A 20% dose reduction was recommended based on PopPK modelling. In this retrospective study we reviewed absolute neutrophil counts over 24 weeks to assess whether the model informed dose achieved optimal myelosuppression in children on maintenance therapy who were either switched to or started on PREVALL TM suspension at 80% of the recommended tablet dose. Our analysis showed that PREVALL TM suspension at 80% of the tablet dose is suits about 50% children on ALL maintenance therapy. We recommend this dose as a starting dose for children receiving 6MP PFOS. Further prospective studies are needed to validate these findings. Children with NUDT15/TPMT mutation could receive doses as low as 2 mg daily giving sustained stable exposure of 6MP.
