Abstract

子宮内膜症に合併した血胸の症例を経験したので報告する．症例は45歳女性．右下腹部痛にて当院を受診した．月経開始後5日目であった．腹部CTにて多量の腹水と子宮右側に造影される腫瘤を認めた．受診後貧血の進行を認め，右卵巣出血の疑いで開腹手術を行った．骨盤内には子宮内膜症を認め，腹腔内には約1500mlの血液貯留を認めた．両側卵巣卵管摘出術を行った．術後の胸部X線写真で右肺野の透過性不良を認め胸水貯留と診断した．胸腔ドレーンを挿入し，約1300mlの血性胸水を排液した．翌日，血胸の原因の診断目的で胸腔鏡下手術を行った．臓側および壁側胸膜と横隔膜に多数の茶色の点状班を認めた．また横隔膜の腱中心に紫色の斑点と複数の小孔を認めた．横隔膜と右肺下葉を部分切除した．横隔膜の組織所見にて異所性子宮内膜症と診断した．
