Malignant Transformation of Endometriosis: A Rare Case of Extrauterine Endometrial Stromal Sarcoma
This paper reports a rare case of extra-uterine endometrial stromal sarcoma (ESS) arising in the setting of extensive deep-infiltrating endometriosis in a 31-year-old woman with chronic pelvic pain. Using pelvic ultrasound and laparoscopic exploration, the authors identified bilateral endometriomas and extensive endometriosis infiltrating bowel and ureter, leading to multiple surgical procedures including nephroureterectomy, bowel resection with ileostomy, subtotal hysterectomy, and bilateral salpingo-oophorectomy. Histology showed estrogen and progesterone receptor–positive low-grade ESS located primarily extrauterine, with concurrent endometriosis in the omentum, large bowel, and ovaries, and no uterine involvement.
