This case report describes a 42-year-old male presenting with atypical symptoms leading to the diagnosis of concurrent cardiac and pelvic-iliofemoral hydatid cysts. We discuss the clinical presentation, diagnostic challenges, imaging findings, surgical management, and postoperative outcomes, emphasizing the importance of considering hydatid disease in differential diagnoses of cystic lesions in endemic regions. A comprehensive review of the literature highlights the rarity of these locations and the need for multidisciplinary management.
