Design Data were collected from two waves of cross-sectional health interviews and examination surveys administered 10 years apart among individuals aged ≥ 35 years in rural China. Fasting blood sugar levels were recorded for each participant. Results From 2013 to 2023, the prevalence and treatment rate of diabetes increased significantly from 6.1% and 75.3% to 8.8% and 96.5%, respectively, among Han majority participants from 3.3% and 67.2% to 12.5% and 94.1%, respectively, among Na Xi ethnic minority participants (P < 0.01), with the greatest relative increase occurring among Na Xi ethnic minority participants. Although the overall awareness and control rates of diabetes did not differ between 2013 and 2023 for both nationalities (P > 0.05), a significant increasing trend in the control rate was observed among diabetic patients with poor access to medical services for both nationalities (P < 0.01). The overall rate of compliance with antidiabetic drugs or insulin injection during the 12 months prior to the survey increased significantly in both the Han majority (71.9% vs. 97.9%) and the Na Xi ethnic minority (67.2% vs. 95.4%, P < 0.01), whereas the increasing rate of having taken measures to control diabetes during the two weeks prior to the survey and regular blood glucose self-monitoring in the prior 12 months was found only among Na Xi ethnic minority participants (P < 0.01). Conclusions The prevalence and treatment of diabetes have increased substantially, and significant changes in diabetes self-management behaviors have occurred in rural southwestern China over the ten years studied, with striking ethnic differences in temporal trends for the observed rates. Our findings highlight the importance of addressing ethnicity in future interventions to further control diabetes and improve self-management behaviors among diabetic patients. Diabetes self-management behaviors temporal trend ethnicity China Introduction Diabetes is a common noncommunicable disease (NCD) with multiple complications that contribute to the large global health care burden [ 1 ] . The prevalence of diabetes worldwide has been increasing over the past few decades, from 211.2 million to 476.0 million between 1990 and 2017, accounting for an overall 129.7% increase [ 2 ] . In 2021, China had the greatest absolute diabetes burden worldwide and a relatively high prevalence. In 2001, close to 141 million patients aged 20–79 years accounted for 26% of diabetes cases worldwide [ 3 ] . From 1990–2019, the number of diabetes patients increased by 160%, from 35.14 million to 91.70 million, in China, and the age-standardized prevalence rate of diabetes increased from 4788 per 100,000 to 8170 per 100,000, representing a 70.6% increase [ 4 ] . To reduce and prevent complications of diabetes, patients afflicted with the condition need to exercise good self-management behaviors [ 5 ] . Self-management refers to the daily activities or actions that an individual must undertake to control or reduce the impact of a disease on their health and well-being to prevent further deterioration [ 6 ] . Successful diabetes self-management (DSM) requires patients to monitor their blood glucose levels frequently and take the necessary steps to keep their blood glucose within physiological levels by adhering to prescribed diets, exercising, taking any necessary medications, and maintaining daily foot care routines [ 7 ] . Previous studies have indicated that the prevalence, treatment and control of diabetes are on the rise globally, including in China, with no significant change in awareness [ 8 , 9 ] . The self-management of diabetes among diabetic patients is not ideal; studies report that only 9.2%-16.7% of Chinese patients with diabetes have adequate self-management behaviors [ 10 ] , while studies on trends in self-management behaviors for diabetes over time are lacking. A better understanding of the trends and influencing factors of diabetes prevalence and self-management behaviors is conducive to better designing or implementing diabetes prevention and treatment strategies. Adherence to DSM is complex and challenging, and many factors, such as demographics, biology, psychology, society, culture and the environment, can have an impact [ 11 ] . Younger age, higher education, level higher economic status, more social support and better health care accessibility were associated with better self-management behaviors [ 12 , 13 ] . Moreover, the prevalence and self-management of diabetes are influenced by race and ethnicity [ 14 – 16 ] . China is a multiethnic country with a large population; in addition to the Han majority, 55 ethnic minorities exist. However, studies examining how diabetes self-management behaviors change over time and the relationship between ethnicity and temporal trends in diabetes self-management behaviors are scarce in China. Yunnan Province, which is located in southwestern China, is the province with the greatest number of ethnic minorities. There are 25 hereditary ethnic minorities, 15 of which are unique to Yunnan Province. Different ethnic groups have different dietary patterns, lifestyles, socioeconomic levels and geographic environments, leading to different manifestations of diabetes [ 17 , 18 ] . However, research on the relationships between ethnicity and changes in diabetes prevalence and self-management has been limited. Therefore, this study aimed to investigate ethnic differences in the trends of diabetes incidence and self-management behaviors between the Han majority and the Na Xi ethnic minority in rural Yunnan Province, China, from 2013 to 2023. Methods Study area and population The primary data in this study were collected from two community-based cross-sectional health interviews and examination surveys conducted in 2013 and 2023 in Yulong Na Xi Autonomous County, Yunnan Province. Yulong Na Xi Autonomous County has a resident Na Xi population of 126,600, accounting for 55.23% of the total population, making it the only Na Xi Autonomous County in China. In 2013, a multistage stratified random sampling method was used to select study subjects. First, all townships in Yulong Na Xi Autonomous County were divided into two levels (high and low) according to the per capita gross domestic product (GDP). One township was randomly selected from each of the two levels, for a total of two townships. Second, the probability proportional to size method was used to randomly select three Han-dominated and three Na Xi-dominated sample villages from each of the two townships. Finally, a simple random sampling method was used to select permanent residents aged ≥ 35 years from villages as study objects. In 2023, a consistent three-stage stratified random sampling selection process was used to select participants from the two selected townships. Data collection and measurement In both 2013 and 2023, each participant who signed an informed consent form was interviewed face-to-face by a uniformly trained investigator using a similar pretested structured questionnaire. General demographic characteristics (sex, age, ethnicity, educational level, annual household income, and access to medical services) and the diagnoses, treatments, and self-management behaviors of patients with diabetes were collected. For each participant, fasting blood glucose (FBG) measurements were taken in the morning by a physician at the First Affiliated Hospital of Kunming Medical University after participants had fasted for a minimum of 8 hours. The detailed methodology has been previously reported [ 19 ] . Ethical approval This study was approved by the Ethics Committee of Kunming Medical University prior to initiating the study (approval number: KMMU2023MEC108). Written informed consent was signed by all participants. Definitions In accordance with the criteria of the 2020 WHO consultation group report on the diagnosis of diabetes mellitus [20] , diabetes was defined as a FBG ≥7.0 mmol/L (126 mg/dl), the use of hypoglycemic agents in the past 2 weeks, and/or a reported previous diagnosis of diabetes by a health professional. Among participants with diabetes, awareness of diabetes was defined as a participant-reported prior diagnosis of diabetes by a physician. Treatment of diabetes was defined as participant-reported use of oral anti-diabetic medications and/or injected insulin. Control of diabetes was defined as a participant who presented with diabetes but with a fasting blood glucose (FBG) concentration <7.0 mmol/L. Taking measures to control diabetes was defined as having taken at least one of the following actions during the two weeks prior to the survey: diet control, weight loss, increased physical activity, and smoking cessation [15, 21] . Using the median income as the cutoff point, participants were categorized by income into two annual household income groups, low or high. Access to medical services was classified into two groups: good access when the walking distance from home to the nearest village medical center was < 30 minutes and poor access when the walking distance from home to the nearest village medical center was ≥30 minutes [22] . Illiterate was defined as the inability to read or write a short simple statement on one's everyday life with understanding [23] . Statistical analysis SPSS 22.0 software was used for data analysis. Data were analyzed with descriptive analysis techniques, chi-square tests, and t tests. Categorical variables are presented as counts and percentages, whereas continuous variables are presented as the mean ± standard deviation (SD). Differences in categorical variables between survey years were compared using chi-square tests for independence, while t tests were employed to analyze differences in continuous variables. Two-tailed P values were used to determine statistical significance, with p<0.05 considered to indicate statistical significance. Results The total number of individuals aged ≥35 years invited to participate in the two survey waves was 5,500 in 2013 and 5,200 in 2023. Of these, 5,247 in 2013 and 5,003 in 2023 consented to participate, for an overall response rate of 95.4% and 96.2%, respectively. Table 1 presents the demographic characteristics and mean FBG values of the Han majority and the Na Xi ethnic minority participants by survey year. From 2013 to 2023, statistically significant decreasing trends in the proportion of adult illiteracy rates and of those with poor access to medical services were observed among Na Xi ethnic minority participants, whereas the opposite trend in the proportion of those with poor access to medical services was found among the Han majority (P<0.01). Furthermore, statistically significant increasing trends in mean FBG were also observed among both Han majority and Na Xi ethnic minority participants (P<0.01). The proportion of participants with higher levels of annual household income was greater for the Na Xi ethnic minority group than for the Han majority group for both survey years (P<0.01). However, compared with males, Han majority and Na Xi ethnic minority females consistently had higher adult illiteracy rates in both survey years (P<0.01). Table 2 shows the prevalence, awareness, treatment and control of diabetes among the two nationalities by survey year. From 2013 to 2023, the prevalence of diabetes increased significantly among both the Han majority and the Na Xi ethnic minority (P<0.01), with the greatest increase occurring among the Na Xi ethnic minority participants. This increasing rate was also observed among the subgroups categorized by sex, education level, income level, and access to medical services in both the Han majority and the Na Xi ethnic minority (P<0.05). Moreover, the treatment rate of diabetes among patients also increased in both Han majority and Na Xi ethnic minority individuals (P<0.01), with greater relative increases occurring among Na Xi ethnic minority participants, particularly among those with lower levels of education and poor access to medical services. Although the overall control rate of diabetes did not differ between 2013 and 2023 for either the Han majority or the Na Xi ethnic minority (P>0.05), a significant increasing trend was observed among diabetic patients with poor access to medical services for both nationalities (P<0.01). The Han majority had a higher prevalence of diabetes than the Na Xi ethnic minority participants did in 2013, whereas the Na Xi ethnic minority had a higher prevalence of diabetes than the Han majority did in 2023 (P<0.05). Table 3 indicates the rates of self-management behaviors among diabetic patients in the Han majority and the Na Xi ethnic minority by survey year. From 2013 to 2023, the overall rate of compliance with antidiabetic drugs or insulin injection in the 12 months prior to the survey increased significantly for both the Han majority and the Na Xi ethnic minority (P<0.01), and an increasing rate was also observed among the subgroups categorized by sex, education level, income level, and access to medical services, with the greatest relative increases occurring among diabetic patients with poor access to medical services for both the Han majority and the Na Xi ethnic minority (P<0.01). However, measures to control diabetes in the two weeks prior to the survey and regular blood glucose self-monitoring during the prior 12 months increased among only Na Xi ethnic minority participants (P<0.01). Compared with Han majority participants, Na Xi ethnic minority participants had higher rates of regular blood glucose self-monitoring during the prior 12 months and of having taken measures to control diabetes during the two weeks prior to the survey in 2023 (P<0.01). Discussion The findings in this study indicate that both the prevalence of diabetes and self-management behaviors among diabetic patients shifted over time among Han majority and Na Xi ethnic minority adults in rural southwestern China from 2013 to 2023, with significant ethnic differences in temporal trends for the observed rates. Our study revealed that the overall prevalence of diabetes among both the Han majority and the Na Xi ethnic minority in rural southwestern China in 2023 was higher than that in other rural areas of China [24, 25] , as well as in other Asian populations, including Vietnamese (7.3%) [26] and Bangladesh (8.5%) [27] , indicating a relatively high prevalence of diabetes in the study area. Furthermore, the data revealed that the prevalence of diabetes increased significantly among both the Han majority and the Na Xi ethnic minority over the past decade, with the greatest increase occurring among the Na Xi ethnic minority participants. As a result, the prevalence of diabetes among the Na Xi ethnic minority exceeded that among the Han majority in 2023. Thus, diabetes was more prevalent among participants of the Na Xi ethnicity. These findings indicate that diabetes presents a significant public health challenge in Chinese ethnic minority communities, and future diabetes prevention and control measures should focus more on Na Xi ethnic minority populations. The treatment of diabetes has increased substantially among the two ethnic groups over a 10-year period, indicating that improving access to health care has progressed in Chinese ethnic minority communities. Moreover, the Na Xi ethnic group experienced greater relative increases in the treatment of diabetes, particularly among those with lower levels of education and poor access to medical services. This may be attributed to the significant improvement in educational level and access to medical services for Na Xi ethnic minorities over the study period. Increased knowledge enables an individual to increase his or her ability to utilize health services. The present study revealed that there was no significant change in the awareness and control of diabetes among Han majority and Na Xi ethnic minority individuals, which is consistent with the findings of previous studies [28, 29] . This may be the result of a lack of health-related knowledge in ethnic minority areas. These findings underscore the urgent need to strengthen diabetes health education programs and improve diabetes management strategies in Chinese minority communities. Our study also revealed ethnic differences in the temporal trends of various self-management behaviors among diabetic patients in this study area. Over the ten-year study period, the compliance rate with antidiabetic drugs and insulin injections increased markedly for both the Han majority and the Na Xi ethnic minority. However, increasing rates of taking measures to control diabetes and regularly self-monitoring blood glucose were found only among the Na Xi ethnic minority population. This may be because the implementation of a national basic public health service program in Chinese ethnic minority communities has successfully improved self-management behaviors among Na Xi ethnic minority diabetic patients. In our study, access to medical services had an apparent effect on temporal trends in control and self-management behaviors among diabetic patients for both the Han majority and the Na Xi ethnic minority. Although the overall control rate of diabetes did not differ between 2013 and 2023 for either the Han majority or the Na Xi ethnic minority, a significant increasing trend was observed among diabetic patients with poor access to medical services. Furthermore, the rate of compliance with antidiabetic drugs or insulin injections rose significantly across the entire socioeconomic status spectrum over the ten-year study period for both nationalities, with the greatest relative increase occurring among participants with poor access to medical services. These findings indicate that diabetes control and management measures have progressed in poor medical service populations. This study has several limitations. First, the true prevalence of diabetes may have been underestimated, as only FBG was used to diagnose diabetes. Second, various diabetes self-management behaviors were based on patient recall and may therefore be subject to bias. Third, the comparative analysis was limited to Han majority and Na Xi ethnic minority populations, which may limit the generalizability of the results to other ethnic groups. Conclusion In conclusion, the prevalence and treatment of diabetes have increased substantially, and significant changes in diabetes self-management behaviors have occurred in rural southwestern China over the ten years studied, with striking ethnic differences in temporal trends for the observed rates. Our findings underscore a critical need for the government to continue to increase diabetes awareness and implement interventions to promote its prevention and control. This study highlights the importance of addressing ethnic factors in future interventions to further control diabetes and improve self-management behaviors among diabetic patients. Fasting blood sugar levels were recorded for each participant.\u003c/p\u003e\u003ch2\u003eResults\u003c/h2\u003e \u003cp\u003eFrom 2013 to 2023, the prevalence and treatment rate of diabetes increased significantly from 6.1% and 75.3% to 8.8% and 96.5%, respectively, among Han majority participants from 3.3% and 67.2% to 12.5% and 94.1%, respectively, among Na Xi ethnic minority participants (P\u0026thinsp;\u0026lt;\u0026thinsp;0.01), with the greatest relative increase occurring among Na Xi ethnic minority participants. Although the overall awareness and control rates of diabetes did not differ between 2013 and 2023 for both nationalities (P\u0026thinsp;\u0026gt;\u0026thinsp;0.05), a significant increasing trend in the control rate was observed among diabetic patients with poor access to medical services for both nationalities (P\u0026thinsp;\u0026lt;\u0026thinsp;0.01). The overall rate of compliance with antidiabetic drugs or insulin injection during the 12 months prior to the survey increased significantly in both the Han majority (71.9% vs. 97.9%) and the Na Xi ethnic minority (67.2% vs. 95.4%, P\u0026thinsp;\u0026lt;\u0026thinsp;0.01), whereas the increasing rate of having taken measures to control diabetes during the two weeks prior to the survey and regular blood glucose self-monitoring in the prior 12 months was found only among Na Xi ethnic minority participants (P\u0026thinsp;\u0026lt;\u0026thinsp;0.01).\u003c/p\u003e\u003ch2\u003eConclusions\u003c/h2\u003e \u003cp\u003eThe prevalence and treatment of diabetes have increased substantially, and significant changes in diabetes self-management behaviors have occurred in rural southwestern China over the ten years studied, with striking ethnic differences in temporal trends for the observed rates.
