Abnormal Uterine Bleeding Is Associated With Increased BMP7 Expression in Human Endometrium

This study investigated whether bone morphogenetic protein (BMP) signaling is dysregulated in women with abnormal uterine bleeding (AUB) by screening 489 endometrial samples from 365 patients for expression of BMP2/4/6/7 ligands, BMPR1A and BMPR1B receptors, and downstream SMAD4/6/7, with expression correlated to clinical record data (including age, BMI, hormone treatment, and histology such as fibroids, polyps, adenomyosis, hyperplasia, and cancer). The key finding was that BMP7 ligand expression was significantly increased in AUB overall and was higher in patients whose AUB pattern involved heavy menstrual bleeding/menorrhagia, while no other BMP ligands, receptors, or SMAD proteins differed across the cohort. A major limitation is that the work is observational/expression-based, identifying associations without establishing causality, and it noted additional differences only for cancer versus benign regarding BMPR1A/B and SMAD4.
