A Case of Malignant Transformation Arising in Rectal Endometriosis with Liver Metastasis

Abstract
腸管子宮内膜症癌化は極めて稀であり，本邦において遠隔転移巣の同時切除の報告はない．今回われわれは，腺癌の術前診断のもと肝転移を含めた直腸切除を行ったところ，最終的に組織学的に直腸子宮内膜症の癌化と診断された症例を経験したので報告する．症例は，53歳の女性．左乳癌術後経過観察目的のCTで肝S6，S8に転移を疑う腫瘤性病変を認め，肝生検で腺癌が検出され，下部消化管内視鏡検査で認めた直腸S状部の1型腫瘍も生検で腺癌と診断された．免疫組織学的染色の結果，肝腫瘍は乳癌転移としては否定的で，直腸癌の肝転移でない可能性が示唆されたが，診断・治療目的に直腸高位前方切除術，肝S6，S8部分切除術を行った．病理組織学的に，腸管子宮内膜症から発生した漿液性腺癌の肝転移の診断となった．術後補助化学療法としてTC療法を施行し，現在6カ月無再発生存中である．
