Factors Predicting Physical Deterioration in Covid-19 Sepsis Patients at a Bangkok Tertiary Hospital

Abstract
Background :The coronavirus disease 2019 (COVID-19) is a pandemic and spread worldwide. Occurrence of deteriorating symptoms leading to death and factors predicting physical deterioration in COVID-19 patients with sepsis are unclear. This has sparked the researcher’s interest in studying how age, comorbidities, BMI, blood glucose level, and smoking history can predict altered physical deterioration in COVID-19 patients with sepsis in Thailand to continue providing care and monitoring patients in this population.Objective: This study aimed to analyze the predictive factors, age, comorbidities, BMI, blood glucose level, and smoking history, to copredict physical deterioration in COVID-19 patients with sepsis.Design: This research is a cross sectional predictive design.Setting ( s ): A tertiary hospital in Bangkok.Participants : The study population consisted of 147 males and females patients aged ≥18 years diagnosed with COVID-19 and symptoms of sepsis and admitted for treatment in a tertiary hospital.Methods : Patients were selected according to the criteria of having two of four presenting symptoms of systemic inflammatory response syndrome (SIRS), diagnosed with sepsis and COVID-19, having Emergency Severity Index (ESI) 1 to 3, diagnosed by or referred from other hospitals due to sepsis and COVID-19, and treated at tertiary hospital for >4 h and treated continuously for 7 days. Patient exclusion criteria included injury in any accident, cardiopulmonary resuscitation, pregnancy, plans to be treated by emergency surgery, referred to be treated at other hospitals, and unstable symptoms when admitted to the emergency department. Data were collected from February to April 2022.Results : Of the 147 patients the findings showed that most patients were female (51%) with the average age of 71.72 years old. Age, comorbidities, body mass index and smoking history were found to predict physical deterioration in COVID-19 patients with sepsis (42.5%) with statistically significant at 0.05 (R2 = 0.425). Comorbidities were mostly likely to predict the severity of physical deterioration in COVID-19 patients with sepsis at 10.45 times (OR=10.45; 95%Cl=3.40, 32.13, p < 0.001). Blood glucose was unable to copredict physical deterioration.Conclusions : Predictive factors in COVID-19 patients with sepsis can be designed to reduce the incidence of physical deterioration. Therefore, nurses should assess and enquire age, comorbidities, body mass index and smoking history.
