Inflammatory cytokines, such as interleukin-6(IL-6), IL-1, and tumor necrosis factor alpha (TNF-α) released from the lung epithelium in COVID-19 infection are very similar to cytokines which are released in inflammatory rheumatic diseases. Immunosuppressive drugs are the cornerstone of treatment in many rheumatologic diseases. Prolonged use of these agents might result in change in the clinical characteristics of these individuals with COVID-19. Objective : To determine the clinical findings and disease severity of COVID-19 infection in patients with rheumatologic diseases who were on immunosuppressive therapy. Method : We retrospectively investigated 1000 subjects who were more than 18years old with rheumatologic diseases on immunosuppressive therapy in the recent three months. Demographic data, rheumatic disease characteristics, medications, and presence of any co- morbid conditions were documented by detail. Presence of severe COVID-19 infection features were compared between users and non-users of different immunosuppressant agents. Multivariable logistic regression was applied, and adjusted odd ratio (OR) and its 95% CI were estimated. Results : 539 subjects had positive history of COVID-19 (COVID-19+). Pre-exposure to prednisolone was associated with severe disease (OR: 2.52, 95%CI (1.192-4.255)). Pervious use of MTX (OR: 0.348, 95%CI (0.210-0.575)) and biologic drugs (OR: 0.332, 95%CI (0.159-0.694)) had protective effect on the severity of the disease. Other traditional disease modifying anti-rheumatic drugs (DMARD) had no clinical effect on the outcome of COVID-19 infection. Conclusion : In the current study, among rheumatoid patients on immunosuppressants, previous treatment with corticosteroids was associated with increased incidence, poor prognosis, and increased mortality. Pre-exposure to MTX or biologic agents such as anti-TNF and anti-CD20 had protective effect against coronavirus. Further meta-analysis and systematic review in large population is needed to accurately determine the effect and mechanism of these drugs on this new viral illness. COVID-19 immunosuppressants rheumatoid patients Introduction The novel coronavirus, which had initially led to pneumonia of unknown etiology in a series of patients in Hubei province in China,, in December 2019, has now precipitated a global health crisis caused by the Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2).(1)Old people and those with comorbidities such as diabetes mellitus and cardiovascular disease are at high risk of severe illness.(2) However, the exact mechanisms of this disease and why some people develop a mild form of the disease while others presented a severe disease with fatal outcome are still not well understood. Cytokine storm has been considered as the main pathogenesis in COVID-19 patients with severe disease.(3)Inflammatory cytokines, such as interleukin (IL) 6, IL-1, and tumor necrosis factor alpha (TNF-α) released from the lung epithelium in COVID-19 infection, are very similar to cytokines which release in inflammatory rheumatic diseases. In critically ill patients, especially those who need intensive care unit (ICU) care, serum level of IL-2,6,10 and TNF-α increases significantly. Also, a decrease in the lymphocytes and natural killer cells has been documented in these patients.(4, 5) Many of the anti-inflammatory drugs and immune modulator agents including tocilizumab, a monoclonal antibody against the interleukin-6 receptor, have been used experimentally as the treatment of COVID-19.(6) The use of tocilizumab was associated with better outcome in patients with severe COVID-19 infection in some recent studies.(7) on the other hand, rheumatoid patients with COVID-19 infection who had a history of Anti-CD20 agent ( rituximab) or Janus kinas inhibitors (JAKi: tofacitinib, baricitinib and upadacitinib ) usage, experienced more severe illness and poor outcome in Sparks et al.’s study.(8) Immunosuppressive drugs, based on molecular target therapy, are the cornerstone of treatment in many rheumatologic diseases, and infections are one of the most common causes of morbidity and mortality among these patients. Considering the rising number of affected individuals across the globe, patients with rheumatologic diseases should be considered as high-risk population. Despite the multitude of studies conducted so far to characterize the clinical findings in patients with COVID-19 infection, the full spectrum of the clinical presentations in these patients remain largely unknown.(9, 10) Due to this limitation and since there is no data about this important issue in Iran, we designed this survey to explore the clinical findings and disease severity of COVID-19 among Iranian patients with rheumatic diseases on immunosuppressive agents. Methods Study subjects and setting: We retrospectively investigated 1000 subjects with rheumatologic diseases who are registered in Iran, Shiraz. The information of the patients was collected through the treatment department of Shiraz University of Medical Sciences (SUMS), the hospitalization centers of COVID-19 patients in Shiraz, and the patients referred to rheumatology clinics. The diagnosis of COVID-19 was based on laboratory-confirmed COVID-19 infection using real-time PCR or lung high resolution CT scan (HRCT) compatible with COVID-19 infection. Men and women aged more than 18 years and the rheumatologic patient on immunosuppressive therapy (prednisolone, methotrexate, azathioprine, mycophenolate, cyclosporine, tacrolimus, cyclophosphamide, rituximab and anti-TNFs) in the three months were included in this survey. The aims of the study were explained to the patients or their relatives in full detail and informed consent was obtained from all subjects or their legal guardians. This study was conducted according to the ethical principles and recommendations outlined in the Declaration of Helsinki and approved by the ethics committee of SUMS with contract number of IR.SUMS.MED.REC.99-01-0122393. Exclusion criteria: Unwillingness to take part in the study, pregnant women, history of current active hepatitis, and untreated cancer were the exclusion criteria of this study. Data gathering: Demographic data including age and sex, rheumatic disease characteristics and the duration of disease, medications, and the history of the presence of any co-morbidities including chronic obstructive pulmonary disease, diabetes, high blood pressure, coronary artery disease, cerebrovascular disease, hepatitis, cancer, chronic kidney disease, and thyroid disease were asked and recorded in detail. If any para-clinical studies were performed recently, they were asked to share them with the research team. For patients admitted in hospital, their laboratory findings on admission and during the treatment were evaluated. The oxygen saturation percentage (O2 sat) in the air room, route of oxygen administration (nasal cannula, non-re breather mask, noninvasive and invasive ventilation), and prognosis of the patients were recorded. The percentage of lung involvement in High Resolution Computed Tomography (HRCT) was also reviewed as reported by expert radiologist by using previous studies.(11, 12)Patients were divided into five groups based on the history of using different types of immunosuppressant drugs (prednisolone; methotrexate (MTX); hydroxychloroquine (HCQ); biologic agents including etanercept, infliximab and rituximab; and other traditional disease-modifying anti-rheumatic drugs(DMARDs) including mycopholate mofetil, azathioprine, cyclosporine, cyclophosphamide, leflunomide and tacrolimus).Mean ± standard deviations (SD) of O2sat in the air room, lung parenchymal involvement and the percentage of the presence of dyspnea, hospital admission, noninvasive and invasive ventilation, and mortality rate were compared between the users and non- users of these five immunosuppressant drugs. The presence of two out of the three risk factors: 1) lung involvement ≥ 50% in HRCT(13) , 2) O2saturation≤ 88%(14), and 3) the need for noninvasive or invasive ventilation or death due to COVID-19infection was defined as severe disease. Statistical Analysis We estimated proportions and means±SD to describe the data. 95%confidence interval (CI) of prevalence was estimated. Independent t-test and chi-square test were used. The effect of confounding variables was controlled by using multivariable logistic regression. Two logistic regression models were fitted to assess the correlates of severe COVID-19 categories. The significance level was setat0.05. Stata11.2 software (StataCorp. LP) and SPSS version 21 were used for data analysis. Results Of1000subjects, 981(male =217, female=764) were included in this study. The ratio of people who were infected with COVID-19 (COVID- 19+) to those who were not infected (COVID-19- ) was 1.2/1(539/442). The rate of hospital admission was 48.05% (number (n)=259) and 61 patients died due to severe illness. The underlying rheumatologic diseases in this population were rheumatoid arthritis (RA)(n=403; 41.1%), vasculitis including Wegner, Bechet, giant cell arthritis, Churg Struss, Takayasu arteritis 21.3(n=209),) systematic lupus erythematous 20.9(n=205), seronegative spondyloarthritis4.7(n=46, uveitis 4.5(n=44),) scleroderma3.7(n=36, Sjogren 1.9(n=19), myositis 1.1(n=11), sarcoidosis 0.5(n=5), and undifferentiated connective tissue disease0.3(n=3).Nonspecific symptoms such as fever, fatigue, anorexia, and insomnia were reported by almost 86.46% of COVID-19+ patients. The prevalence of the symptoms of respiratory tract infection and gastrointestinal involvement was 76.8 and 33.58, respectively. Oxygen saturation in the room temperature ranged from 52%to 98% with mean±SD of 88.80±7.65 among COVID-19 +patients. The mean of total lung involvement in HRCT was 29.45±27.36in the affected patients. In the COVID-19+ subjects, presence of HTN (p.value:0.007) and DM(P=0.001) was significantly more than COVID-19- patients. Consumption of prednisolone was statistically more prevalent in COVID+-19 patients (n=403(74.8 %), 95%CI: 71.33-78.67 vs. n=274(62%), 95%CI: 61.95-62.05). (P<0.001) However, MTX usage was lower in COVID-19+ (n=352(65.3%), 95%CI: 60.98-69.02) in contrast to COVID-19- population (n=332(75.1%), 95%CI: 70.96-79.04). (P =0.001) (Table1) Mean±SD of erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) and lactate dehydrogenase (LDH) were 55.30±34.31, 60.13±30.90 and 703.69±338.42 in COVID-19 patients. Presence of severe COVID-19 infection features was compared between users and non-users of different immunosuppressant agents. (Table 2) Presence of respiratory failure and lung involvement was significantly higher in subjects who used prednisolone. The rate of these complications was significantly lower in MTX and users of biologic agents. According to multivariable analyses, increasing age (OR:1.034,95%CI (1.015-1.053)) and using prednisolone prior to COVID-19infection (OR:2.52,95%CI (1.192-4.255)) were associated with severe disease. Pervious use of MTX (OR: 0.348, 95%CI (0.210-0.575)) and biologic drugs (OR:0.332,95%CI (0.159-0.694)) had protective effect on the severity of the disease. Other traditional DMARDs had no direct effect on the severity of COVID-19 infection. (Table3) Discussion Cytokine storm has been considered as a causative factor in the pathogenesis of COVID-19 infection. Inflammatory cytokines such as IL-6, IL-1, and TNF-α in the infection of COVID-19 are very similar to the cytokines involved in the development of rheumatic diseases.( 4 , 5 ) Considering the background of abnormal immune response and the use of various immunosuppressants in patients with rheumatic diseases, it is necessary to investigate the characteristics of COVID-19 and its consequences in this population.( 15 ) In the current survey among 981 rheumatic patients in Shiraz, Fars, Iran, 539(54.94%) subjects were infected with COVID-19. Use of prednisolone was associated with higher rates of infection and poor outcome. However, MTX and biologic agents had a lower rate of infection, milder clinical features, and better outcome in our study. In previous studies, the risk of COVID-19 infections was considered high in rheumatic patients due to dysregulation of the immune system and use of immunosuppressant agents which increase the risk of opportunistic infections( 16 ). JAK2 inhibitors, anti-TNF agents and glucocorticoids have been considered as the risk factor for the severity of the disease and ARDS in some studies.( 17 )However, this view has been changed with the identification of the role of cytokines and auto-inflammation in the pathogenesis of COVID-19 in recent studies.( 18 ) Hydroxychloroquine, which was first regarded as one of the useful therapies in COVID-19 patients, had no protective effect in our investigation. In Rentschet al.’s study, among 194637 patients with diagnosis of RA and SLE, no statistically significant difference was detected between prior users and non-users of hydroxychloroquine.( 19 )Post-treatment with hydroxychloroquine had also no significant effect on the rate of hospitalization and better clinical outcome.( 20 ) Corticosteroid is defined as an effective treatment in severe COVID-19 infection ( 21 ).However, the effect of the pervious use of these agents on the severity and outcome of COVID-19 is still not clear. In a cohort study done by Ennio Giulio Favallietal., prior use of corticosteroid even in low dose had a potentially adverse outcome (OR ranging from 1.23 to 3.2).( 22 )These findings were compatible with our study results. Based on COVID-19 Global Rheumatology Alliance (GRA), corticosteroids were associated with unfavorable outcome in rheumatoid patients with COVID-19infection. However, disruption of steroid during acute phase of infection was not recommended in this study.( 23 ) Prior use of biologic DMARDS was accompanied with lower rate of COVID-19 infection and better outcome in our study. TNF is the most pro-inflammatory cytokine- mediating immune factors. Anti-TNF antibody agents may be effective in COVID-19 infection by modulation of the immune system and expression of angiotensin converting enzyme 2 (ACE2) receptor.( 24 )It was shown that the risk of COVID-19 infection in patients with rheumatic disease son biologic DMARDs was the same as the general population and was accompanied with favorable outcomes.( 25 ) Prior use of biologic DMARDs was also not associated with higher rate of COVID-19 infection, hospital admission, and mortality rate.( 26 , 27 ) In our study, pre-exposure to MTX was associated with better outcome, lower mortality rate, and the need for invasive ventilation 0.348, 95%CI (0.210–0.575). In addition to anti-inflammatory effect of MTX, this might be due to the effect of MTX on inhibition of dihydrofolate reductase pathway the efficiency of which on infectious disease had been recently discovered.( 24 ) Further clinical trials are needed to determine the exact effect of MTX on COVID-19 infection. In the current study, other traditional DMARDs including mycophenolate mofetil, azathioprine, cyclosporine, cyclophosphamide, leflunomide, and tacrolimus had no effect on COVID-19infection and its severity. Studies on the effect of prior use of this class of drugs on COVID-19 are limited and have yielded conflicting results.( 28 )Larger sample size might be needed for evaluation of their consequence and their mechanism on COVID-19, which is still unknown.( 29 ) This is the first study in the south of Iran that investigated the clinical characteristics of COVID-19 among patients with different types of rheumatoid diseases who used immunosuppressant agents. One of the most important strengths of the current study was the investigation of the effects of different groups of immunosuppressant drugs with various mechanisms on different aspects of COVID-19 infection. Lack of sufficient laboratory data was an important limitation in our study, and further investigation is recommended to determine the level of inflammatory markers in the presence of immunosuppressant agents. Due to the fact that this was a retrospective study, the dosage and duration of the drugs were not known. The sample size in different drug subgroups was also not enough to compare with other subgroups. Conclusion The characteristics, management, and treatment of COVID-19 infection in patients with auto- immune rheumatoid disease who are on immunosuppressant agents are still challenging. In the current study, previous treatment with corticosteroids was associated with increased incidence, poor prognosis, and mortality. In addition, pre-exposure to MTX or biologic agents such as anti-TNF and anti-CD20 had a protective effect against coronavirus. Further meta-analysis and systematic review in larger population is needed to accurately determine the effect and mechanism of these drugs on this new viral illness. The information of the patients was collected through the treatment department of Shiraz University of Medical Sciences (SUMS), the hospitalization centers of COVID-19 patients in Shiraz, and the patients referred to rheumatology clinics. The diagnosis of COVID-19 was based on laboratory-confirmed COVID-19 infection using real-time PCR or lung high resolution CT scan (HRCT) compatible with COVID-19 infection. Men and women aged more than 18 years and the rheumatologic patient on immunosuppressive therapy (prednisolone, methotrexate, azathioprine, mycophenolate, cyclosporine, tacrolimus, cyclophosphamide, rituximab and anti-TNFs) in the three months were included in this survey. The aims of the study were explained to the patients or their relatives in full detail and informed consent was obtained from all subjects or their legal guardians. Inflammatory cytokines, such as interleukin-6(IL-6), IL-1, and tumor necrosis factor alpha (TNF-α) released from the lung epithelium in COVID-19 infection are very similar to cytokines which are released in inflammatory rheumatic diseases. Immunosuppressive drugs are the cornerstone of treatment in many rheumatologic diseases. Prolonged use of these agents might result in change in the clinical characteristics of these individuals with COVID-19.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eObjective\u003c/strong\u003e:\u003c/p\u003e\n\u003cp\u003eTo determine the clinical findings and disease severity of COVID-19 infection in patients with rheumatologic diseases who were on immunosuppressive therapy.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eMethod\u003c/strong\u003e:\u003c/p\u003e\n\u003cp\u003eWe retrospectively investigated 1000 subjects who were more than 18years old with rheumatologic diseases on immunosuppressive therapy in the recent three months. Demographic data, rheumatic disease characteristics, medications, and presence of any co- morbid conditions were documented by detail. Presence of severe COVID-19 infection features were compared between users and non-users of different immunosuppressant agents. Multivariable logistic regression was applied, and adjusted odd ratio (OR) and its 95% CI were estimated.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults\u003c/strong\u003e:\u003c/p\u003e\n\u003cp\u003e539 subjects had positive history of COVID-19 (COVID-19+). Pre-exposure to prednisolone was associated with severe disease (OR: 2.52, 95%CI (1.192-4.255)). Pervious use of MTX (OR: 0.348, 95%CI (0.210-0.575)) and biologic drugs (OR: 0.332, 95%CI (0.159-0.694)) had protective effect on the severity of the disease. Other traditional disease modifying anti-rheumatic drugs (DMARD) had no clinical effect on the outcome of COVID-19 infection.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eConclusion\u003c/strong\u003e:\u003c/p\u003e\n\u003cp\u003eIn the current study, among rheumatoid patients on immunosuppressants, previous treatment with corticosteroids was associated with increased incidence, poor prognosis, and increased mortality. Pre-exposure to MTX or biologic agents such as anti-TNF and anti-CD20 had protective effect against coronavirus.
