Abstract
Abnormal bleeding is common in women of childbearing . For some insertion of a levonorgestrel-releasing IUD – easily done in a family practice setting – may be all the treatment they need. In response to a case study of a 40-year-old multiparous woman seeking treatment for heavy menstrual bleeding and cramping this article discusses various courses of treatment for women experience heavy menstrual bleeding (HMB) or menorrhagia. In fact HMB and dysmenorrhea are among the most common reasons for office visits and missed work among women in this age group. In addition to have a negative impact on quality of life HMB can causes severe anemia. The 52 mg levonorgestrol-releasing IUD has been shown to be effective in treating multiple conditions associated with menorrhagia and dysmenorrhea including fibroids hemostatic disorders obesity-related uterine bleeding idiopathic HMB adenomyosis and endometriosis. The medical management of endometrial proliferation is sometimes treated with supplemental progesterone. While IUDs are safe for most women there are several absolute contraindications to the LNG-IUD including current breast cervical or endometrial cancer having just had a septic abortion or current pelvic inflammatory disease chlamydia or gonorrhea. For women who are potential candidates education is vital and patients should be receive adequate counseling about the benefits and adverse effects.
