Em pacientes com endometriose, a fertilização in vitro (FIV) tem eficácia comparável às de mulheres sem a doença?

Introdução: A endometriose é uma doença sistêmica multifatorial associada a infertilidade feminina, e a escolha do tratamento ideal é muito discutida na medicina reprodutiva, sendo a fertilização in vitro (FIV) uma das opções. Objetivo: Avaliar a eficácia da FIV em mulheres com endometriose comparando com mulheres sem a doença. Metodologia: Foi realizada uma busca nas bases de dados PubMed, BVS e SCIELO, através dos descritores: “Endometriosis”, “Fertilization in Vitro”, “infertility”, “pregnancy success”, “Live Birth”, definidos pelo DeCS. Critérios de inclusão: estudos observacionais e ensaios clínicos randomizados publicados entre 2014-2024. Critérios de exclusão: artigos não relacionados a pergunta científica, publicados anteriores a 2014, revisões sistemáticas e integrativas. Encontrou-se 49 artigos, e após análise, foram selecionados 8 para compor a revisão. Resultados: Dentre os 8 artigos, 50% avaliaram mulheres com endometriose comparadas a mulheres sem a doença, e destes, 50% obtiveram resultados semelhantes nos dois grupos, recomendando FIV em pacientes com endometriose. Outros 75% dos estudos, que avaliaram apenas mulheres com endometriose, submetidas a administração ultralonga de GnRH, não demonstraram eficácia, sendo dispensável no processo de FIV. Conclusão: São necessários mais estudos para avaliar a eficácia da FIV em mulheres com endometriose, porém, observa-se que é um tratamento recomendado para este grupo, visto que alguns estudos demonstram taxas de sucesso semelhantes às observadas em mulheres com outros tipos de infertilidade.
