following this, medical schools in the U.S., U.K., Australia, and other countries began adopting the standardized patient model. Feedback-based teaching started earlier abroad [ 17 ], with the renowned American educator Benjamin Bloom advocating for frequent and timely feedback alongside solid academic content in the 1950s. Currently, the combination of standardized patient + feedback-based teaching has developed into a well-established theoretical and practical model in foreign countries, whereas domestic adoption is still in the early stages with a lower prevalence. Despite this, the use of these teaching methods has been gradually increasing in recent years with some notable educational outcomes. The integration of real-life teaching and AI-assisted teaching systems has developed rapidly abroad, while in China, it started later but has been advancing quickly, mainly driven by large language models, with significant progress in both development speed and practical application. This study provides a theoretical basis for developing new teaching models.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eMethod and result: \u003c/strong\u003eA total of 30 general medicine resident supervisors from training bases were randomly divided into an experimental group and a control group, with 15 participants in each group. The experimental group was trained using the \"Standardized Patient + Feedback-Based Real-Life Teaching + AI-Assisted System Teaching\" model, while the control group followed traditional teaching methods. After six months of training and another six months of supervision, the supervisors’ abilities in standardized patient role-playing teaching, Mini-CEX (Mini-Clinical Evaluation Exercise) teaching, and feedback-based teaching were assessed. A self-designed satisfaction survey was used to evaluate the satisfaction of skill mentors, core members of the training base, and general medicine residents with the supervisors.\u003c/p\u003e\n\u003cp\u003eThe experimental group showed significantly better performance than the control group in standardized patient role-playing teaching ability (T=3.760, P\u0026lt;0.01), Mini-CEX assessment ability (T=3.299, P\u0026lt;0.02), and feedback-based teaching ability (T=3.866, P\u0026lt;0.01).
