This case series discusses three women with endometriosis who developed pelvic abscesses after oocyte retrieval despite antibiotic prophylaxis, questioning the necessity and optimal choice of antibiotics while noting povidone-iodine douching may reduce infection rates.

Abstract
Background: Pelvic inflammatory disease with progression to pelvic\nabscess is a rare complication after oocyte retrieval during in vitro\nfertilization cycles. However, in patients with endometriosis the risk\nappears to be increased. Many authors agree on the need for antibiotic\nprophylaxis during the oocyte retrieval in these patients, but there is\nno consensus regarding the best antibiotic. Case: We discuss 3 clinical\ncases of tubo-ovarian abscess in women with endometriosis after oocyte\nretrieval despite antibiotic prophylaxis between 2004 and 2011 at our\ncenter, and discuss our experience in the context of earlier reports.\nConclusion: It is unclear whether antibiotic prophylaxis is necessary\nin these women, and which antibiotic is best. Only douching with\npovidone-iodine appears to decrease the rate of pelvic infection.
