This study retrospectively compared the efficacy of three mouthwash agents; benzydamine hydrochloride, sodium bicarbonate (NaHCO3), and ankaferd hemostat in preventing and treating chemotherapy-induced OM. Materials and Method : Data from 82 patients were analyzed over a three-week period, assessing OM severity and laboratory parameters. Results Here we found that benzydamine hydrochloride and ankaferd hemostat were associated with lower rates of OM at the third week, whereas NaHCO3 use correlated with a higher incidence of OM. Additionally, higher platelet counts were linked to reduced OM risk. Discussion These findings suggest that benzydamine hydrochloride and ankaferd hemostat may offer better local mucosal protection than NaHCO3 in this patient population, highlighting the potential for improved supportive care strategies in chemotherapy-induced OM management. oral mucositis hematologic malignancy chemotherapy-induced oral mucositis oral mouthwash treatment benzydamine hydrochloride sodium bicarbonate (NaHCO3) ankaferd hemostat Figures Figure 1 Introduction Oral mucositis (OM) is characterized by inflammation and ulceration of the oral mucosa, often presenting with erythema and pain. The incidence of OM varies widely, ranging from 40% to 100%, depending on factors such as the type of malignancy, chemotherapy regimen, radiation dosage, patient age, oral health, and neutrophil count ( 1 , 2 ). Oral mucosal epithelial cells are particularly susceptible to chemotherapy-induced damage compared to other cell types ( 3 ). Risk factors for OM are generally categorized as patient-related or treatment-related. Patient-related risk factors include young age (under 20 years), female gender, inadequate oral care, infections (bacterial, fungal, or viral), malnutrition, dry mouth, and immunosuppression. Treatment-related factors include the type and dose of chemotherapeutic agents (especially alkylating agents and antimetabolites), as well as the location and dose of radiotherapy ( 4 , 5 ). Cancer treatments can induce the production of reactive oxygen species (ROS) and activate nuclear factor kappa B (NFkB) and inflammatory cytokines, leading to mucosal damage, ulcer formation, and the development of mucositis ( 6 ). OM typically begins during periods of neutropenia, with erythematous areas appearing in the oral cavity 4–5 days after the initiation of chemotherapy. Ulcers usually develop after 7–10 days, and these lesions tend to heal within approximately two weeks from the start of chemotherapy ( 7 , 8 ). Patients with OM may experience severe pain, difficulty swallowing, taste alterations and weight loss ( 8 ). Several classification and staging systems for OM have been described, with the World Health Organization (WHO) system being the most widely used. This system grades OM severity from 0 to IV ( 7 , 9 ). Over time, numerous agents have been investigated for the prevention and treatment of OM. Recommended oral care practices include brushing teeth twice daily and rinsing the mouth with 0.9% saline or sodium bicarbonate (NaHCO3). Other interventions used in the prevention and management of OM include benzydamine hydrochloride, oral glutamine, chlorhexidine gluconate, low-level laser therapy, and oral cryotherapy ( 7 , 8 , 10 ). Among newer agents, Ankaferd hemostat (ABS; Ankaferd Blood Stopper®), a plant-based product, has recently been introd uced ( 9 ). According to a recent review, the most promising results in reducing OM severity were observed with mouthwashes containing Dentoxol, Plantago major extract, and thyme honey, as well as with zinc oxide paste and oral L-glutamine supplementation ( 11 ). Evidence demonstrates that low level laser therapy (LLLT) can potentially be used as a routine preventative intervention for OM in patients undergoing chemotherapy and radiotherapy, or combination therapy ( 10 ). Benzydamine is an analgesic, anti-inflammatory, and antimicrobial agent that has been shown to alleviate the symptoms of OM. Sodium bicarbonate (NaHCO3) is a non-irritating, non-dehydrating compound that creates an alkaline environment in the mouth but does not possess anesthetic properties ( 3 ). Ankaferd hemostat is approved for external and dental use, including the management of minor or major bleeding following surgery. In addition to its hemostatic effects, Ankaferd hemostat exhibits anticancer, anti-angiogenic, antifungal, antimicrobial, antioxidant, and antimutagenic properties. Studies have demonstrated its effectiveness in both the treatment and prophylaxis of OM. For example, one study found that using Ankaferd hemostat as a mouthwash for OM prophylaxis in patients undergoing chemotherapy for colorectal cancer provided effective protection against OM ( 9 , 12 ). The clinical use of all three agents—benzydamine, NaHCO3, and Ankaferd hemostat—for both OM prophylaxis and treatment has been documented in the literature ( 9 , 13 , 14 ). Material and methods Patients and data collection: A total of 82 patients were included in the study; 46 patients (OM naive) who used prophylactic oral topical mouthwash and 36 patients who developed OM while receiving chemotherapy regimens with a moderate to high risk of OM. The clinical and laboratory data of the patients, who received chemotherapy in the Hematology Service between November 2023 and January 2025 at Afyon Health Sciences University, were analyzed by file evaluation. It was aimed to compare the efficacy of these topical oral mouthwash products in patients with hematologic malignancies. Patients, older than 17 years old were retrospectively selected based on defined eligibility criteria, including the development of OM after chemotherapy and the absence of prior OM history. The file information of the 82 patients were reviewed and the laboratory values and OM scores of the patients at the 1st week, 2nd week and 3rd week from both the beginning of the treatment and the onset of OM were analysed. The efficacy of topical oral mouthwash products used for both OM prophylaxis and treatment was analysed. In addition to the leukocyte (Wbc), neutrophil (neu), lymphocyte, hemoglobine (Hgb), thrombocyte (Plt), LDH, AST, ALT, bilirubin levels, Ferritin, B12, Folate, TSH levels, disease diagnosis, disease stage, ECOG performance scores (from grade 0 to grade 4) and OM scores at the first, second and third week follow-up were recorded. Classification and scoring of the OM; Staging of the OM was performed according to the WHO mucositis evaluation form and OM scores of patients receiving chemotherapy were recorded at three-week follow-up. This is a grading system developed by WHO according to the clinical appearance and functional status of OM. The classification is as follows: OM grade 0 (normal), grade 1 (mild focal changes (erythematous areas, no pain and tenderness yet), grade 2 (painless ulcers, mild pain sensation), grade 3 (painful erythema, edema or ulcers; depth of > 2 mm and less than half of the mucosa, no bleeding, can be fed orally with liquid diet), and grade 4 (erythema, edema or ulcers; more than half of the mucosa, severe pain, bleeding, no nutrition orally, parenteral nutrition support may be required) ( 9 ). Selection and monitoring of alternative oral mouthwash types: In our hematology servis, in addition to systemic antibacterial, antiviral and antifungal therapies, benzydamine HCL, NaHCO3, and less frequently the solution form of ankaferd hemostat which is provided optionally, are used topically as mouthwash in oral care for both propylaxis and treatment of OM. During chemotherapy, patients who developed OM while using one of the following as a prophylactic topical mouthwash: NaHCO3, benzidamine HCL or ankaferd hemostat, were randomly assigned to continue using the same mouthwash product or switch to a different product, based on clinical necessity and the clinician's preference. During the follow-up, the topical mouthwash agent was continued in combination with systemic treatments until OM resolved. OM scores were recorded at first, second and third weeks for patients both receiving prophylactic oral mouthwash agents and those receiving treatment due to OM during follow-up. Benzydamine HCL and NAHCO3 were available at our hospital pharmacy, but patients who obtained ankaferd hemostat solutions, which were not available at the pharmacy, applied as a 2x5 milliliter mouthwash on the required days of the treatment process. We prepared the ankaferd hemostat mouthwash solution by mixing it with water, explained to the patients how to apply, and gave it to them. (method for preparing Ankaferd Hemostat mouthwash: Firstly, ensure that the water has been boiled at 100°C and cooled, pour 4 ml of water into the 5 ml measuring cup given to the patient, then add 1 ml of ankaferd hemostat solution to the remaining space. Patients perform moutwash twice daily using 5 ml of Ankaferd Hemostat gargle). Statistical Analysis: The compatibility of continuous variables with normal distribution was examined with the Shapiro Wilk test. In independent two-group analyses, Mann Whitney U test was used in cases where the data did not exhibit normal distribution or the number of samples was less than 30 in at least one group, and Independent-Sample T Test was used in cases where the data exhibited normal distribution. Pearson Chi-Square Test, Fisher's Exact Test, Fisher's Freeman Halton Exact Test, Yates Correction and Post Hoc Bonferroni Correction were used to analyze independent categorical variables. In repeated measures analyses, Repeated Measures ANOVA was used for normally distributed data and Friedman's Test was used for non-normally distributed data. Logistic Regression Analysis was performed to determine the factors affecting the likelihood of mucositis. Data were analyzed using IBM SPSS Statistics 25 © 2017 software. A significance level of p < 0.05 was accepted. An approval has been obtained from the Ethics Committee of Afyon Health Sciences University for the study (no: 2024/478 and date: 06.12.2024). Results Individuals who received treatment and prophylaxis with local agents for OM were similar in terms of age, marital status, diagnoses, treatment protocols and disease status (Table 1). Table-1. In the prophylaxis group, 17.4% developed grade 1 OM in the second week and 25% in the third week. In the treatment group, grade 1 OM was 52.8% at baseline, 38.9% in the second and 12.1% in the third week. The differences between OM grades in the prophylaxis and treatment groups were statistically significant at second week (p < 0.05) (Table 3 ). In the analysis of the incidence of the OM according to the local mouthwash products at third week, while the incidence of OM was similar in patients given ankaferd and benzydamine HCL, the rate of patients with OM at third week given NACO3 was significantly higher than the rate of patients without OM. In the intra- and inter-group comparison of laboratory findings, the Wbc-1, neutrophil-1, PLT-1, and TSH parameters were significantly higher in the prophylaxis group (Table 4 ). In patients with ECOG score 2, the risk of developing OM in the second week increased 5.143 times, while it was 12 times higher in patients with ECOG score 4. An increase of one point in plt measurement decreased the risk of developing OM (OR = 0.995) (Table 5 ). In individuals who used ankaferd hemostat as OM prophylaxis and treatment, the proportion of patients without OM was higher than those with OM at both 2nd and 3rd week (p > 0.05) (Table 5 , Fig. 1 ) Discussion In this study, we compared the efficacy of three topical oral mouthwash products—NaHCO3, benzydamine HCL, and Ankaferd hemostat—used during the treatment of hematologic cancers. No significant differences in OM development were observed among the three agents during the first and second weeks of treatment. Although OM rates with Ankaferd hemostat did not differ significantly in the second and third weeks, the proportion of patients without OM was higher than those with OM. At the third week, OM rates were similar between patients receiving Ankaferd hemostat and benzydamine HCL, whereas the incidence of OM was significantly higher in patients treated with NaHCO3 compared to those without OM. Notably, the probability of developing OM decreased by 72.5% at the third week in patients receiving benzydamine HCL prophylaxis, while it was 5.33 times higher in those using NaHCO3. OM can be prevented, and its management requires a multidisciplinary approach. Although several studies have compared and supported the efficacy of topical oral mouthwash products for the prevention and treatment of OM in cancer patients, the overall effectiveness of these agents remains controversial, and further research is needed ( 9 ). A recent study demonstrated that oral administration of probiotics could reduce the incidence of OM ( 15 ). A systematic review reported that the most effective methods for treating and preventing OM included laser therapy, cryotherapy, antimicrobial agents, Lactobacillus brevis lozenges, zinc supplementation, and benzydamine ( 16 ). Laser therapy, in particular, was associated with improved healing rates of OM, likely due to reduced ROS generation ( 17 ). In another recent study comparing the efficacy of Plantago major extract, chlorhexidine, and sodium bicarbonate in the treatment of chemotherapy-induced OM in patients with solid tumors, no significant differences were found in healing time or pain intensity among the three treatment groups ( 18 ). The MASCC/ISOO clinical practice guidelines for the management of mucositis secondary to cancer therapy recommend the use of multi-agent combination oral care protocols for the prevention of OM. However, these guidelines do not specifically endorse the use of saline or NaHCO3 rinses for the prevention or treatment of OM in cancer patients, due to limited supporting data ( 19 ). In our study, we evaluated three agents—benzydamine HCL, NaHCO3, and Ankaferd hemostat—administered individually (not in combination), typically alongside systemic antiviral, antibacterial, and antifungal therapies during chemotherapy. We found that the risk of OM at the third week was lower with Ankaferd hemostat and benzydamine HCL, while the incidence of OM was higher with NaHCO3 in both prophylaxis and treatment groups, with no observed adverse effects (p values: 0.380, 0.031, respectively). The lack of anesthetic effect of NaHCO3 may account for its lower efficacy in treating OM compared to the other agents. Additionally, although Ankaferd hemostat appeared effective in both prophylaxis and treatment, the lower incidence of OM did not reach statistical significance, likely due to the small sample size and limited use in our patient population. Diabetes mellitus, low white blood cell count, neutropenia, lymphopenia, low hemoglobin levels, malnutrition, poor oral hygiene, tobacco use, and low ECOG performance status have all been identified in the literature as potential risk factors for the development of OM ( 20 ). The relationship between smoking and OM remains controversial; while smoking has proinflammatory effects, some studies have reported a higher risk of OM among smokers ( 21 ). Additionally, both hypothyroidism and hyperthyroidism have been associated with OM, potentially due to impaired periodontal health resulting from salivary secretion abnormalities, bone structure disorders, and glossitis ( 22 ). In our study, during the second week in the prophylaxis group, factors such as age, diagnosis, treatment protocol, smoking and alcohol use, tooth brushing, hemoglobin levels, WBC and neutrophil counts, LDH, TSH, free T3, and free T4 levels were not significantly associated with OM risk. However, ECOG scores of 2 and 4, as well as platelet count, were significantly associated with OM risk. Notably, an increase in platelet count was associated with a decreased risk of developing OM (OR = 0.995), suggesting a potential protective effect. Conclusion In our study comparing the efficacy of topical oral mouthwash products for the prevention and treatment of OM in patients undergoing chemotherapy, the incidence of OM at the third week was higher in both the prophylaxis and treatment groups using NaHCO3 compared to the other agents (p values: 0.380 and 0.031, respectively). This suggests that other agents may offer better local mucosal protection than NaHCO3. In addition, it was observed that ankaferd hemostat, a new agent in OM, was associated with a lower incidence of OM in both prophylaxis and treatment, although this was not statistically significant (Table 5 , Fig. 1 ). Consistent with previous literature, the risk of OM was higher in patients with an ECOG score of 4 compared to those with a score of 2. Notably, a one-unit increase in platelet count, a finding not previously reported in the literature, was associated with a lower risk of OM. Limitations: Much more trials, preferably prospective and randomized controlled with larger patient populations and homogeneous patient groups are needed to determine the efficacy, advantages and disadvantages of these molecules.
