The second case involved a 38-year-old undergoing surgical evaluation for polycystic adnexal findings who had bilateral endometriomas, pelvic adhesions, and suspected adenomyosis; after endometrioma excision and adhesiolysis, she developed perimenstrual febrile episodes up to 40°C three months postoperatively despite internal and gynecologic infectious workup, and symptoms resolved after endometrial ablation for bleeding control. As a case series of two patients, the major limitation is that findings are not generalizable beyond these individual observations.
