This retrospective study evaluated how accurately preoperative clinical diagnostics identified endometriosis among 543 women aged 16–55 who underwent surgery at the University Women’s Clinic in Lübeck (Jan 2005–Dec 2006), comparing cases with operative-confirmed endometriosis (n=156; including first diagnosis vs recurrence) to controls with benign lower-gynecologic conditions where endometriosis was excluded. It found that in the first-diagnosis group, clinicians raised no suspicion in 57.4% at the clinic and 70.3% among referring specialists, whereas suspicion was less frequently missed in the recurrence group. In controls, endometriosis was suspected in a small number of patients but not confirmed surgically, yielding high specificity (95.6% clinic, 96.9% referring physicians) alongside moderate sensitivity (70.9% clinic, 63.7% referring physicians), with the highest false-negative rates occurring in early-stage disease.
