OP07.06: 3D junctional zone indices: relationship to heavy menstrual bleeding and dysmenorrhea of adenomyosis

Abstract

To determine relationship of junctional zone (JZ) indices among women with adenomyosis with symptoms of heavy menstrual bleeding and dysmenorrhea versus those women with adenomyosis but without symptoms. This is a prospective cross sectional analytical study including women with adenomyosis. 3D-transvaginal ultrasound was carried out recording JZ indices; maximum junctional zone (JZmax), minimum junctional zone (JZmin), JZ difference, maximum myometrial thickness, JZ ratio and observation of JZ alterations (distortion and infiltration of the myometrium and presence or absence ill defined JZ) in subjects with and without symptoms of adenomyosis. At 3D ultrasound assessment, women with symptoms of adenomyosis had significantly higher values of JZmax (mean 13.3 +/- 1.35, P < .001), JZdiff mean (8.98 +/- 1.80, P < 0.001), maximum myometrial thickness mean (32.46 +/- 9.53, P < .020) and JZ ratio (44.2% +/- 12.5%, P /= 8 mm, JZ diff >/= 4 mm, JZ Ratio > 50% based on previous studies by Exacoustous et al and the presence of an ill-defined JZ were significantly observed in the group with symptoms. Subjects with symptoms had significantly higher JZ indices and were observed to have ill-defined JZ compared to subjects without symptoms. Because the 3D coronal section of the uterus permits accurate evaluation and measurement of the JZ and its indices, it can be invaluable tool and adjunct to 2D-TVS, not only in the diagnosis of adenomyosis but in staging the severity of the disease. Once clear criteria have been established for such staging, clinicians can set up proper protocols for managing the disease and monitoring various treatment regimens for adenomyosis.
