Clinical evaluation and imaging techniques are the first-line methods for diagnosis, but laparoscopy remains the gold standard. The purpose of this study was to study the clinical symptoms, radiological findings, and factors associated with endometriosis at the Regional Hospital Bamenda. Methods : This was a hospital-based case-control study carried out for five months from 20 th January 2024 to 30 th May 2024 at the gynaecologic unit of the Regional Hospital Bamenda. The study population comprised all women who consulted at the gynaecologic consultation unit. The cases were all women diagnosed with endometriosis. Each case was matched by age with healthy controls from the outpatient consultation unit. Data was collected using a predesigned questionnaire and entered into CS Pro 8.0.1. Descriptive statistics and regression analysis were used for data analysis with significance at p<0.05 and 95% confidence interval. Results: We enrolled 64 cases and 64 controls. All patients diagnosed with endometriosis had pelvic pain, with 75% experiencing cyclical pain. Dysmenorrhea ( 93.8%) and cyclical dyschezia ( 9.4%) were the most common gynaecologic and non-gynaecologic symptoms, respectively. The most common ultrasound finding was endometrioma ( 95.3%). After adjusting for confounders, a history of abdominal surgery (aOR:3.86; 95%CI:1.43 - 11.3 , p= 0.010) and infertility (aOR:4.20; 95% CI:1.50,13.0; p= 0.008) were independently associated with the occurrence of endometriosis, whereas parity ≥2 (aOR:0.37; 95%CI:0.14-0.92, p= 0.036) and a menstrual cycle of ≥29days (aOR:0.14 95%CI:0.05-0.42, p<0.001) were protective factors. Conclusion : All Cases presented with pelvic pain, and endometrioma was the most common radiological finding. Screening and management should be considered for patients with a history of abdominal surgery and infertility. Symptoms Radiological findings Factors associated endometriosis Cameroon INTRODUCTION Endometriosis is a prevalent inflammatory condition characterised by endometrial tissues outside the uterus, primarily on pelvic organs and tissues[ 1 ]. This is an oestrogen-dependent disease that arises during a woman’s reproductive years and poses challenges in diagnosis and management[ 2 ]. Globally, endometriosis is estimated to affect one in ten women of reproductive age[ 3 ]. This represents approximately 190 million women of reproductive age[ 1 , 4 ]. The exact prevalence is unknown in Africa but varies with each country[ 5 ]. A study in Nigeria, which examined women undergoing a diagnostic laparoscopy, reported a prevalence rate of 48.1% [ 6 ], which is one of the highest prevalence rates in Africa. The disease can be asymptomatic, but has varying symptoms including chronic pelvic pain, dysmenorrhea, deep dyspareunia, dysuria, dyschezia, fatigue, and infertility[ 7 ]. These symptoms have a significant impact on the education, work and social relationships of individuals, including social isolation, absenteeism, inability to work long hours and avoiding sexual intercourse with their partner [ 1 , 2 , 7 ]. Moreover, endometriosis is associated with infertility, which has a serious and psychological impact, especially in Africa, where children are considered as wealth [ 7 ]. Considering the aforementioned impacts, it is important to recognise that endometriosis has a serious burden on women's health and should be considered as a public health concern rather than an isolated individual's condition[ 8 ]. Several menstrual and reproductive factors have been identified in Western populations, such as an earlier age at menarche, shorter menstrual cycle lengths, greater menstrual volume, and nulliparity, all of which have been reported as risk factors or indicators of endometriosis[ 9 , 10 ]. However, in the African population, where there is a higher number of deliveries, elevated infection rates, and limited access to various imaging techniques and diagnostic tools, these risk factors may differ. As a result, the presence of various clinical symptoms and ultrasound findings can facilitate a presumptive diagnosis in low-resource settings. In addition, factors associated are indispensable to elaborate efficient and contextualised strategies against this disease that has a serious epidemiological burden as described above in a low and middle-income country. Our study aimed to fill this gap by studying the clinical symptoms, radiological findings and factors associated with endometriosis in one hospital of the North West Region in Cameroon. MATERIALS AND METHODS Study design and population We conducted from January 20th to May 30th 2024 a hospital-based case-control study at the Regional Hospital Bamenda, a third-category health facility acting as a referral hospital in the North-West Region of Cameroon. It is located in Bamenda, which is the capital city of the North West Region. It has many specialists, including surgeons, radiologists and obstetrician/gynaecologists. The study population consisted of all women consulting at the outpatient gynaecological and obstetrics consultation unit during the study period. Definition of participants Cases were defined as women diagnosed with endometriosis. Upon arrival, after obtaining informed consent, clinical evaluation helps to look for symptoms suggestive of endometriosis, such as dysmenorrhea, defined as painful menstruation[ 11 ], dyspareunia, defined as pelvic pain associated with intercourse[ 11 ] or chronic pelvic pain, defined as recurring or persistent pain in the pelvic region that lasts for six months or more[ 11 ]. Afterward, for patients with at least one clinical symptoms, a pelvic ultrasound was performed in search of endometrioma(usually bilateral and rounded, with regular margins and a homogeneous echotexture, as well as being hypoechoic, containing diffuse areas of low echogenicity or debris[ 12 ]), rectal nodules(nodules or irregular hypoechoic lesions in the intestinal wall[ 12 ]) or parietal nodules(nodules or irregular hypoechoic lesions in the intestinal wall[ 12 ]). As recommended by the 2022 ESHRE Guideline[ 13 ], the diagnosis of endometriosis was based on at least one clinical symptom and one radiological finding. Women with no history and no clinical evidence of any gynaecological pathology at the outpatient consultation unit were considered controls. They were matched to cases by age (± 1 year) in a 1:1 ratio. Sample Size Calculation For sample size calculation, we considered a prevalence of 53.6% of nulliparity as exposure in cases[ 14 ] and a prevalence of 26.1% of nulliparity in the control group[ 10 ].. With a desired statistical power of 85% and a significance level (alpha) of 0.05, the minimum required sample size 58 cases and 58 controls using a 1:1 ratio. Study variables A questionnaire designed for the purpose of this study was used, available as supplementary material. We collected sociodemographic data including age, level of education, employment status, marital status and religion. Obstetrical history was assessed through gravidity and parity, grouped into ranges of 0, 1, and ≥ 2, also known as multigravidity and multiparity, respectively. Past history was evaluated through the history of abdominal surgery, family history of endometriosis, alcohol consumption, physical exercise and body mass index, subsequently categorised into underweight, normal, overweight and obese. Menstrual and reproductive factors recorded were: menarche, menstrual cycle length, average duration of menstrual bleeding, hormonal contraceptives and infertility. The clinical symptoms assessed were pelvic pain and its characteristics, dyspareunia, cyclical dysuria, cyclical haematuria, dyschezia and rectorrhagia. The radiological findings collected were the orientation of the uterus and the presence of adenomyosis, pelvic adhesions, echoic free fluid in the pelvis, endometrioma, peritoneal nodules and rectal nodules. Data management and analysis The data were entered into CS Pro version 8.0.1 and analysed using the statistical software R version 4.3.3 and RStudio 2024.04. 1 + 748.pr02. Means or medians were computed respectively for normally distributed or non-normally distributed continuous variables, while frequencies and percentages were calculated for categorical variables. In order to identify factors associated with endometriosis, proportions of categorical variables were compared using chi-square test or Fisher’s exact test, respectively, when expected counts were ≥ 5 or < 5. Means of numeric variables were compared using the unpaired sample t-test when the distribution was normal. When the distribution was not normal, the median was compared using the Mann-Whitney U test. A multivariate logistic regression model including significant variables from bivariate analysis was used to identify independently associated factors with endometriosis. Significant level was set at p-value < 0.05, and a 95% confidence interval RESULTS Out of 1,018 gynaecologic consultations, 65 patients were diagnosed with endometriosis. After one patient declined, a case group of 64 women was formed. An equal number of healthy women were recruited as controls, making a total of 128 participants for the study Clinical symptoms and radiological findings of cases All of the cases presented with pelvic pain, with dysmenorrhea and dyspareunia accounting for 93.8% and 53.1% respectively as the most common gynaecological symptoms and cyclical dyschezia (9.4%) as the most common non-gynaecological symptoms. Among the different types of radiological findings of endometriosis, endometrioma accounted for 95.3%, peritoneal nodules for 3.1% and 46.1% had an associated adenomyosis. About half of the participants (53.1%) had pelvic adhesions.(Table 1 ). Equally, there was no association between endometriosis and sociodemographic characteristics (Table 2 ). Menstrual and reproductive characteristics associated with endometriosis Cases had a lower median parity (0[0.0, 2.0]) compared to controls (1.0 [0.0 ,4.0]) with a p value of 0.057. Furthermore, multiparity was shown to significantly reduce the odds of developing endometriosis (cOR = 0.37, p = 0.017 ). Median gravidity in both groups and multigravidity was not associated with endometriosis. In addition, menstrual cycle length ≥ 29 was found to be a protective factor of endometriosis (cOR = 0.17,p < 0.001 ), but there was no significant association between age of menarche and average duration of bleeding. Infertility was significantly associated with endometriosis(cOR = 3.92, p = 0.002 )(Table 3 ). After adjusting for confounders, factors independently associated with endometriosis were: a history of abdominal surgery (aOR:3.86, 95%CI:1.43–11.3), multiparity (aOR:0.37; 95%CI:0.14–0.92), menstrual cycle length of ≥ 29 days(aOR: 0.14; 95%CI: 0.05–0.42) and infertility(aOR: 4.20; 95% CI: 1.50–13.0). Early diagnosis and the identification of risk factors in a defined context are important to elaborate polices to fight against this disease. In our study, all cases had pelvic pain with dysmenorrhea and cyclical dyschesia, the most gynaecological and non-gynaecological symptoms, respectively. The most common radiologic finding was endometrioma. Factors independently associated with endometriosis were history of abdominal surgery, multiparity, menstrual cycle length of ≥ 29 days and infertility. In our study, the most prevalent gynaecological symptoms were dysmenorrhea (93.8%) followed by periovulatory pain (68.8%) and dyspareunia(53.1%). A similar pattern was described by other study.[ 14 , 15 ]. Fawole et al. in Nigeria[ 6 ] reported dyspareunia(58.5%) as the most common symptom, followed by dysmenorrhea(52.6%). This can explain the pain experienced by these patients, therefore it is important to have well-established strategies for early diagnosis and screening to fight against the disease. According to the ESHRE guidelines of 2020, echography has been placed as first-line imaging in the diagnosis of endometriosis[ 13 ]. This is due to its high sensitivity(97.6%) and specificity (73.8%) in the diagnosis of endometrioma and deep infiltration endometriosis[ 16 ]. This could explain our findings where we found that endometrioma had the greatest echographic findings, accounting for 95.3% of all ultrasonographic findings. This was similar to the findings of Bilkissou et al[ 15 ]. Hence in addition to high-prevalence symptoms like dysmenorrhea and dyspareunia as described by other studies[ 6 , 14 , 15 ], ultrasound could be used as a first-line imaging technique in the diagnosis of endometriosis in low and middle-income countries where there is limited access to standard imaging techniques Infertility was associated with endometriosis. This finding is in accordance with the findings of Saha et al[ 10 ]; however, it is not clear whether endometriosis is the cause or consequence of infertility. In our study, multiparity was found to be a protective factor but not gravidity. This finding is similar to that of Saha et al. [ 10 ] in Sweden and Burghaus et al. in Germany[ 9 ] who reported that increasing gravidity and parity reduced the risk of having endometriosis. Hence, based on this, we can hypothesise that infertility could be a consequence of endometriosis, by preventing women from achieving pregnancy, thus increasing parity helps as a treatment. This implies that a cohort study will help to bring light on whether endometriosis is a cause or consequence of endometriosis, which is determinant for strategies to fight against the disease, knowing the importance of childbearing in the African population With respect to menstrual cycle characteristics, it has been hypothesised according to the Samsons theory of retrograde menstruation that increasing exposure to menstrual bleeding is a risk factor for endometriosis, implying that early menarche, a short cycle and long duration of menstrual bleeding increase the risk of developing endometriosis[ 17 , 18 ]. Our study revealed menstrual cycle duration of more than 29 days was a protective factor similar to that of Burghaus et al in Germany[ 9 ]. This finding can help clinicians identify individuals at risk. Patient with consistently shorter cycles may be counselled and evaluated for endometriosis earlier if symptoms. There was no association between age at menarche or duration of menstrual bleeding and endometriosis, as compared to other studies[ 10 , 19 , 20 ] reporting an inverse relationship between age at menarche and the risk of developing endometriosis. This could be explained by our limited sample size. Further investigation with a larger sample is needed to shade more light on these factors. During abdominal surgery, changes in peritoneal tissues occur due to surgical trauma, which may allow ectopic cells to bind to these tissues [ 19 ]. Similarly, the theory of direct inoculation hypothesises that direct injury to the peritoneum resulting from abdominal surgeries may facilitate spread, adhesion and invasion by endometrial fragments regurgitated into the pelvis[ 17 ]. This could explain our findings of the association between abdominal surgery and the occurrence of endometriosis. These findings are similar to the results of Lui et al. in China[ 19 ]. Hence importance of meticulous techniques to minimise the spillage of endometrial tissue during surgeries. Considering the mechanism by which hormonal contraceptives specifically oral contraceptives, suppress ovulation and hence reduce menstrual bleeding, their use might have a protective effect on the development of endometriosis[ 21 ], this is seen in the sduy of Burghaus et al[ 9 ], however, there was no association with endometriosis in our study and equally described by Saha et al[ 10 ]. This lack of association may be due to limitations in our sample size, the characteristics of our study population. This highlights the need to for further research to identify the subpopulation for which a hormonal contraceptive may be effective in preventing or managing symptoms, looking at the burden of the disease. Limitations Limitations of our study included: Absence of ultrasound and other diagnostic techniques in the control group, given the asymptomatic nature of the condition, it is possible that some participants in the control group could have had subclinical forms of endometriosis. However, the highly symptomatic presentation of the disease, including chronic pelvic pain, infertility, will make endometriosis be found in very few participants of the control group. We could not use the imaging gold standard, MRI, in our study due to limited resources. Nevertheless, we used ultrasound, which has good specificity and sensitivity in diagnosing endometriosis, as described by Asgari et al. [ 16 ]. Due to our limited resources, biomarkers such as CA-123, inflammatory markers, and microRNA could not be investigated. These could be potential associated factors. Regardless, we collected the principal clinical factors that could be accessible in the population CONCLUSION Cyclical pelvic pain was the most common symptom, followed by dysmenorrhea and dyschezia. Endometrioma was the most frequent radiological finding. The study revealed, having a history of surgery and infertility was inversely associated with endometriosis. Furthermore, multiparity and longer menstrual cycles were identified as protective factors against the disease. These results highlight the importance of identifying specific clinical presentations and associated factors within this population, which can significantly improve diagnostic accuracy in low-resource settings. This information is crucial for developing and implementing effective public health strategies, informing clinical practice, and tailoring patient education programs to improve the management and outcomes of endometriosis. Clinical evaluation and imaging techniques are the first-line methods for diagnosis, but laparoscopy remains the gold standard. The purpose of this study was to study the clinical symptoms, radiological findings, and factors associated with endometriosis at the Regional Hospital Bamenda.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eMethods\u003c/strong\u003e: This was a hospital-based case-control study carried out for five months from 20\u003csup\u003eth\u003c/sup\u003e January 2024 to 30\u003csup\u003eth\u003c/sup\u003e May 2024 at the gynaecologic unit of the Regional Hospital Bamenda. The study population comprised all women who consulted at the gynaecologic consultation unit. The cases were all women diagnosed with endometriosis. Each case was matched by age with healthy controls from the outpatient consultation unit. Data was collected using a predesigned questionnaire and entered into CS Pro 8.0.1. Descriptive statistics and regression analysis were used for data analysis with significance at p\u0026lt;0.05 and 95% confidence interval.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults:\u003c/strong\u003e We enrolled 64 cases and 64 controls. All patients diagnosed with endometriosis had pelvic pain, with 75% experiencing cyclical pain. Dysmenorrhea ( 93.8%) and cyclical dyschezia ( 9.4%) were the most common gynaecologic and non-gynaecologic symptoms, respectively. The most common ultrasound finding was endometrioma ( 95.3%). After adjusting for confounders, a history of abdominal surgery (aOR:3.86; 95%CI:1.43 - 11.3\u003c/p\u003e\n\u003cp\u003e, p= 0.010) and infertility (aOR:4.20; 95% CI:1.50,13.0; p= 0.008) were independently associated with the occurrence of endometriosis, whereas parity ≥2 (aOR:0.37; 95%CI:0.14-0.92, p= 0.036) and a menstrual cycle of \u0026nbsp;≥29days (aOR:0.14 95%CI:0.05-0.42, p\u0026lt;0.001) were protective factors.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eConclusion\u003c/strong\u003e: All Cases presented with pelvic pain, and endometrioma was the most common radiological finding.
