Abstract

espanolEl manejo multidisciplinario del dolor pelvico cronico (DPC), con la participacion planificada e interactiva de diferentes especialistas (rehabilitador, urologo, ginecologo, gastroenterologo, coloproctologo, especialista en dolor, psiquiatra, psicologo, fisioterapeuta…), cobra suma importancia tanto en su estudio como en su tratamiento. Es necesario un abordaje temprano, multidisciplinario, con inicio rapido y multimodal de tratamientos, y atencion a la esfera psicologica. Estas estrategias evitaran fenomenos de sensibilizacion central y desarrollo de memoria del dolor, que va a perpetuar y cronificar estos cuadros. El abordaje terapeutico del DPC se dirigira tanto al tratamiento de la enfermedad que lo provoca como al tratamiento del DPC en si mismo. La existencia de una gran variedad de modalidades terapeuticas refleja la limitada eficacia de estas, siendo necesario combinarlas o el uso alternativo de varias de ellas. EnglishThe multidisciplinary management of chronic pelvic pain (CPP), with the planned and interactive participation of different specialists (physiatrist, urologist, gynecologist, gastroenterologist, coloproctologist, pain specialist, psychiatrist, psychologist, physiotherapist) is important in the treatment. It is necessary an early approach, multidisciplinary, rapid onset and multimodal treatments, and attention to the psychological sphere. These strategies will avoid phenomena of central sensitization and development of memory of the pain that will perpetuate and chronify these pictures. The therapeutic approach to CPP will be directed both to the treatment of the disease causing it and to the treatment of CPP itself. The existence of a great variety of therapeutic modalities reflects the limited effectiveness of these, being necessary the combination or the alternative use of several of them.
