Methods A cross-sectional study was conducted with 39 HCPs in Pécs, Hungary. Burnout was measured using the MBI and MOLBI scales. Patient complexity was assessed via the Charlson Comorbidity Index (CCI) and the Karnofsky Performance Scale (KPS). Results Patients treated in the center-based unit had a significantly higher comorbidity burden (CCI: 6.62 vs. 5.55, p = 0.003) and poorer functional status (KPS: 67.9 vs. 85.7, p < 0.001) compared with those treated in the satellite unit. HCPs working in the center-based unit reported significantly higher burnout levels than those in the satellite unit, as reflected by MBI total scores (62.8 ± 11.5 vs. 52.8 ± 14.3, p = 0.023), primarily driven by emotional exhaustion. MOLBI-measured exhaustion showed a significant positive correlation with patient CCI scores ( r = 0.426, p = 0.028). In exploratory regression analysis, dialysis unit type emerged as an independent predictor of burnout severity ( p = 0.022). Conclusion Higher patient clinical and functional complexity in center-based units is directly linked to increased HCP burnout. Effective prevention strategies include workload adjustments based on patient acuity, staff rotation, and providing psychological support. Burnout syndrome Hemodialysis Nursing management Patient comorbidity Center-based dialysis Satellite unit Figures Figure 1 Figure 2 Figure 3 Figure 4 Introduction Burnout syndrome is a significant occupational health issue among healthcare professionals, especially in specialties characterized by high workloads, substantial emotional demands, and prolonged patient care responsibilities [ 1 , 2 ]. It is defined by emotional exhaustion, depersonalization, and diminished personal accomplishment, and is linked to decreased job performance, increased incidence of medical errors, and higher staff turnover rates [ 3 – 5 ]. Dialysis care represents a particularly demanding work environment for nurses. Healthcare professionals working in hemodialysis units are exposed to time pressure, repetitive high-risk procedures, advanced technology, and sustained relationships with chronically ill patients with end-stage renal disease (ESRD) [ 6 – 8 ]. Previous studies have consistently shown a high prevalence of burnout among dialysis staff, with reported rates ranging from 32% to over 40%, and emotional exhaustion as the dominant dimension [ 1 , 4 , 9 ]. In addition to individual susceptibility, organizational and work-related factors significantly contribute to burnout [ 10 , 11 ]. Dialysis services are provided in various treatment settings. Center-based units generally care for patients with greater clinical complexity, higher comorbidity burdens, and increased functional dependency, whereas satellite units typically serve more stable patients [ 12 ]. Although these differences in patient case mix are well established, there is limited evidence regarding the impact of dialysis unit type on the severity of burnout among healthcare professionals. A small number of studies, including an Australian pilot study, have indicated higher perceived stress levels among staff in center-based units [ 13 ]. Objective indicators of patient clinical complexity may provide valuable insights into staff workload and occupational stress. The Charlson Comorbidity Index (CCI) and Karnofsky Performance Scale (KPS) are validated tools widely used to assess comorbidity burden and functional status, respectively [ 14 , 15 ]. However, their relationship with burnout among dialysis healthcare professionals has not been thoroughly investigated. In Hungary, healthcare systems face workforce shortages, an aging staff population, and high patient-to-staff ratios, all of which may increase vulnerability to burnout [ 16 – 18 ]. While burnout among Hungarian healthcare professionals has been documented, data specifically focusing on dialysis personnel are scarce. [ 9 , 19 ]. Therefore, this pilot study aimed to compare the burnout levels of healthcare professionals working in center-based and satellite dialysis units and explore the association between burnout and objective measures of patient clinical complexity. Methods Study design and setting This descriptive, cross-sectional pilot study was conducted in two dialysis units in Pécs, Hungary: a high-acuity center-based and a satellite dialysis unit. This study aimed to examine the association between the workplace environment, patient clinical complexity, and burnout among dialysis healthcare professionals (HCPs), in line with previous occupational health research in nephrology settings [ 1 , 4 , 11 ]. Participants and sampling A total of 39 healthcare professionals participated in the study using convenience sampling. All staff members from the two dialysis units were invited to participate voluntarily and anonymously. The inclusion criteria were employment in the dialysis unit for at least three months, age ≥ 18 years, and approval by the healthcare provider. The exclusion criteria were employment for less than three months and the presence of malignant disease or chronic inflammatory conditions, as these factors may influence psychological well-being and somatic symptoms [ 19 , 25 ]. Instruments for staff assessment Burnout was assessed using the Maslach Burnout Inventory (MBI), the most widely used and validated instrument for measuring burnout among healthcare professionals [ 26 , 30 ]. The MBI evaluates three dimensions–emotional exhaustion, depersonalization, and reduced personal accomplishment–using 22 items rated on a 7-point Likert scale. Higher scores indicate more severe burnout. The Mini Oldenburg Burnout Inventory (MOLBI) was also administered to assess burnout, focusing on exhaustion and disengagement as the core dimensions of the syndrome [ 16 ]. The MOLBI comprises 10 items rated on a 4-point Likert scale and has demonstrated strong psychometric properties in occupational health studies. Depressive symptoms were measured using the Beck Depression Inventory (BDI), a validated self-report questionnaire widely used to assess the severity of depressive symptoms in clinical and nonclinical populations [ 30 ]. Cognitive vulnerability was assessed using the Dysfunctional Attitude Scale – Short Form (DAS-SFI), which evaluates maladaptive beliefs associated with depression and stress-related disorders [ 23 ]. Workplace psychosocial factors were assessed using the Hungarian-adapted version of Caldwell et al.’s Social Support Questionnaire, known as the Support Dimension Scale (SDS), which has demonstrated satisfactory reliability in Hungarian healthcare populations [ 26 ]. The internal consistency of the instruments in the present sample was acceptable, with Cronbach’s alpha coefficients comparable to those reported in previous studies [ 16 , 26 , 30 ]. Assessment of patient clinical complexity The clinical status of the patients was assessed using objective indicators to quantify the workload related to patient care. The comorbidity burden was measured using the Charlson Comorbidity Index (CCI), a validated prognostic tool widely used in chronic disease and dialysis populations [ 12 ]. Functional status and physical dependency were assessed using the Karnofsky Performance Scale (KPS), which rates patient function on a scale of 0–100 [ 13 ]. Both indices have been previously applied in nephrology research to characterize patient severity and care demands [ 18 , 22 ]. Ethical Considerations This study was conducted in accordance with the Declaration of Helsinki. All participants provided informed consent. Ethical approval for our study was granted by the Local Ethical Board of the University of Pécs (No: 9938-PTE 2024), dated Dec 01, 2024. Statistical Analysis Data were analyzed using SPSS Statistics (v.26). Continuous variables were assessed for normality using the Shapiro–Wilk test and Q–Q plots. For group comparisons, independent-samples t-tests were used for normally distributed variables, and Mann–Whitney U tests for non-normally distributed variables. Categorical variables were compared using the χ2 test or Fisher’s exact test when cell counts were small. Associations were examined using Pearson’s correlation for normally distributed variables and Spearman’s correlation for non-normally distributed variables (e.g., CCI, KPS vs. MBI/MOLBI subscales). Exploratory stepwise linear regression analyses were performed to identify independent predictors of burnout severity, with the MBI total score as the dependent variable. The candidate independent variables included workplace type, sociodemographic and work-related factors (age, marital status, number of children, work schedule), BDI, DAS-SFI, and somatic complaints. Model fit was reported using R2 and adjusted R2 values. Multicollinearity was assessed using the variance inflation factor (VIF) and tolerance. Missing data were infrequent and were handled using listwise deletion. All tests were two-sided, and p < 0.05 was considered statistically significant. Due to the pilot study's sample size (N = 39), the analyses were exploratory in nature, and effect sizes (Cohen’s d, r, or Cramér’s V) were provided where relevant to assist with interpretation. Results Patient profiles by dialysis setting This study showed that there was a significant difference in how patients responded to treatment between the center-based and satellite dialysis units. As demonstrated in Table 1 and Figure 1, patients receiving treatment at the center-based facility had a higher comorbid disease burden, as reflected by higher Charlson Comorbidity Index (CCI) scores (6.62 vs. 5.55, p = 0.003). In addition, functional status was lower in patients receiving treatment at the center-based facility, as reflected by lower Karnofsky Performance Scale (KPS) scores (67.9 vs. 85.7, p < 0.001), indicating a higher degree of physical dependency. Study population characteristics The study included 39 healthcare professionals (HCPs) who responded to the survey, representing a 97.5% response rate. Table 1 describes the respondents’sociodemographic and occupational characteristics. The majority of the respondents were women (79.5%), with an average age of 48.7 years. In addition, 14 respondents (35.9%) worked in the center-based unit, while 25 respondents (64.1%) worked in the satellite units. More than 45% of the participants had over two decades of experience in healthcare. Prevalence and severity of burnout Severe burnout was found in 25.6% of the sample, and the prevalence of burnout was high (89.7 %). Severe burnout was found more often in the center-based unit than in the satellite unit, and the difference was significant (p = 0.011, Figure 2). In the satellite unit, severe burnout was present in only 12.0% of the respondents, while in the center-based unit, severe burnout was present in 50.0% of the respondents. According to MBI and BDI tests, more severe burnout occurred among the workers of the center compared to those of the satellite unit (50.0 vs. 12.0%, p = 0.011). Regarding all the employees, burnout was associated with single marital status (p = 0.012), having no child or one child (p = 0.014, p = 0.031), continuous work schedule (p = 0.042), moderate or severe dysfunctional attitudes (p = 0.020, p = 0.010), and severe depression (p = 0.013; Supplementary material Table S2). The center-based group had significantly higher mean total scores on the Maslach Burnout Inventory than the satellite group (p = 0.023, Table 3, Figure 4). Subscale analysis showed a significant difference in emotional exhaustion and depersonalization, with the highest scores for the center-based group, while the scores for the satellite group were much lower and not significant. No significant difference was found in the subscale for personal accomplishment (Figure 3). Burnout measured by the Mini Oldenburg Burnout Inventory The Mini Oldenburg Burnout Inventory showed a burnout prevalence of 71.8%. No significant differences were found between the center-based and satellite units for the total score of the Mini Oldenburg Burnout Inventory, the subscale for exhaustion, and the subscale for disengagement (Table 3). Psychological distress and somatic complaints Burnout levels were closely related to psychological strain and physical symptoms. As illustrated in Table 2 and Supplementary Table S1, staff in the center-based unit reported more depressive symptoms and more dysfunctional attitudes. Physical problems were also more prevalent. Musculoskeletal pain and sleep problems were particularly prominent among the staff in the center-based unit (p = 0.015 and p = 0.021, respectively; Figure 3). SDS questionnaire scores of the employees showed that burnout was associated with emotional exhaustion (66.7% vs. 0.0%, p = 0.048), mental exhaustion (60.0% vs. 0.0%, p = 0.023), self-esteem improvement (60.0% vs. 25.0%, p = 0.037), and doing nothing to reduce exhaustion (60.0% vs. 25.0%, p = 0.015). Both were higher in workers of the center (Supplementary material, Table S3). Link between patient severity and staff burnout The correlation test showed a meaningful relationship between the severity of the patient's illness and staff burnout. A higher patient comorbidity index (CCI) was positively related to the staff's level of exhaustion measured by the MOLBI (r = 0.426, p = 0.028; Table 4). In contrast, a poorer functional status (lower KPS) was related to the staff's lower level of personal accomplishment (Figure 4). Predictors of the intensity of staff burnout Table 4 displays the relationship between the categories of staff burnout and sociodemographic and professional characteristics. Some interesting findings emerged regarding the type of workplace, marital status, number of children, type of work schedule, dysfunctional attitudes, and depression. In the stepwise linear regression analysis (Table 5), the type of dialysis unit to which the staff belonged (a center-based unit vs. a satellite unit) was an independent predictor of the intensity of staff burnout (β = 0.38, p = 0.022). Other predictors included musculoskeletal pain, depression, and dysfunctional attitudes. It should be noted that the regression models were somewhat unstable because of the pilot study design and small sample size. Discussion This preliminary study explored how the work environment in dialysis units, the clinical complexity of patients, and professional burnout are interconnected among staff at both a center-based dialysis unit and a satellite dialysis unit. The study's primary finding was that staff at the center-based unit experienced significantly higher burnout levels than those at the satellite unit. This disparity was primarily attributed to greater emotional exhaustion. These results align with other research indicating that emotional exhaustion is a key component of professional burnout among dialysis staff [ 18 – 20 ]. The difference in the degree of professional burnout in the two dialysis units was closely related to the differences in patient characteristics between the two units. Patients treated in the center-based dialysis unit showed significantly higher comorbidity and poorer functional status than those in the satellite dialysis unit. This study supports the findings of other studies, indicating that dialysis units with more complex and functionally dependent patients place a higher workload on the staff in terms of emotional and physical strain [ 21 – 23 ]. Notably, the study findings indicate that the workload in dialysis units cannot be measured solely by the number of patients. These findings are consistent with previous studies, which have highlighted the high rate of burnout in the healthcare workforce caring for patients on dialysis, which is far greater than that in many other medical specialties [ 18 , 20 , 26 ]. Although international studies on burnout in the workforce caring for patients on dialysis report a range of 32% to 42% [ 27 – 29 ], the rate of burnout in the present study was much higher. This might be partly explained by the difficulties and problems in the Hungarian healthcare system, workforce shortages, high patient-staff ratios, and lack of resources [ 30 – 32 ]. One of the key discoveries of this study was the positive link between the patients' comorbidity burden and the burnout levels among healthcare professionals. The study identified a positive correlation between the Charlson Comorbidity Index and exhaustion levels, as assessed by the Mini Oldenburg Burnout Inventory. This result aligns with occupational stress theories, which propose that high job demands without sufficient resources can lead to burnout by depleting energy reserves [ 33 , 34 ]. For healthcare workers caring for dialysis patients, the demands posed by these patients may be a significant contributing factor. Consistent with earlier studies, the patient's functional status, as measured by the Karnofsky Performance Scale, was linked to personal achievement. Providing care for individuals with significant dependency needs may restrict opportunities for positive feedback and the perception of successful treatment, both of which are recognized as factors that can mitigate burnout [ 35 , 36 ]. These results underscore the necessity of taking into account the patient's functional status when considering staff well-being in dialysis centers [ 22 ]. Burnout was not an isolated phenomenon in this study but was closely related to depression symptoms, dysfunctional attitudes, and physical complaints, particularly musculoskeletal pain and sleep issues. These findings are consistent with previous research that has demonstrated the close interrelation between burnout, depression, and physical health complaints among healthcare staff [ 27 , 31 , 37 ]. In dialysis facilities, standing for long periods, repetitive work activities, and physical assistance to dependent patients may all contribute to musculoskeletal problems and, thereby, to the vicious cycle of physical and emotional exhaustion [ 23 , 32 ]. Sociodemographic and occupational factors also influence the severity of burnout. In line with previous research, higher levels of burnout were more commonly found among staff with continuous work schedules, unstable social support, and fewer children [ 28 , 29 , 33 ]. Of interest was the finding that the number of years of working in healthcare was not an independent predictor of burnout severity, suggesting that it is not merely the fact of having worked for a long time that influences burnout, but rather other factors [ 33 , 37 ]. In the regression model, the type of dialysis unit was an independent predictor of burnout severity, even when psychological and physical factors were controlled for. This finding highlights the importance of organizational factors in the development of burnout and supports recommendations for interventions at this level [ 35 , 38 ]. Potential interventions include matching staffing levels to patient acuity, alternating work between dialysis centers and satellite units, and providing psychological support and supervision for standard dialysis treatment [ 35 , 38 ]. Study limitations Several limitations should be considered. The experimental design and relatively small sample size limit generalizability and may contribute to the instability of the regression models [ 35 ]. The cross-sectional nature of the study precludes causal inference, and self-reported measures may be subject to response biases. However, the use of objective measures of patients’ clinical complexity is an important strength of this study [ 24 ]. Conclusion In conclusion, the findings of this pilot study suggest that burnout among dialysis healthcare professionals is strongly influenced by the workplace environment and patient case mix. Among patients treated in dialysis units in centers, higher comorbidity burden and functional dependency were associated with increased burnout severity among staff. The findings highlight that burnout in nephrology extends beyond being a mere personal psychological issue, reflecting the impact of organizational structure and workload allocation. To validate these results and assess specific interventions aimed at preserving staff well-being and ensuring the sustainability of high-quality dialysis care, larger, multicenter longitudinal studies are necessary. Burnout was measured using the MBI and MOLBI scales. Patient complexity was assessed via the Charlson Comorbidity Index (CCI) and the Karnofsky Performance Scale (KPS).\u003c/p\u003e\u003ch2\u003eResults\u003c/h2\u003e \u003cp\u003ePatients treated in the center-based unit had a significantly higher comorbidity burden (CCI: 6.62 vs. 5.55, \u003cem\u003ep\u003c/em\u003e\u0026thinsp;=\u0026thinsp;0.003) and poorer functional status (KPS: 67.9 vs. 85.7, \u003cem\u003ep\u003c/em\u003e\u0026thinsp;\u0026lt;\u0026thinsp;0.001) compared with those treated in the satellite unit. HCPs working in the center-based unit reported significantly higher burnout levels than those in the satellite unit, as reflected by MBI total scores (62.8\u0026thinsp;\u0026plusmn;\u0026thinsp;11.5 vs. 52.8\u0026thinsp;\u0026plusmn;\u0026thinsp;14.3, \u003cem\u003ep\u003c/em\u003e\u0026thinsp;=\u0026thinsp;0.023), primarily driven by emotional exhaustion. MOLBI-measured exhaustion showed a significant positive correlation with patient CCI scores (\u003cem\u003er\u003c/em\u003e\u0026thinsp;=\u0026thinsp;0.426, \u003cem\u003ep\u003c/em\u003e\u0026thinsp;=\u0026thinsp;0.028).
